Build Attunement by killing monsters and completing events to improve the loot awarded after the boss, and defeat Afflicted demons to add time to the run. Tributes let you target reward types for a future run, but stronger Tributes also make that run more challenging.
How a Kurast Undercity run works
The activity is a sequence of dungeon stages under a rapidly depleting timer. Fight through each area, build Attunement, and reach the final boss before time runs out. Blizzard’s Undercity overview describes the core loop and its timer, Attunement, and Afflicted demons.

Manage the timer
Defeating Afflicted demons extends the timer, giving your group more time to reach the boss. Prioritize those enemies when you encounter them rather than treating every fight as a race past all optional targets.

Killing monsters and completing events grants Attunement. The amount you earn affects the quality of loot after the boss, so use the time available to build it while continuing toward the end of the run.

What Tributes do
Tributes are keys earned throughout Sanctuary and inside the Undercity. Using one augments a future Undercity run to target a particular kind of reward. Blizzard says higher-quality Tributes offer greater spoils but make the next run considerably more challenging; the official feature overview explains that trade-off.
Choose a stronger Tribute when you are prepared to take on the added difficulty. The available official information here does not establish exact payouts for every Tribute, so treat the reward target as a way to shape a run—not as a guarantee of a particular item.

Runes and other documented reward changes
Blizzard’s July 1, 2025 patch notes for version 2.3.0 documented several Undercity reward changes. These are dated patch-note details, not a complete current reward table.

- Every Undercity run drops at least one Rune.
- The Runes Tribute rewards more Runes per Attunement, with rewards scaling by Torment Level.
- The Tribute of Titans’ drop rate was reduced from Lair Bosses and increased from randomized Tribute sources. In Torment Tiers, randomized Tribute drops had equal chances to be Tribute of Harmony or Tribute of Titans.
- Tribute of Harmony could infrequently drop from random Tribute sources, while its Nightmare Dungeon drop rate was slightly reduced.
These changes are recorded in Blizzard’s Diablo IV Patch Notes (2.3), under the July 1, 2025 build 2.3.0 notes. Blizzard’s latest listed patch-notes entry is build 3.2.2, dated September 30, 2026; it does not announce an Undercity mechanic change. Access and expansion requirement
Kurast Undercity is part of the Vessel of Hatred expansion. Blizzard’s launch overview lists Xbox, PlayStation, Battle.net, and Steam as platforms for the expansion. Because that is launch-era information, confirm current platform and edition availability with the relevant store before buying. The official information cited here does not establish a complete current unlock walkthrough.
