DELTARUNE is an anagram of UNDERTALE: both names use the same letters in a different order. That wordplay is clear, but it does not, by itself, reveal why Toby Fox chose the title or prove that DELTARUNE is a straightforward sequel.
What does the title have to do with Undertale?
The two titles contain the same letters, rearranged. Nintendo Life described DELTARUNE as an anagram when reporting on an interview with Fox. That establishes the wordplay, not a confirmed explanation of his naming choice.

Does the anagram mean Deltarune is a sequel?
Not on its own. In the official DELTARUNE FAQ, Fox says the game takes place in a different world, with different characters who have lived different lives. He also says that does not rule out connections between the games. The FAQ therefore describes a distinct setting while leaving room for links; the shared letters alone do not settle the games’ story relationship.
Has Toby Fox explained why he chose the name?
The sources cited here do not establish a direct statement from Fox explaining his motive for making the title an anagram. It is safe to call the name deliberate-looking wordplay, but any specific claim about what the rearrangement means should be treated as interpretation, not confirmed canon.
