What Crutch Use Can—and Cannot—Tell You About a Knee Injury

Crutches indicate a need for walking support or modified weight-bearing—not a particular knee diagnosis. Follow your clinician’s instructions and know which symptoms need prompt assessment.

Being given crutches after a knee injury means you may need help walking or limiting weight through the leg. It does not, by itself, tell you whether you have a fracture, a ligament or meniscus tear, or how serious the injury is. The diagnosis and your clinician’s weight-bearing instructions—not the crutches alone—are what matter.

Does needing crutches mean your knee injury is serious?

Not necessarily. Crutches are a mobility aid, not a diagnosis or severity grade. A clinician may recommend them to make walking safer or to control how much load goes through an injured leg. The reason can differ from person to person, so needing crutches cannot reliably compare the seriousness of two knee injuries.

Knee injuries can involve bone, ligaments, cartilage such as the meniscus, tendons, or muscles. Chelsea and Westminster Hospital NHS Foundation Trust describes crutches as a way to help some patients walk with a good gait pattern, depending on the extent of injury. Can crutches tell you if you tore a ligament or meniscus?

No. Crutch use alone is not proof of a fracture, ligament tear, meniscus injury, or any other specific diagnosis. Nor does it establish a severity grade. Diagnosis depends on clinical assessment; do not infer it from the walking aid you were given.

Why might a clinician give you crutches?

Crutches can help you move while reducing or controlling load through the injured leg. They may be recommended because of your injury, pain, walking ability, or the need to protect the leg while it heals. Their use should fit the weight-bearing plan set by the clinician who assessed you.

That plan may be non-weight-bearing or partial-weight-bearing. If you are unsure whether you may put weight through the leg, contact your treating team rather than choosing a technique based on the fact that you have crutches. When can you stop using crutches?

There is no universal timetable for stopping. Follow the advice from the clinician managing your injury, including when it is safe to increase weight-bearing. Improved or pain-free walking does not necessarily mean the injury is fully healed; monitor symptoms and follow professional advice, especially before returning to sport.

Gateshead Health’s guidance on knee fractures describes progressing from two crutches to one and then none when walking comfortably without a limp, while following hospital advice about when full weight-bearing is safe. When should a knee injury be assessed urgently?

Seek prompt medical advice if you have any of the following warning signs:

  • A very painful knee, or inability to move it or put weight on it.
  • Severe swelling or a change in the knee’s shape.
  • A locked knee, giving way, or painful clicking.
  • Feverishness along with redness or heat around the knee.
  • After significant trauma, immediate inability to continue activity, marked swelling, a pop at the time of injury, or numbness or tingling down the leg.

The NHS advises people in the UK to contact NHS 111 now for the first group of symptoms; the appropriate urgent-care route varies by location. Leeds Community Healthcare NHS Trust also lists high-impact trauma, a pop, numbness or tingling, a locked knee, repeated giving way, and a hot, red, or very swollen knee as reasons to seek medical advice.
