Fallout 4 can crash even without third-party mods: the cause may be game files, drivers, configuration settings, a console issue, or installed Bethesda Creations and add-ons. Start by identifying your platform and when the crash happens, then work through the checks for that platform. “No mods” does not necessarily mean no Creations or add-ons.
First, narrow down what “without mods” means
Note whether the crash happens at launch, while loading a save, or during gameplay, and whether it began after a game or driver update. Also check for a specific error message and whether the same action reliably triggers the crash. These details help distinguish a general stability problem from an issue tied to a particular file, setting, update, or installed Creation.

On Xbox and PlayStation, check for Bethesda Creations and add-ons even if you have not installed third-party mods. Bethesda says these can cause problems after a game update if their creators have not updated them.
If you play on PC
Bethesda’s PC guidance addresses several possible causes; it does not establish that any one of them explains every crash. Try the checks below in order, changing one thing at a time so you can tell whether it affects the problem.

- Check system requirements. Compare your computer with the game’s requirements. Bethesda notes that hardware can contribute to performance problems, but that general guidance does not diagnose a particular PC. See Bethesda’s “Fallout 4 has stopped working” guidance.
- Update the graphics driver. Bethesda recommends downloading the latest driver from your graphics-card maker and fully removing the previous driver before installing the new one. Follow the maker’s instructions for your specific card.
- Restore edited configuration files if applicable. If you changed Fallout 4’s INI settings, back up the files before restoring defaults. Bethesda’s “Please Stand By…” PC guidance identifies Fallout4.ini, Fallout4Custom.ini, and Fallout4Prefs.ini in the default
Documentsmy gamesFallout4directory. - Try Borderless Windowed Mode. Open the Fallout 4 launcher, choose Options, and select Borderless Windowed Mode.
- Repair or verify the game files. Use the repair or verification option in the launcher where you installed Fallout 4. Bethesda links instructions for Steam, the Microsoft Xbox app, and Epic Games Launcher from its PC crash guidance. Bethesda notes that verification may leave one or more files unverified when they are local configuration files; do not force changes to those files through verification.
- If you use Steam, clear launch options. Bethesda warns that some Steam launch options may cause crashes. Remove any custom Fallout 4 launch options and test again.
- Check DirectX. Make sure DirectX is current.
If the crash persists, Bethesda’s PC troubleshooting page recommends contacting support and providing a DxDiag file, which records system and hardware details.
If you play on Xbox
Bethesda cautions that Creations and add-ons may stop working properly after a game update until their creators update them. Its Xbox crash guidance recommends removing Creations to check whether they are involved.

- Check whether any Creations or add-ons are installed or subscribed, even if you do not consider them mods.
- Disable or delete individual Creations, or remove them to test the game without them.
- Power cycle the console, then try the game again.
- If you cannot load the game to remove content, Bethesda advises clearing reserved space.
- If the crash remains, uninstall and reinstall the game. Do not delete your save files when uninstalling.
If you play on PlayStation
Bethesda’s PlayStation crash page says that an update on November 25, 2025 resolved several issues that may have caused crashing. This is a dated statement about issues addressed in that update, not confirmation that every later or current crash has been fixed. Make sure your game is fully updated before trying further steps. - Close Fallout 4 and restart it.
- Power cycle the console.
- Disable or delete installed Creations and test again.
- If the problem continues, reinstall the game without deleting your save files.
- If those steps do not help, try launching the game offline.
What Fallout 4 updates do—and do not—tell you
Bethesda’s April 25, 2024 next-generation patch notes include stability improvements and specific platform fixes, including a PlayStation crash while loading a save made immediately before dying. The May 13, 2024 Update 2 notes list versions Xbox Series S|X 1.10.765, PlayStation 5 1.10.985, and PC, PlayStation 4, and Xbox One 1.10.984. They also describe general stability improvements and particular crash fixes, including cases involving queued mod downloads on PlayStation 5 and network reconnection after suspend/resume on Xbox One. These notes document fixes for specific conditions; they do not identify the cause of an unmodded crash on an unspecified platform.

When the crash continues
Before contacting support, record your platform, the game version if available, when the crash occurs, any error message, whether the issue started after an update, and whether Creations or add-ons are installed. On PC, include the DxDiag file Bethesda requests. Use the support page for your platform: PC “Please Stand By…”, PC “Fallout 4 has stopped working”, Xbox, or PlayStation.
