Illidan has hooves because the power of the Skull of Gul’dan transformed him into a demon. Blizzard says the change gave him curved horns, cloven hooves, and leathery wings.
How Illidan’s hooves came to be
After Illidan consumed the power of the Skull of Gul’dan, he underwent a demonic transformation. Blizzard’s official story summary directly connects that event to his new physical traits: “In so doing, he grew wicked curved horns, cloven hooves, and leathery wings and became the very thing he hunted—a demon.”
So the hooves are part of Illidan’s transformation, not merely a costume detail. Blizzard’s account establishes the cause and result, but does not give a separate biological or magical explanation for why his feet specifically became hooves.
Do all demon hunters have hooves?
No such rule is established in Blizzard’s broader descriptions. Its Demon Hunter class overview says the Illidari wield demonic powers and develop demonic features, but does not say that every demon hunter grows hooves. Illidan’s specific change is tied to consuming the Skull of Gul’dan’s power.

How Metamorphosis fits in
Blizzard describes the demon hunter ability Metamorphosis as a transformation into a hellish form that enhances the class’s powers in its demon hunter class preview. That is broader class context; it is not the event that explains Illidan’s hooves. His hoofed form is attributed to the Skull’s power and his resulting transformation into a demon.
