You must recover and repair the Unalloyed Gold Needle, give it to Millicent, deliver Valkyrie’s Prosthesis, complete her stops at Windmill Village and the Consecrated Snowfield, then choose between a yellow sign to aid her or a red sign to challenge her. The yellow outcome is the route that can lead to Miquella’s Needle and a cure for the Frenzied Flame.
Start the quest and obtain the Unalloyed Gold Needle
The opening sequence is documented in Eldenpedia’s Millicent quest entry and begins with Sage Gowry. Follow these steps in order.
1. Speak with Sage Gowry
Go to Gowry’s Shack in Caelid and exhaust Sage Gowry’s dialogue. His request starts the needle portion of the quest. His location and dialogue are also listed in Eldenpedia’s Sage Gowry entry.
2. Defeat Commander O’Neil
Travel to the Aeonia Swamp and defeat Commander O’Neil. He drops the snapped Unalloyed Gold Needle needed for the next step.
3. Have Gowry repair the needle
Return the snapped needle to Gowry at his shack. Reload the area or fast-travel away and back so that he can repair it, then speak with him again to collect the repaired Unalloyed Gold Needle.
4. Give the repaired needle to Millicent
Find Millicent at the Church of the Plague and give her the repaired needle. Rest or reload the area, then exhaust her dialogue. She rewards the Prosthesis-Wearer Heirloom.
5. Return to Gowry’s Shack
Reload once more and return to Gowry’s Shack. Exhaust Millicent’s dialogue there before moving to the Altus Plateau.
Move Millicent through Altus Plateau and the Haligtree
Millicent’s later locations are fixed progression points. Missing the prosthesis at Shaded Castle is the most common reason her next appearance does not occur.

6. Meet her at Erdtree-Gazing Hill
Find Millicent near the Erdtree-Gazing Hill Site of Grace on the Altus Plateau and exhaust her dialogue.
7. Retrieve Valkyrie’s Prosthesis
Go to Shaded Castle and take Valkyrie’s Prosthesis from the chest there. Return to Millicent and give it to her. This item is the gate to her remaining Altus Plateau progression.
8. Complete Windmill Village
Defeat the Godskin Apostle at Dominula, Windmill Village. Afterward, speak with Millicent near the Windmill Heights Site of Grace.
9. Find her in the Consecrated Snowfield
Continue to the Consecrated Snowfield and speak with her at the Ancient Snow Valley Ruins Site of Grace.

10. Reach the Prayer Room
Proceed to Elphael, Brace of the Haligtree, and speak with Millicent at the Prayer Room.
Reveal the final summon signs
11. Defeat the Ulcerated Tree Spirit
Near the Drainage Channel, defeat the Ulcerated Tree Spirit. Reload the area afterward. The yellow and red summon signs then appear, allowing you to choose the conclusion of Millicent’s quest.
Yellow sign or red sign: rewards and consequences
The two signs are mutually exclusive. Your choice determines which talisman you receive and whether you can recover the needle for Miquella’s Needle.

Choose the yellow sign before entering the encounter if you want the option to undo the Frenzied Flame condition later. Choose the red sign only when ending the branch for Millicent’s Prosthesis is an intentional trade-off.
How to obtain Miquella’s Needle
Miquella’s Needle is not awarded immediately after the yellow-sign encounter. It requires three additional actions:
- Use the yellow sign to aid Millicent. This is the only outcome that lets you recover the Unalloyed Gold Needle after the encounter.
- Defeat Malenia. After her defeat, keep the recovered needle.
- Use the needle on the Scarlet Aeonia in Malenia’s arena. Interacting with the Scarlet Aeonia transforms it into Miquella’s Needle, which can undo the Frenzied Flame condition.
Common progression checks
- Gowry has not repaired the needle: reload or fast-travel away from Gowry’s Shack, then return and speak with him again.
- Millicent is not at the expected location: exhaust her dialogue at the previous stop, including her Church of the Plague and Gowry’s Shack conversations, and confirm that Valkyrie’s Prosthesis was taken from Shaded Castle and delivered.
- No summon signs appear: defeat the Ulcerated Tree Spirit near the Drainage Channel and reload the area.
- You chose the red sign: the yellow-sign reward, the recovered needle and Miquella’s Needle route are no longer available on that quest progression.
