When an Altar of Lilith seems to be missing in Diablo IV, the cause is usually one of three things: the map marker is not showing, the altar object is absent at the spot where you expect it, or the altar count on your screen does not match your progress. Each has a different cause and a different check, so identify which one you are seeing before you try any fix.

Identify which symptom you have

The three symptoms look similar from the outside, but they have separate causes. Start with the one that matches what you see.

Symptom What you see Most likely cause First check
Missing map icon The altar is not marked on the world map The location has not been discovered yet. Guides state that altar locations are not flagged on the map until they are found. Compare the region with an external interactive guide to confirm the expected spot
Missing altar object The marker is present, but the altar is not visible where the map says it should be The site is hard to reach, sits underground, or is blocked by terrain or structures. A player report on Blizzard’s forums described an altar in this state. Confirm the exact position and approach, then reload (see the steps below)
Count or progress mismatch The altar total or Renown-related display differs from what you expected You are looking at a different character. Seasonal and alternate characters can have separate map, collection, and Renown states. Compare only the character you are checking, and check each state separately

A missing map icon is often expected

If an altar does not appear on the map before you have reached it, that is normal. Altar locations are not marked until the player has discovered them, so an empty spot on an undiscovered site does not mean the altar is gone. Visit the area first, then check the map again. Use an external guide only to confirm where the altar should be, not to replace discovery in the game itself.

Work through the checks in order

If the marker is present but the altar object is absent, or you cannot find the site at all, run these checks in order.

  1. Confirm the exact location. Open the region map and zoom into the area the altar belongs to. Compare its position with a region guide or checklist, because the in-game map may not show it until you discover it.
  2. Check the approach. Look for a route, structure, or terrain that blocks the way to the spot. A player on Blizzard’s forums reported an altar that was underground or otherwise inaccessible from the surface. If the spot is blocked, the altar may be present but unreachable, so you should search the surrounding area for another route.
  3. Leave the game and rejoin. A forum participant reported that an altar appeared after they left the game and returned. This was one player’s experience, not an official fix. It takes little time, so it is a reasonable check if the altar is still absent after steps 1 and 2.
  4. Recheck after reloading. If the altar is still missing after rejoining, confirm its location once more before assuming a bug. Note the character, realm, and region so you can report them if needed.

Check the right character before assuming a bug

Many reports of missing altars come from seasonal or alternate characters, not the main character you expect to see progress on. A 2024 player post on Blizzard’s forums described missing altar and map state on a seasonal character. A third-party guide notes that Renown display and rewards can differ on a new seasonal character.

Because these are separate states, do not treat a missing marker or a lower count on one character as proof that your account reward is missing. Compare the following for that character alone:

  • Map discovery: Has the location been found on this character?
  • Altar collection: Are the altars you have reached counted on this character?
  • Renown and rewards: Does the Renown display match what this character has earned?

Choose the right reference for checking locations

An in-game map and an external guide answer different questions. Use the table below to decide which one to rely on.

Tool Useful for Limits
In-game world map Confirming whether a location has been discovered on your character Altars are not marked until they are discovered, per guide coverage
Game8 regional map (2025 guide) Finding expected altar positions, with search and filter controls for pins, and a collectible checker Third-party. It cannot inspect or repair your saved game state. Its altar count for Fractured Peaks is 28 (2025 guide).
D4 Forge map page (accessed 2026) Cross-checking a total altar count across the game (160 listed) Third-party map-page data, not an official Blizzard statistic. Whether this count matches Game8’s regional figures is not stated.

Treat all counts and map positions from guides as data that may change with patches, seasons, and map updates. Confirm them against your own in-game map before relying on them.

When to report a persistent problem

If the altar is still absent after you confirm its location, check access, and reload, report it through Blizzard’s current support channels. Include the following details:

  • The character name and whether it is a seasonal or alternate character
  • The realm and region
  • The precise altar, with the nearest landmark or region name
  • Whether the map marker is missing, the altar object is missing, or the count is wrong

Player reports describe symptoms, not a confirmed official defect. No current universal fix from Blizzard was established in the reporting available for this article, so the steps above are the reliable starting point.

The steps above are based on guide and forum reports available as of October 2026. Seasonal rules and map counts can change with updates, so check the current patch notes if an instruction no longer matches what you see.
