Missable counts depend on what a guide means by “missable.” PowerPyx identifies two campaign-spanning risks: Lent and Been There, Done That. XboxAchievements.com describes a broader set of 17 missables, including Déjà Vu during the combat tutorial and several quest outcomes. The difference reflects counting criteria: some goals are only available during an opening or quest window, while some quest-related trophies can be revisited using permanent saves at the start of main quests, as PowerPyx describes. Lent: set the restriction from the opening
PowerPyx advises avoiding meat and animal kills from the beginning for Lent. Treat this as a run-long restriction: do not wait until the achievement is near the top of your checklist to change habits. If you are unsure whether an action counts, consult your platform’s current guide before taking it.
Been There, Done That: make regional content part of the route
This achievement concerns completing most of the game’s content, so avoid treating the main quest as the only progress that matters. Finish available regional side quests and tasks before advancing major story points, and read quest objectives carefully before triggering the next stage. Quest-window goals and recovery saves
Some achievements are tied to a short opening or a particular quest decision. PowerPyx says the game retains a permanent save at the start of each main quest, which can make many quest-related goals recoverable. Before assuming a missed objective is permanently lost, check whether your game exposes the relevant quest-start save and whether that save predates the required event.
A practical route for protecting completion

- While a region is active: clear its available side quests and tasks before pushing the main story into a major transition. Check objectives for indications that proceeding will move the quest forward.

- Before starting a DLC opening: make another named manual save, particularly if you are also maintaining a broad base-game completion route.
- After a suspected miss: inspect your in-game save options for the permanent start-of-main-quest save described by PowerPyx, then compare it with the specific platform checklist. Confirm current behavior in your own game, since patches and save behavior may change.

Does difficulty affect the platinum, and how should DLC fit in?
PowerPyx says difficulty does not affect the base-game PlayStation platinum. The same guide notes that Hardcore Mode was added after launch and has its own DLC trophies, and recommends handling those in a separate Hardcore playthrough. The KCD2.org roadmap likewise places its pair of Hardcore achievements in a run with every negative perk enabled at character creation.
