For the Genocide Route fight, equip the Burnt Pan if you want each healing item to restore 4 extra HP. Choose Cloudy Glasses or the Torn Notebook instead if you want longer invulnerability and slower KARMA (KR) drain. Fill your eight inventory slots with the strongest heals available to your run, and keep selecting FIGHT to advance the encounter.
Which equipment should you use?
Sans dodges attacks, and ordinary attack and defense values do little to solve this fight: defense does not reduce his constant 1 damage, while attack does not prevent him from dodging. The equipment choice is therefore about healing or surviving between hits, not increasing damage. The Undertale Wiki’s Sans battle reference describes the encounter mechanics and equipment effects.

The Burnt Pan’s bonus applies to each healing item, so its value depends on how many items you use. Cloudy Glasses and Torn Notebook work differently: INV equipment makes damage get ignored every few frames, slowing the ongoing KR drain. The battle reference also mentions Stained Apron and Temmie Armor for healing or INV effects, but neither is obtainable on the Genocide Route, so neither is a viable choice for this fight.
Which healing items are most useful?
Healing amounts below are in-game mechanics listed in the Undertale Wiki’s consumables reference. Instant Noodles restore 90 HP in Serious Mode, the mode used for this battle.

Carry the best combination you can actually obtain on your run. The Burnt Pan adds 4 HP to each consumable heal, but the eight-slot capacity means each healing item displaces another; weigh the bonus against the INV benefit of alternate equipment.
When should you heal?
KARMA can keep draining HP during your turn, so healing while that effect is active may waste some of the recovery. When practical, wait until the KR effect ends before using an item. Sans does not attack during his false-mercy sequence, making that a safe opportunity to heal.

Keep choosing FIGHT. Sans can dodge up to 24 attacks, producing “MISS,” but continuing to attack is required to advance the battle. Checking or choosing other actions does not move the fight dialogue forward. Since ordinary attack bonuses do not stop the dodges, prioritize preserving enough health and healing items to withstand the encounter while making the required attacks.
