Instead of revealing every enemy with a conventional weapon, players exhale and hum into the headset microphone to create an in-game orb that exposes hidden Wraiths. They then aim, shoot and dodge while trying to maintain controlled breathing.
That makes Zengence an inventive breathwork and wellness game—not a proven treatment for anxiety, depression or other mental-health conditions. Research on related VR breathing systems is promising in places, but it does not establish that Zengence itself has been clinically validated.

What is Zengence?
Zengence: Take Aim with Every Breath is a first-person fantasy shooter developed by DeepWell DTx, a company that positions games as immersive mental-health therapeutics.
It is available for Meta Quest, with the developer specifically citing Quest 2 and Quest 3 compatibility. The Meta listing categorizes it as a shooter and action game, with an all-ages rating and fantasy violence. The product includes 30 levels, daily challenges and a separate Zen Garden intended for calmer breathwork practice.

DeepWell describes Zengence as a way to practise staying regulated during stressful situations. That is the developer’s design rationale and product positioning, not independent proof that the game treats a diagnosed mental-health condition.
How the breathing mechanic works
Breathing is not merely an activity happening alongside the game. It is a functional control input:
- Wraiths begin concealed in the fantasy environment.
- The player exhales while humming or vocalizing into the Quest headset microphone.
- A correctly timed breath produces an in-game orb.
- The orb reveals hidden Wraiths, including enemies that may be harder to detect.
- The player aims with the controllers and fires at the revealed targets.
Better control of exhalation and vocal toning can make the orb more powerful. Music, timing, aiming and enemy attacks turn the breathing exercise into a repeating gameplay loop. Coverage of the game also describes it as avoiding a conventional game-over state: taking a hit can reduce the final score rather than ending the session. That may make the experience less punitive, although combat visuals and simulated threats can still feel stressful to some players.
The public product information confirms microphone-based breathing and humming interaction. It does not establish that the game measures respiratory signals, heart rate or heart-rate variability with clinical-grade accuracy. “Biofeedback” here should therefore be understood as an immediate response to the player’s breath and voice input, not as a medical monitoring claim.

Why might rhythmic breathing help?
Slow, regular breathing is commonly used in relaxation and stress-management exercises. Respiratory biofeedback can make breathing easier to notice and deliberately adjust by providing immediate feedback. Zengence adds several game-design mechanisms to that basic idea:
- Attention redirection: Players focus on timing, sound, aiming and enemies instead of rumination.
- Immediate reinforcement: A successful exhalation produces a visible effect that also helps the player progress.
- Repetition: Levels and daily challenges may encourage regular practice.
- Practice under pressure: The player is asked to maintain the mechanic while dealing with in-game threats.
- Vocal toning: Humming gives the exhalation an audible rhythm rather than relying only on silent pacing.
DeepWell’s argument is that people can practise breathing through simulated pressure rather than only in a quiet meditation setting. That could make the exercise more engaging for players who struggle to sustain conventional meditation. It remains a hypothesis about engagement and skill transfer, however, rather than a demonstrated clinical outcome for every user.
What independent research actually shows
Related VR breathing studies are encouraging but limited
A 2021 study examined a mobile VR respiratory-biofeedback game designed to encourage diaphragmatic breathing. It involved 45 healthy undergraduate participants completing six sessions over about a week. The researchers reported improvements in breath awareness, diaphragmatic-breathing measures, relaxation, perceived stress, burnout symptoms and relaxation-related self-efficacy. Read the study.
Those findings should not be presented as a Zengence trial. The research used a prototype rather than necessarily the commercial game, had a small and relatively homogeneous sample, lacked an active control group and had limited follow-up. It could not determine whether any benefit came from VR, breathing, biofeedback, restorative scenery or gamification. It also did not establish that improvements transferred to everyday stressful situations.

Another study used VR biofeedback games during experimentally induced stress and reported greater heart-rate variability in trained participants than in untrained participants. That suggests VR games may help people practise regulation under pressure, but the study concerned research systems—not Zengence. See the PubMed record.
VR is not automatically better than ordinary breathing
A 2024 systematic review and meta-analysis of six randomized controlled trials found no significant advantage for VR breathing interventions over non-VR breathing interventions across overall mental health, stress, anxiety, mood, heart rate or heart-rate variability. View the review.
This is the most important qualification for Zengence. Immersion may improve attention, enjoyment or adherence for a particular person, but the headset itself has not been shown to make breathing exercises inherently more effective than simpler guided breathing.
A 2025 scoping review identified 18 VR breathing experiences and concluded that VR can help focus attention, reduce distractions, offer real-time feedback and create controlled practice environments. A scoping review maps available work; it does not prove that every commercial product works or that VR outperforms non-VR practice.

A 2026 study of a separate system called BreathMe tested breathing rhythms and interaction modes in 40 college students. Its results may inform future VR breathing design, but BreathMe is not Zengence and its findings cannot be used as direct evidence for this game.
Is Zengence therapy?
The careful description is breathwork-focused wellness or a digital-therapeutic-style game. DeepWell uses mental-health and therapeutic language for its products, but that does not by itself establish regulatory approval, clinical effectiveness, reimbursement eligibility or equivalence to psychotherapy.
Zengence may be useful as a structured self-regulation exercise. It should not replace psychotherapy, psychiatric care, prescribed medication or emergency support. There is currently no independently verified large randomized clinical trial showing that Zengence treats panic disorder, PTSD, depression, anxiety disorders or another diagnosed condition.

Who should try it—and who should skip it?
Zengence may suit:
- Quest owners who find conventional meditation boring or difficult to sustain.
- Gamers who want audiovisual prompts for practising controlled breathing.
- People who enjoy fantasy action and do not mind humming aloud.
- Anyone seeking a short wellness routine rather than formal treatment.
It may be a poor fit for people who dislike headsets, motion, fantasy combat or vocalizing. It is also unsuitable as a substitute for individualized mental-health care.

Watch for VR discomfort
VR can cause nausea, dizziness, eye strain or disorientation. Stop if symptoms develop instead of pushing through them. Simulator sickness is a recognized practical issue in VR breathing applications.
Do not force the breathing
Follow the game’s instructions rather than taking unusually deep, rapid or forceful breaths. Stop if you feel light-headed, distressed or short of breath. More breathing is not automatically better, and anyone with a breathing disorder or a medical concern should seek appropriate professional advice before deliberately changing their breathing pattern.
Microphone and privacy considerations
The mechanic depends on the headset microphone. Background noise, other people speaking, a low-volume hum, microphone permissions, mute settings and room acoustics may affect recognition. If the game does not respond, check microphone permissions, make sure the headset is not muted, move somewhere quieter and restart the app.

Buying and setup guide
- Check the current Meta Quest store listing for availability and price. The price was not available in the product data reviewed and can change.
- Confirm that you have a compatible Meta Quest headset. DeepWell specifically identifies Quest 2 and Quest 3.
- Install the game and grant microphone access if prompted.
- Start with the tutorial or Zen Garden to learn the breath and humming input in a lower-pressure setting.
- Use the controllers to aim and fire after the orb reveals the Wraiths.
- Try daily challenges only if the core breathing interaction feels comfortable.
If you do not already own a headset, the total cost is not just the game price. Check the official Meta Quest hardware store for current headset pricing and bundles. Zengence is most straightforward as an add-on for an existing Quest owner, not necessarily as a low-cost wellness purchase from scratch.
Verdict
Zengence is a clever attempt to make breath practice feel like an interactive game: exhale and hum to reveal enemies, then stay composed while fighting them. Its strongest case is engagement. It may give some players a memorable reason to practise controlled breathing more consistently than a conventional meditation app.
The evidence does not justify calling it a clinically proven treatment, and research so far does not show that VR breathing is broadly more effective than ordinary breathing exercises. Treat Zengence as a promising wellness experiment and a game with a distinctive control system—not as a replacement for professional mental-health care.
