Neuralink’s second PRIME Study participant, identified as Alex, used the company’s implanted Link to control cursor movement and directional looking in Counter-Strike 2. He also used a mouth-operated Quadstick for other inputs, making the demonstration a hybrid assistive-control system—not unrestricted, brain-only gaming.

In a video published by Neuralink on August 21, 2024, Alex is shown playing Counter-Strike 2 from a wheelchair with a laptop and assistive equipment. Neuralink says the implanted Link let him control where he looked in the game while the Quadstick handled additional controls.

That distinction matters. The demonstration shows how a brain-computer interface can add a second control channel to an existing accessibility setup. It does not show the implant independently replacing a mouse, keyboard, gamepad, or every function of the Quadstick.

Neuralink’s update says Alex underwent surgery at Barrow Neurological Institute, was discharged the following day, and had a smooth recovery. After the Link was connected to his computer, the company says he began controlling a cursor with his brain in less than five minutes. Following the initial research session, Neuralink says he used the system independently to play Counter-Strike 2.

How Alex’s controls were divided

System Role described by Neuralink
Implanted Link Cursor movement and directional looking
Quadstick Mouth-operated joystick, sip-and-puff inputs, and lip-position input for functions such as clicking or selection
Counter-Strike 2 Received the combined inputs as game controls

Before the implant, Alex’s Quadstick had a practical limitation: its single joystick required him to switch between moving and aiming. Neuralink says the Link allowed him to control directional looking with brain signals while using the Quadstick for other actions. That made simultaneous movement and aiming possible, according to the company.

The improvement is therefore best understood relative to Alex’s previous setup—not relative to an able-bodied player using a conventional mouse and keyboard. Neuralink supplied an additional input method rather than eliminating the need for assistive hardware.

Who is Alex?

Neuralink identifies the participant only as Alex, saying his name was shared at his request. The company describes him as a former automotive technician who worked on vehicles and heavy machinery before a spinal-cord injury resulted in quadriplegia. There is no basis in the supplied sources for identifying or speculating about him beyond those details.

What else did he use the Link for?

The gaming footage is only part of Neuralink’s account. The company says that on his second day using the Link, Alex opened Autodesk Fusion 360 for the first time and designed a custom mount for his Neuralink charger. The mount was then 3D-printed and incorporated into his setup.

Neuralink also described a mode-switching interface intended to change mouse functionality while Alex worked in CAD software. This example points to a broader use for BCIs: not simply controlling a game, but regaining access to design, fabrication, technical hobbies, and other computer-based work.

What the Neuralink Link actually is

The Link is an investigational, fully implantable and wireless brain-computer interface. The PRIME Study record describes the N1 Implant as a skull-mounted, wireless, rechargeable device connected to electrode threads placed in the brain by Neuralink’s R1 surgical robot.

The study is listed as a first-in-human early-feasibility study evaluating initial safety and functionality in people with tetraparesis or tetraplegia. The ClinicalTrials.gov record retrieved for this article was updated January 9, 2026, and listed the study as recruiting.

That status is not the same as commercial approval. As the FDA explains, an Investigational Device Exemption permits a device to be studied in a clinical investigation; it does not mean the device has been cleared or approved for broad commercial sale. FDA guidance on implanted BCIs describes these systems as investigational neuroprostheses intended to help restore lost motor or sensory capabilities.

How brain signals become cursor movement

  1. Electrodes record neural activity associated with an intended movement.
  2. Decoding software interprets those signals as commands, such as moving a cursor.
  3. The computer receives the decoded commands as input.
  4. The user practices and refines control through feedback and calibration.

This should not be confused with reading arbitrary thoughts. The available evidence supports a system trained to decode particular movement intentions. It does not establish that the Link reads private thoughts, memories, emotions, complete sentences, or untrained intentions.

What the video demonstrates—and what it does not

What it demonstrates

  • Brain-derived cursor control, according to Neuralink’s account.
  • Use of an implanted BCI alongside existing assistive technology.
  • A practical recreational application involving a complex computer interface.
  • A potential route to greater digital independence for someone unable to use conventional controls.
  • Use of computer-aided design for a tangible, self-directed project.

What it does not establish

  • That Alex controlled every game function through brain signals alone.
  • That the Link can decode arbitrary thoughts.
  • That the implant is ready for consumers or approved for general use.
  • That it improves gaming performance for able-bodied players.
  • That the system is safe, durable, or effective for the general public.
  • That the footage provides a complete measure of latency, accuracy, or competitive performance.

The supplied sources do not establish whether the session was online or offline, whether footage was edited, how many controls were assigned to the Link, or whether Alex could fire, reload, crouch, switch weapons, and interact using the implant alone. They also provide no gameplay latency, score, rank, kill/death ratio, or frame-by-frame performance data.

The reliability caveat: Neuralink’s first participant

Neuralink’s second-participant update also discusses its first participant, identified by the company as Noland. Neuralink said some electrode threads retracted, temporarily reducing performance, and that performance later recovered after the threads stabilized.

The company said it had implemented surgical mitigations for Alex and had observed no thread retraction in him at the time of the August 2024 update. That is a report about the status at that time, not evidence that the known reliability issue has been permanently eliminated.

Brain surgery is invasive, and long-term durability remains an open question. Performance may also depend on signal quality, calibration, software, fatigue, hardware placement, and maintenance. Results from a small early-feasibility study cannot be generalized to everyone with paralysis.

What Neuralink says it is working toward

Neuralink says its development goals include decoding multiple clicks, multiple simultaneous movement intentions, full mouse and video-game-controller functionality, and handwriting intentions for faster text entry. It has also described possible control of physical devices such as robotic arms and wheelchairs.

Those are future or developing capabilities, not results established by the Counter-Strike 2 demonstration.

Why the hybrid setup is still significant

Calling the video “mind-controlled gaming” captures the visual hook but misses the accessibility achievement. For Alex, the relevant question is not whether a BCI can replace every input device. It is whether the technology can reduce a bottleneck that makes existing equipment difficult to use.

  • Mechanical Circular D-pad: Designed for quick reactions and accuracy in every direction, this D-pad elevates your gaming experience with superior responsiveness.

Adding brain-derived directional control to a mouth-operated controller could make simultaneous actions possible without forcing the user to switch modes. The Fusion 360 example may be even more consequential: it shows a potential path back to designing and making objects, not just playing a game.

For people considering assistive technology today, the practical option remains established hardware such as the Quadstick. Neuralink is not a product available for ordinary purchase, and joining a patient registry or clinical study is not the same as buying a device. Neuralink’s own study information says participation does not guarantee a benefit.

The bottom line on Neuralink and Counter-Strike 2

Alex did use brain signals from Neuralink’s implanted Link to control part of Counter-Strike 2, but he played with the Link alongside a Quadstick mouth controller. The important result is not telepathy or unrestricted thought-only gaming. It is the possibility that an investigational BCI can supplement assistive hardware with another control channel, helping a person navigate complex software, play games, and pursue creative work with greater independence.
