In Red Dead Redemption 2, Dead Eye works differently in Story Mode and Red Dead Online. In Story Mode, aim and press the right stick on a controller to activate it; in Online, it does not slow time and instead triggers the effect of your equipped Dead Eye Ability Card. Check which mode you’re playing before troubleshooting anything else.
First, check which mode you’re playing

If Dead Eye is not activating in Story Mode
Use the activation input while aiming
On a controller, aim with LT on Xbox or L2 on PlayStation, then press the right stick (R3). If you use keyboard and mouse, a remapped layout, or the input does not register, check the current prompt and the in-game Controls menu. A universal keyboard mapping is not established here, so rely on the bindings shown for your setup.

Check the Dead Eye meter
Dead Eye lasts only while its meter has capacity. If it runs out, the effect ends; replenish it before treating a short activation as a control failure. Kills and certain consumables, including Chewing Tobacco and Miracle Tonic, can replenish Dead Eye.

Consider your story progress
Dead Eye features unlock in stages. Early on, you may get time slowdown without the target-marking behavior you expect. The progression described for Story Mode moves from initial slowdown to automatic marking, then manual marking, followed by highlighting critical locations and access to additional critical locations. If Dead Eye activates but does not let you mark several targets, you may not have reached the relevant stage yet.

If Dead Eye behaves differently in Red Dead Online
Do not expect time slowdown
Online does not reproduce Story Mode’s time-slowing effect. Dead Eye activates the effect of your equipped Dead Eye Ability Card, so the result depends on your card rather than on Story Mode’s targeting progression.
Check the equipped card and its effect
Confirm that a Dead Eye Ability Card is equipped and check what it does. If you expect to mark targets, look for Paint It Black. Rockstar update notes describe perfect accuracy as limited to Paint It Black; that is a description of the card’s behavior, not a guarantee that every shot will hit.

Allow for card resource use and balance changes
Dead Eye activation and firing can consume Dead Eye. Rockstar’s Title Update 1.11 notes document historical changes, including increased initial Dead Eye consumption for cards and increased per-shot consumption for Paint It Black. Those notes explain why old guides may describe different resource use, but they do not establish exact current balance values.

There is no single established cause for every Dead Eye problem. The likely next check depends on the mode, platform and control layout, meter state, equipped Online card, and Story Mode progression. Rockstar has also documented historical Dead Eye-related fixes and balance changes, but those notes do not establish that a current activation problem is a widespread bug. One old Online fix concerned missing facial animations with an empty Dead Eye Core; it does not show that an empty core is generally the cause of Dead Eye failing to activate.
