There are a few limited downsides to accepting the Brand of the Absolute in Baldur’s Gate 3: certain companions give one-time disapproval, and the brand can lead to an adverse dialogue interaction. In exchange, it unlocks situational dialogue and benefits from several pieces of equipment. The BG3 Wiki does not list accepting the brand as forcing an evil route, but it does not provide an exhaustive audit of every story flag or patch-specific effect.
What changes if you accept the brand?
The brand is an optional, permanent bonus offered by Priestess Gut in Act One. According to the BG3 Wiki’s Brand of the Absolute entry, accepting it adds a “Branded” tag to your character’s Detailed View, enables certain dialogue options, and causes one-time -1 disapproval from certain companions.
The wiki does not list a forced evil route as a consequence. That is not a guarantee about every possible story interaction: the available mechanics pages do not establish that every companion response or story flag has been exhaustively checked.
What are the downsides?
Some companions disapprove once
The documented approval cost is a one-time -1 from certain companions, not a recurring penalty. The cited wiki entry does not identify every affected companion in the material summarized here, so do not assume the reaction is identical across the whole party.

Some dialogue can go against you
The brand does not make every Absolute-aligned character friendly. For example, Brakkal recognizes a branded character and refuses to talk to them. Other interactions can be advantageous instead: the wiki says Fezzerk leaves Barcus Wroot alone, and describes situations where Gekh Coal, Corsair Greymon, or Lump and the other ogres may back down. These are specific dialogue outcomes, not a general ability to persuade cult members.
The tag may fail to appear in some patch-specific cases
The wiki flags a possible Patch 7 issue in which an attempted branding may not add the “Branded” tag. Check the character’s Detailed View after the interaction to confirm whether it succeeded; dialogue and equipment effects tied to the tag may not work as expected if it is missing. The wiki is community-maintained, rather than official Larian documentation, and behavior may be patch-sensitive.

What do you gain from it?
The tag opens situational dialogue, and some equipment has benefits for branded characters. The BG3 Wiki’s Absolute set page lists Absolute’s Protector, Absolute’s Talisman, Absolute’s Warboard, Gloves of Power, and Ring of Absolute Force among the relevant items. For example, Absolute’s Protector reduces spell damage received from the Absolute by one point, while Absolute’s Talisman grants a branded wearer advantage on death saving throws.
Do not treat every item in that list as unusable without the brand. The wiki specifically says Ring of Absolute Force functions fully without it.

Absolute’s Talisman as a concrete example
Absolute’s Talisman, worn by Priestess Gut in the Shattered Sanctum, can cast a self-only version of Aid and grants advantage on death saving throws to a branded wearer. It occupies the amulet slot, and its Aid effect is removed if you unequip it. The brand-related benefit is therefore most relevant if you plan to wear the talisman, rather than simply acquire it.

A party member can receive the brand by speaking with Priestess Gut in the Shattered Sanctum. If Gut is unavailable after the conversation, Warlock Greez can brand party members, according to the wiki entry.

Do not confuse accepting the brand with following Gut to the adjacent chapel. The Priestess Gut entry describes a separate interaction: complying with her request there leads to a potion that can result in imprisonment. That outcome belongs to the chapel encounter choice, not to the act of taking the brand by itself.
Should you accept it?
For a character who can tolerate a small, one-time approval hit and may use branded gear, accepting the brand has clear mechanical upsides. If preserving companion approval or role-play consistency matters more than those benefits, declining is reasonable. Either way, the documented choice is optional; the wiki lists situational costs and benefits, not a required commitment to an evil playthrough.
