Player skill, character, build, and whether a connected route counts as part of the fight can all change the order.
How this ranking works
The order below ranks the boss encounters themselves under ordinary run conditions, rather than treating every preceding room or route as part of the fight. Four criteria guide the comparison: pattern difficulty, damage density or arena pressure, build-to-build variance, and counterplay clarity. These are practical editorial criteria, not a validated scoring system; no player survey or reproducible difficulty data establishes a canonical order.
Route cost and expected player strength also matter. A boss faced later in a run may feel easier with a stronger build, while an optional endgame fight can be attempted under very different circumstances. The Home route is a special case: Dogma, the Ultra Horsemen, and The Beast form a connected sequence, so ranking The Beast alone understates the challenge of completing that route.

Satan
Satan takes the top spot in this editorial ranking because the encounter demands that players handle changing attack patterns while preserving room to maneuver. The fight is also part of a broader route decision: the difficulty of reaching and taking on Satan depends on the run that led there. Its placement is a judgment, not a measured consensus.

Isaac
Isaac is ranked just below Satan. The pressure comes from reading attacks and maintaining safe positioning through the encounter, rather than from any verified health or damage statistic. Players whose builds make dodging or sustained damage easier may find the comparison between the two fights different.

The Lamb
The Lamb is another demanding late-run encounter. Its position reflects the combined pressure of its patterns and the possibility that a run reaches it with a build that does not offer reliable defensive options. The available evidence does not establish a universal player ranking that separates The Lamb from the other major endgame bosses.

Hush
Hush is a sustained positioning challenge: the fight asks players to keep tracking attacks while finding safe space in the arena. Its difficulty can shift substantially with the build, especially when a run lacks tools that make prolonged combat more forgiving. Hush is optional endgame content, so the circumstances under which players attempt it vary.

Mother
Mother is ranked fifth for a long fight that tests resource management as well as pattern recognition. That extended demand can make the encounter feel severe when a build is inconsistent. It is a frequent contender in anecdotal player discussion, but those comments do not show that the community has settled on a single hardest boss.

Ultra Greed
Ultra Greed adds coin-related hazards to the combat, giving the arena pressure a distinct source beyond the boss’s attacks. That mix can complicate positioning and make the encounter feel less predictable from build to build. This list places him below the top five, though the added hazards may make him a higher-ranked threat for some players.

Mega Satan
Mega Satan ranks seventh. As with the other late-run fights, a player’s build and route shape how difficult the encounter feels; this ranking does not claim an independently verified numerical comparison. Its lower position here reflects the editorial criteria above, not an objective measurement of the fight.

Delirium
Delirium’s changing patterns make the fight notably variable. That uncertainty is the reason it remains on this list despite ranking eighth: players may have less consistent opportunities to learn and respond than they do in fights with more legible counterplay. Delirium is also named alongside Mother in anecdotal discussion of the hardest final bosses, but that discussion is not a representative poll.

The Beast
The Beast is ranked ninth when considered as a single boss encounter. That distinction matters: the Home route leads through the Ascent and then a sequence involving Dogma, the Ultra Horsemen, and The Beast. A player judging the difficulty of completing the route should count the entire gauntlet, not just this final fight, and may rank the overall challenge much higher.

The Adversary
The Adversary rounds out this particular top ten. Ranking it tenth does not mean every run will make the fight easy; build strength and player familiarity still affect the experience. It sits lower here because this ranking reserves its highest places for encounters judged to demand greater sustained pressure, pattern handling, or adaptation to variance.
Why other players may rank them differently
There is no source-backed universal difficulty order for these bosses. In one player discussion, respondents name Mother and Delirium as especially difficult, illustrating the disagreement rather than resolving it. The discussion is anecdotal, not a representative poll, and no verified statistic or official ranking establishes which boss is objectively hardest.
Three choices can change a personal ranking:
- What counts as the encounter: The Beast alone is different from completing the Home-route sequence of Dogma, the Ultra Horsemen, and The Beast.
- Your build and character: An inconsistent build can magnify a long fight or make an unclear pattern harder to manage.
- Your measure of difficulty: A player who struggles most with sustained positioning may put Hush higher; one who finds variable patterns hardest may elevate Delirium.
Route context for the hardest fights
The route notes available for this ranking describe the Mother route as progressing through Downpour or Dross, Mines or Ashpit, Mausoleum or Gehenna, and then Corpse. The Home route uses the Ascent and ends with Dogma, the Ultra Horsemen, and The Beast. Hush and Delirium are optional endgame encounters. These route descriptions come from a secondary guide, not an official developer reference, so check the game itself for current route requirements and mechanics.
