Curing Ra’s al Ghul at the end of the Shadow War mission brings him back to fighting strength. He kills Nyssa Raatko, escapes Batman, and stays free to recover. The League still leaves Gotham. Destroying the cure instead leaves Ra’s in GCPD custody to die and lets Nyssa’s rebels win.
Where the choice appears
The decision comes at the end of Shadow War, one of the Season of Infamy: Most Wanted expansion missions. WB Games Support says players find these missions under Most Wanted and unlock them as they progress through the main story, so you need to reach that point in the campaign before the mission is available.

If you administer the Lazarus cure
Curing Ra’s is the option that keeps him alive and mobile. In the mission accounts reviewed:
- Ra’s recovers enough to fight.
- He attacks and kills Nyssa Raatko.
- He escapes Batman and remains free to recover.
Nyssa’s death is the cost of this branch. Ra’s is not left dying in a hospital, but he is not defeated either.

If you destroy the cure and the machinery sustaining Ra’s
Destroying the cure and the equipment keeping Ra’s alive changes the outcome for both leaders:
- Batman fights the League loyalists.
- He stops Nyssa from killing Ra’s directly.
- He takes Ra’s into GCPD custody, where Ra’s is left to die.
- Nyssa’s rebels prevail over the loyalists.
This is the branch in which Ra’s is captured rather than escaping, and Nyssa is the one who ends up leading the League’s remaining forces.

The two options differ mainly in what happens to Ra’s and Nyssa. Ra’s survives and escapes in one branch and dies in custody in the other, and the faction that prevails is different.
What stays the same
In both branches the League leaves Gotham, so the choice does not change the city-level result of the Shadow War storyline. The sources reviewed describe the mission’s local consequences only. They do not describe how either branch affects the ending of the main Batman: Arkham Knight campaign.
