Equip a grenade in the Pip-Boy, then hold the grenade control to aim and release it to throw. The default controls are Left Alt on PC, RB on Xbox, and R1 on PlayStation; the same control also performs a bash or power attack when tapped.
Equip a grenade first
- Open the Pip-Boy and go to Inventory.
- Open the Weapons section, select the grenade you want, and equip it.
- Close the Pip-Boy to return to gameplay.
The Destructoid guide, updated September 8, 2024, describes selecting and equipping a grenade before throwing it.

Hold the grenade control, aim, and release
Hold the platform’s grenade control to ready the throw, aim with the reticle, then release the control. Bethesda’s manuals label this a shared Bash/Power Attack/Grenade action: the Xbox manual specifies holding the control for a grenade, and the PC manual assigns the action to Left Alt.

Why does my character bash instead of throwing?
The grenade action shares a control with bash and power attack. Tapping the control can therefore trigger a bash instead of readying a grenade. Make sure a grenade is equipped, then hold the control and release it after aiming.

If holding the default button still does not throw a grenade, check the in-game control settings: you may have remapped the action, or your edition’s controls may differ. The defaults above are documented for PC and Xbox One; the PlayStation R1 reference comes from the PS4 manual’s search excerpt.

Destructoid’s guide reports that a held grenade has no timer and does not explode in the player’s hand. That detail is from the guide rather than Bethesda’s manual control descriptions.
