An Ultra Wormhole above a Gym, accompanied by a countdown, signals that an Ultra Beast is about to arrive for a Raid Battle. The wormhole is the pre-raid indicator; the Ultra Beast that appears afterward is the raid boss.
What the Ultra Wormhole means
Pokémon GO’s Help Center explains that a Gym about to be taken over by an Ultra Beast displays an Ultra Wormhole and a countdown clock. When the countdown ends and the Ultra Beast arrives, Trainers can challenge it in a Raid Battle like a standard raid. Pokémon GO Help Center: Raid Battle types

What happens after the countdown
- Join the raid. When the Ultra Beast arrives, challenge it as you would a standard Raid Boss. The Help Center recommends teaming up because Ultra Beasts are extremely powerful.
- Use the catch opportunity after the battle. The game may award Beast Balls, special balls similar to Premier Balls that can be used only to catch Ultra Beasts.
- Catch it before the balls run out. You can use Berries during the catch attempt. If you run out of Beast Balls, the Ultra Beast flees.
The number of Beast Balls awarded depends on damage dealt, whether your team controls the Gym, and how quickly the Ultra Beast is defeated. Pokémon GO Help Center: Raid Battle types

Does a wormhole tell you which Ultra Beast is coming?
The wormhole and countdown indicate an upcoming Ultra Beast Raid, but the cited guidance does not identify which Ultra Beast will appear. Check the in-game raid listing or a current official Pokémon GO announcement for the active boss. A 2022 event announcement said Ultra Beasts would appear in five-star raids during that event; it is historical event information, not confirmation of the current raid rotation.
