How to read this difficulty ranking
This is a mechanics-based editorial comparison, not a ranking derived from clear rates or death statistics. Grinding Gear Games’ official patch notes do not publish comparable player failure rates or build-adjusted difficulty measurements. Encounter tuning and access rules can change, so the patch version matters.

Difficulty also has more than one dimension: the actions a fight demands, how severely a mistake is punished, the encounter version and the character’s ability to meet its demands. A boss that is manageable for one build or player may be a major obstacle for another.
Which Path of Exile 2 bosses are most difficult?
1. Uber Arbiter of Ash: a high-end difficulty candidate
The Uber Arbiter of Ash is a defensible pick for the hardest encounter in this comparison. It is important to distinguish it from the regular Arbiter: patch 0.3.0 introduced an Uber version accessed with Calamity Fragments obtained from Citadel bosses under the specified Atlas progression. The same update changed the regular encounter to static difficulty with infinite portals and rewards matching its previous Difficulty 4 rewards. Grinding Gear Games’ 0.3.0 patch notes describe those changes.

The fight’s phase transition is another consideration. In patch 0.4.0, the Arbiter began changing phase at a lower life threshold and healed to full during the transition. That patch detail makes phase management relevant when assessing the fight; it does not establish a measured difficulty rank. The 0.4.0 patch notes document the change.
2. Zarokh, the Temporal: objective pressure during the fight
Zarokh, encountered in the Trial of the Sekhemas, tests more than damage output. Players must collect hourglasses during the encounter, and missing one has a specific consequence. In patch 0.3.0, failing to collect every hourglass stopped failing the trial outright and instead damaged Honour. Zarokh then entered a secondary phase in which the hourglasses and slow effect continued at a much slower rate. The 0.3.0 notes record these rule changes.

That makes Zarokh a compelling mechanics-focused challenge: players have to manage an objective while responding to the boss, with Honour loss as the failure consequence. The revised rule should not be confused with earlier versions of the encounter.
3. The Trialmaster: a dangerous attack, not a proven rank
The Trialmaster’s channeled blood slam is a concrete reason to treat the fight as demanding. In patch 0.2.0, Grinding Gear Games increased the damage of that attack while reducing life for both The Trialmaster and Zarokh. These are tuning changes, not evidence that either boss is objectively harder than the other. The 0.2.0 notes describe the adjustments.

Other Pinnacle encounters belong in a broader comparison
Olroth, Xesht, The King in the Mists and the regular Arbiter of Ash should not be excluded from a comparison of Pinnacle encounters. Patch 0.2.0 adjusted life for those bosses as well as for The Trialmaster, Zarokh and Arbiter of Ash. It also made Difficulty 0 easier while intending Difficulty 4 to preserve its previous challenge. A comparison that mixes tiers or versions without naming them can therefore give a misleading impression. Grinding Gear Games’ 0.2.0 patch notes provide that context.

Why patch version changes the answer
Boss difficulty is not fixed when encounter rules change. For example, in patch 0.1.1 the Arbiter had up to six shared respawn attempts, with fewer at higher difficulty. By patch 0.3.0, the regular Arbiter encounter had infinite portals. Those rules affect the cost of mistakes, but neither portal count is a statistic of how often players win. The 0.1.1 patch notes and the 0.3.0 notes document the change in context.

For a fair comparison, identify the patch, encounter version and difficulty tier. Avoid comparing a regular encounter at one setting with an Uber encounter at another as if they were equivalent.

If you want one editorial pick, the Uber Arbiter of Ash is a strong high-end candidate. For fights centered on mechanics, Zarokh and The Trialmaster are also reasonable choices. The official notes establish specific encounter rules and tuning, but do not settle an exact order among these bosses or prove a universal hardest boss.
