What happens when you confront Istvan?
The choice comes during Storm, as Henry escapes Trosky Castle after being captured and tortured. He is described as wounded and weakened: the torture debuff lowers his Strength, Agility, and Vitality for the quest. The choice determines how he deals with Istvan, not whether Istvan survives.

Accept the duel
Henry fights Istvan one-on-one. Walkthroughs characterize the fight as difficult in Henry’s weakened condition. They recommend managing stamina, parrying and riposting, and considering armor, potions, and a backup weapon in case Istvan disarms Henry. Kill Istvan dishonorably
Henry stabs Istvan and throws him out of a window, killing him without a duel. This avoids the immediate combat encounter, making it the lower-risk option if Henry is in poor condition or you do not want the fight. The route is described in the PowerPyx walkthrough.

Either way, Henry continues through the castle to recover his confiscated equipment and keys, find the orders, and escape. PowerPyx says Radzig Kobyla’s longsword is awarded regardless of whether Henry duels Istvan.
Gamer Guides says Henry’s method is mentioned in the ending, but does not specify the exact scene or establish whether the choice changes other quests or the ending state. Treat that as a possible acknowledgment, not evidence of a substantial ending branch or proof that the choice has no later effect.
