But “not open-world” does not mean there is no roaming: the current game has a separate Free Ride mode with optional activities and places to explore.
How the campaign is structured
2K describes Mafia: The Old Country as a story-led Mafia origin entry set in 1900s Sicily. You follow Enzo Favara, a former child laborer in the sulfur mines who joins Don Torrisi’s crime family. Hangar 13 president Nick Baynes described the design as “a focused, linear experience” built around storytelling and period immersion in Take-Two’s May 8, 2025 announcement.

That describes the campaign’s structure: the story leads you through a directed sequence, rather than making an open world the main loop. It does not mean every setting is a corridor. PlayStation’s game description includes varied Sicilian environments and travel by horseback and early automobiles, alongside stealth, knife fights, and gunplay.
What you can do in Free Ride
Free Ride is the separate option for more open-ended play. The current PlayStation listing describes activities and extras available through the mode:

These activities give you reasons to roam and replay challenges, but they do not turn the story campaign into an open-world structure. For players asking what remains beyond the story missions, Free Ride is the relevant answer: optional activities and exploration, not a second open-world campaign.
What the Man of Honor expansion adds
The Man of Honor expansion adds two chapters to Enzo’s story, set in winter 1905 Sicily, where he works alongside Ennio Salieri. It also adds Free Ride challenges and jobs, including races, combat, and stealth encounters, as well as weapons, outfits, vehicles, and horses. 2K lists the expansion for PS5, Xbox Series X|S, and PC via Steam in its 2026 launch announcement.

The useful distinction is not whether a game has places you can explore; it is how the main story and roaming fit together.
