What happens at the choice?
The decision takes place during The Secret of Cabot House at Parsons State Insane Asylum. After the confrontation in the containment area, Jack asks the Sole Survivor to kill his father, Lorenzo, while Lorenzo asks to be released. The quest then sends you back to Cabot House, where you can still choose which side to support. Their competing accounts are character viewpoints, not a settled verdict on who is right.

Choose based on what you want
Pick Jack’s side for the weapon
If Lorenzo’s Artifact Gun is the reward you care about, kill Lorenzo in the cell. Do not expect the weapon immediately: return to Jack after about a week of in-game time.
Pick Lorenzo’s side for recurring serum
If you want Mysterious Serum, free Lorenzo and help him defeat the Cabots. The serum is a repeatable reward, but it is handed over one vial at a time rather than as an unlimited supply all at once.

Free Lorenzo first if you are undecided
Freeing him does not lock you into helping him. If you decide you want to protect the Cabots, you can oppose Lorenzo later at the family confrontation.

A Massachusetts Surgical Journal is located in Lorenzo’s cell. It is an in-game collectible and separate from the choice’s main rewards.
