Spoilers for Mafia: The Old Country’s ending. The ending does not confirm a direct sequel, but it leaves room for future Mafia stories to build on its characters, family histories and lore. Hangar 13’s stated direction is to tell standalone stories across the franchise timeline, with events that ripple between games—not to promise that Enzo Favara will lead the next one.
What The Old Country’s ending means
Game Director Alex Cox described the finale as a “bittersweet victory.” Enzo’s closeness to Cesare, he explained, makes it impossible for Enzo to reject the offered hand. Cesare’s final act is tragic: he kills his only friend while trying to prove himself worthy of a code that has humiliated him. Cox said the ending played out as intended, though he acknowledged that more time in the closing chapters could have improved the pacing. In a spoiler-filled developer AMA, Cox said the response was more divisive than he expected, and that the passion in players’ reactions showed how much they cared about the story.

That emotional resolution is not the same as a sequel hook. It gives a future game a consequential history to draw on, but the cited developer comments do not say that Enzo, Cesare or their story will continue in the next installment.
What Hangar 13 has said about the next Mafia game
Cox has said that Hangar 13 has never made a traditional sequel and expects some elements to carry across games. He called The Old Country “the foundation of our universe lore.” In a separate interview with Xbox Wire, he described the plan as telling “more epic stories from across the Mafia timeline”—stories that remain standalone while events ripple from one to another and reward returning fans.

It does not establish a named next game, release window, lead character, setting or plot. Any specific sequel proposal is speculation unless Hangar 13 or 2K announces it.

The Man of Honor expansion adds two chapters set in Sicily in 1905, with Enzo working alongside Ennio Salieri. It extends The Old Country; it is not a separately announced next game. 2K announced the expansion and the four-game Mafia: The Omertà Collection, which includes The Old Country Definitive Edition with DLC alongside the Definitive Editions of Mafia, Mafia II and Mafia III.

Why the next story could look beyond Enzo
The Old Country is set in 1900s Sicily and follows Enzo Favara. Its place in the franchise’s wider timeline gives Hangar 13 room to explore how people, families and events connect across different eras without continuing Enzo’s plot scene by scene.
2K reported in 2025 that the Mafia franchise had sold in more than 35 million copies across consoles and PC. That is a franchise-wide figure, not sales for The Old Country, and it says nothing by itself about the next game’s content. The more useful clue remains the developers’ description of a series built around standalone stories and recurring connections.
