Freeing True Soul Nere in Grymforge triggers a cutscene and a faction confrontation; it does not simply save him and end the quest. Nere throws Meerna into the lava, and your earlier conversations help determine whether you fight beside him or against him. The other deep gnomes can still be freed after either battle, but the encounter has a short gameplay timer.
What happens when you free Nere?
Nere is trapped behind a cave-in in Grymforge, in Act One, while deep gnomes are being forced to clear the rubble. Interacting with the cave-in to speak with him sets the confrontation in motion and affects the Save the Grymforge Gnomes quest. The BG3 Wiki quest page details the encounter’s branches.
When Nere emerges, he throws Meerna into the lava. The quest reference says she cannot be saved, even if you interrupt the cutscene by starting combat. What follows depends on your dialogue and any earlier agreements: the conflict can pit you against Elder Brithvar and the mutineers, or against Nere and his followers.
If you stay silent or tell Nere to finish the slaves, he massacres the gnomes. Asking him not to kill innocents does not by itself secure their safety: Nere orders Sergeant Thrinn to kill them, and the scene can still lead into a faction battle. The encounter is therefore not a simple choice between “save Nere” and “save the gnomes.”

Which side can you take?
Previous conversations and deals shape who turns hostile and whether you can choose between the factions. These are the broad outcomes; the exact battle depends on how you reached the confrontation.

Either route can lead to the other gnomes being freed, provided you follow up with the relevant dialogue after combat. Speak to them promptly: the quest reference warns that resting before doing so may cause them to leave.

How the Grymforge timer works
The timer begins when a party member interacts with the rubble to speak with Nere. It advances when you take a long rest or leave the Grymforge region; it is a gameplay-specific sequence, not a real-time countdown.
- First advance: A warning scene occurs.
- Second advance: Nere dies, the cave-in is cleared, and the duergar and gnomes depart. The rescue quest fails.
If you want to resolve the encounter and rescue the gnomes, do so before taking repeated rests or leaving the area.

What Nere’s fate changes later
If Nere survives and stays loyal to the Absolute
Nere appears briefly in Act Two as a zombie servant to Balthazar. This outcome depends on both his survival and his continued loyalty; freeing him does not guarantee that he remains an ally.
If Nere dies or is defeated
You can remove his head and deliver it to Sovereign Spaw or Glut for the separate Deliver Nere’s Head quest. If you reveal the truth about the Absolute
There is also a dialogue path in which you tell Nere that the Absolute is a front for mind flayers. The quest reference says he can abandon the cult and leave Grymforge. In other words, “rescuing” him from the cave-in does not necessarily mean recruiting him or keeping him alive for the rest of the story.

- To free the other gnomes: Resolve the battle, then use the appropriate post-fight dialogue with Nere or Brithvar and speak to the gnomes promptly.
- To pursue Nere’s head: Nere must die or be defeated so you can take his head for Spaw or Glut.
- To see Nere in Act Two: He must survive and remain loyal to the Absolute.
- To avoid the automatic failure: Do not let two timer advances pass after interacting with the rubble.
