The Rotten Winged Sword Insignia is a talisman in Elden Ring that increases attack power as you land successive attacks. To obtain it, progress Millicent’s quest to Elphael and choose to help her defeat her sisters. That choice determines which quest reward you receive, so decide before entering the encounter.
What the Rotten Winged Sword Insignia does
The in-game description, reproduced by the Elden Ring Wiki, reads: “A talisman depicting a raised prosthetic blade. An honor bestowed upon the valkyries who serve the Goddess of Rot. Greatly raises attack power with successive attacks. The four sisters were born in the Swamp of Aeonia, and came to the Haligtree under the aegis of Gowry. And yet, those buds were doomed to never blossom.” Source

In practical terms, the talisman rewards landing attacks in sequence. Elden Ring Wiki’s guide reports three bonus tiers—6%, 8%, and 13%—and says the bonus drops a tier after 1.5 seconds without a hit. Those figures and timing are guide-reported mechanics, not confirmation from the game publisher. The effect is most relevant to a setup that can maintain attack chains; its value depends on weapon, attack cadence, the encounter, and what else you could equip.

- Progress Millicent’s quest until you reach Elphael, Brace of the Haligtree.
- At the quest encounter there, choose to assist Millicent against her sisters.
- After the encounter, Millicent gives you the Rotten Winged Sword Insignia.
The available guides establish this final route and reward, but do not provide a complete, verified walkthrough of every earlier quest prerequisite. If you have not reached Elphael with Millicent, consult a current quest walkthrough for the earlier stages before relying on this short route.

Choose the quest reward before the fight
The decision to assist Millicent is consequential: the alternative outcome awards Millicent’s Prosthesis instead. The referenced community guide says these two rewards cannot both be obtained in the same playthrough. You can use the Rotten Winged Sword Insignia with Millicent’s Prosthesis, but that combination requires having both items available, which the quest outcomes do not allow in one run.

Does a game purchase include the talisman?
No. The talisman is an in-game quest reward, not a separate product or an item granted by buying a particular edition. Bandai Namco’s official Elden Ring product page lists physical and digital editions and platform selections for the game: official product page.
