None of those labels carries an official percentage. Inactive is different: it is a later game-day roster listing, not another injury designation.
What each NFL injury designation means

The first three definitions are from the NFL’s 2017 Injury Report Policy. That policy uses qualitative terms, not standardized chances of playing. “Questionable” therefore does not officially mean a 50/50 chance, and “doubtful” does not map to a published probability.
How practice reports differ from game status
A practice report records participation; a Game Status Report describes the club’s anticipated player availability. A player can be limited or absent from practice and still be expected to play. Under the NFL policy, a player who practiced with a limitation but is certain to play need not appear on the Game Status Report. The policy also says that when a player appears on a Practice Report but not the Game Status Report, the club is considered certain he will play; if he is later deactivated, an explanation is required.
The labels are not limited to injuries. The NFL policy requires clubs to report players who will not play for injury or non-injury reasons, and to explain when a player’s game status is in question for a non-injury reason.

When teams issue and update reports
For the 2026 season, the NFL’s Important Dates calendar says practice reports are due by 4:00 p.m. New York time, or as soon as possible after practice. The exact reporting days depend on the upcoming game’s day. For a Sunday game, teams submit practice reports Wednesday, Thursday and Friday, with the Game Status Report due Friday. Other game days follow a shifted schedule, so consult the league calendar for unusual weeks.
Teams must report a change in a player’s condition after the initial Game Status Report. The report is an assessment of expected availability at that point, not a guarantee that the player’s status cannot change.

Inactive is the final roster-related information in this sequence, separate from the earlier injury designation. NFL Football Operations says the club’s inactive list is included in its Game Day Administration Report and delivered to the referee at the officiating meeting held 90 minutes before kickoff. The older injury policy also requires a club to report a new injury or other event before that meeting if it may affect a player’s availability.
Because a designation is an earlier availability assessment and inactive is a later game-day listing, a player can be listed as questionable and ultimately be inactive. To confirm a particular player’s status, check the team’s or league’s game-day inactive announcement.
