What you need to summon Duke Fishron
- A Truffle Worm, a rare critter found in an Underground Glowing Mushroom biome during Hardmode.
- A Bug Net, Lavaproof Bug Net, or Golden Bug Net to catch it.
- A fishing pole and access to the Ocean.
The Truffle Worm tends to burrow away when approached, so move quickly once you spot one. It is consumed when Duke Fishron is summoned, even if you have an item that reduces bait consumption. Official Terraria Wiki: Truffle Worm

How to spawn Duke Fishron
- Catch a Truffle Worm. Find one in an Underground Glowing Mushroom biome during Hardmode and capture it with one of the listed nets.
- Put it first in your bait order. Place the Truffle Worm in the top-left bait slot. Bait is used from top-left to bottom-right, so putting the worm first makes it the bait used for this cast. Official Terraria Wiki: Fishing
- Fish at the Ocean. Cast your line with the Truffle Worm equipped as bait.
- Reel in when the bobber bobs. Duke Fishron will emerge after the catch; he pauses briefly, then begins attacking. He is invulnerable during the summoning animation. Official Terraria Wiki: Duke Fishron
Why Duke Fishron might not spawn
Check your bait and its order
Confirm that the Truffle Worm is in the bait inventory and at the top-left of the bait order. If it is behind other bait, a different item may be used first. If you want to fish normally without risking the worm, put it in the last bait slot instead.
Check the location and whether the boss is already active
You must fish in the Ocean, and Duke Fishron must not already be alive in the world. No earlier boss defeat is required to summon him. Official Terraria Wiki: Duke Fishron

Review pool and world-edge conditions for unusual setups
If you are fishing from a custom arena or a small pool, check the current version’s summon conditions: the wiki lists minimum liquid-pool sizes and a bobber-distance limit from the true world edge. It says the bobber can be at any height; the listed threshold differs for honey versus water or lava. These implementation details may change between updates, so consult the current entry if a standard Ocean cast works but an unusual setup does not. Official Terraria Wiki: Duke Fishron

No. Duke Fishron has no boss prerequisite and is optional for progression, so you can attempt him as soon as you can catch and use a Truffle Worm. He is a serious threat to an unprepared player, and the fight starts shortly after the summon.
