Gamer Guides lists the die as carried by a bandit at X: 11407, Y: 13452 in the Trosky region. Treat this as a third-party map lead rather than an official location marker; NPC state or availability may vary.

A Fandom wiki search result also names the Raborsch spring point of interest and a beehive west of Lower Semine Mill as possible locations. The result names Dicer Filip and Master Bartholomew of Olomouc as carriers as well. The wiki page could not be opened, so these are leads to check in-game, not confirmed or exhaustive inventory records.
Can you steal the die from the Hired Hand?
Gameranx reports that the Hired Hand at Troskowitz Inn may carry the die. Its guide suggests waiting until he has stopped playing dice before attempting to pickpocket or rob him, and says the die may reappear after one or two in-game days. This is an older guide-level method; the available evidence does not establish that it has been revalidated on the current build.

Does Saint Antiochus’ Die still always roll a 3?
No guaranteed 3 should be assumed. The in-game description reproduced by Gamer Guides says it always rolls a 3, but Deep Silver’s official Patch 1.4 notes say: “Fixed probability of values thrown with Saint Antiochus’ die.” The notes do not give the current probabilities or explain the resulting distribution, so claims that it guarantees a 3—or guarantees a win—are not reliable current advice.

Saint Antiochus’ Die is an item used in the game’s dice play, not a verified separately sold physical die or a quest objective. Gamer Guides lists its spawn ID as die_Antiocha; Deep Silver also refers to it under Dice in the Patch 1.4 notes.
