If Windows shows “The program can’t start because xapofx1_5.dll is missing from your computer,” the affected application cannot load one of its audio components. The xapofx1_5.dll error usually points to a missing or damaged legacy DirectX runtime, not a need to upgrade DirectX 12.

You may also see one of these messages:

The code execution cannot proceed because XAPOFX1_5.dll was not found.
Reinstalling the program may fix this problem.
C:\Windows\SYSTEM32\XAPOFX1_5.dll is either not designed to run on Windows
or it contains an error.
Try installing the program again using the original installation media or
contact your system administrator or the software vendor for support.
Error status 0xc000012f.
Error loading xapofx1_5.dll.
The specified module could not be found.

The safest repair is to install Microsoft’s legacy DirectX components. Download the official DirectX End-User Runtimes (June 2010), extract it, and run DXSETUP.exe as administrator. Then verify or repair the affected game or program through its launcher.

Do not download a loose copy of xapofx1_5.dll from a random DLL website. A standalone file can be malicious, the wrong architecture, incompatible with the application, or unable to fix a missing dependency.

What is xapofx1_5.dll?

xapofx1_5.dll is part of Microsoft’s XAPOFX audio-effects technology for XAudio2. XAPOFX provides effects such as echo, equalization, mastering limiting, and reverb for Windows software and games. Microsoft’s XAPOFX overview documents these effects and their use through XAudio2.

The file is associated with the older XAudio 2.7-era DirectX SDK components. Those components are installed separately from the modern DirectX functionality included with current Windows versions. Microsoft’s DirectX End-User Runtimes (June 2010) package supplies legacy side-by-side libraries used by older games, including XAudio 2.7, XInput, XACT, D3DX, and related components.

The program that installed or bundled the file may also place a copy in its own directory. That local copy is not automatically trustworthy simply because the filename is familiar. A legitimate file should come from Microsoft’s runtime installer or the application’s original installer.

Why DirectX 12 does not necessarily fix it

Windows 10 and Windows 11 include modern DirectX components, but the DirectX version shown by Windows does not represent every optional legacy library that older games may require.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Microsoft’s DirectX installation guidance for game developers explains that optional SDK components are installed side by side. A newer DirectX version does not automatically replace every older component, and installers should run DXSETUP.exe rather than merely checking whether a particular DLL or version number exists.

Windows includes newer XAudio2 and XAPOFX functionality, while older software can still depend on the legacy XAudio 2.7 runtime. Microsoft’s XAudio2 version documentation describes this distinction.

That is why a computer can have DirectX 12 installed and still display an xapofx1_5.dll error.

The likely causes

The error normally falls into one of these categories:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
What you see Most likely cause Best first action
Missing or “not found” message when starting an older game Legacy DirectX components are absent Install the June 2010 runtime
Only one game fails Damaged or incomplete game installation Verify the game files
“Bad Image,” “not designed to run on Windows,” or 0xc000012f Wrong, damaged, or incompatible DLL Reinstall the official runtime and verify the game
The file disappears after installation Antivirus quarantine or security software interference Check Windows Security Protection history
Several unrelated programs fail Windows component corruption Run DISM, then SFC
The installer reports an internal error Installer, permissions, dependency, or log-related problem Use the offline package and inspect DirectX logs
Anti-cheat reports an untrusted system file System-file corruption or a modified local file Repair Windows components and verify the game

The first repair should still be the official legacy DirectX runtime unless the error clearly began after antivirus quarantine, a failed game update, or broader Windows damage.

Fix 1: Install the official June 2010 DirectX runtime

This is the most appropriate repair for a missing xapofx1_5.dll message.

The offline package is useful because it contains the legacy files locally and does not depend on downloading each component during setup.

Download and extract the package

  1. Open Microsoft’s DirectX End-User Runtimes (June 2010) download page.
  2. Download directx_Jun2010_redist.exe.
  3. Save it to a local folder such as Downloads.
  4. Double-click the downloaded file.
  5. Accept the license agreement.
  6. When asked for an extraction location, create a simple local folder such as:

text
C:\DXRedist

  1. Allow the package to extract its files.

This first program is only the extractor. It does not complete the DirectX installation by itself.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Run DXSETUP.exe

  1. Open File Explorer.
  2. Browse to C:\DXRedist, or to the folder you selected.
  3. Find DXSETUP.exe.
  4. Right-click it.
  5. Select Run as administrator.
  6. Accept the license terms.
  7. Let setup complete.
  8. Restart Windows.

After restarting, launch the affected game or application again.

How to tell whether it worked

The repair worked if the program starts normally or if the missing xapofx1_5.dll message disappears. The program may still show a different error if another dependency is missing, so record any new message instead of repeatedly reinstalling the same package.

If the same error remains:

  • Run DXSETUP.exe once more as administrator.
  • Verify the game files.
  • Check whether the application is 32-bit or 64-bit.
  • Look for related missing files such as XAudio2_7.dll, X3DAudio1_7.dll, XINPUT1_3.dll, or older D3DX files.
  • Continue to the architecture and game-repair sections below.

Fix 2: Try the DirectX web installer when appropriate

Microsoft also provides the DirectX End-User Runtime Web Installer, whose downloaded file is dxwebsetup.exe.

Use the web installer when:

  • The computer has a reliable internet connection.
  • You want setup to download the required legacy components.
  • You do not need an offline installer for several computers.
  • The June 2010 package has not yet been downloaded.

The basic procedure is:

  1. Open Microsoft’s web installer page.
  2. Download dxwebsetup.exe.
  3. Right-click the file and select Run as administrator.
  4. Follow the setup wizard.
  5. Restart Windows.
  6. Retry the affected application.

If the web installer reports an internal system error, freezes, or fails repeatedly, switch to the offline June 2010 package. Do not keep repeating the failing web installation without checking the logs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The web installer and offline package install legacy components alongside modern DirectX. They do not downgrade DirectX 12 or replace the current Windows graphics runtime.

Fix 3: Use the DirectX installer bundled with the game

Many older games include their own DirectX redistributable in a folder named Redist, DirectX, or __Installer.

If the game has one, use that installer first:

  1. Open File Explorer.
  2. Browse to the game’s installation folder.
  3. Look for folders named Redist, DirectX, or __Installer.
  4. Open the folder.
  5. Find DXSETUP.exe.
  6. Right-click it and select Run as administrator.
  7. Finish the installation.
  8. Restart Windows.
  9. Launch the game again.

A publisher-supplied installer may contain the specific legacy component set expected by that game. If it fails, use Microsoft’s June 2010 offline package and inspect the DirectX logs.

Do not simply copy one DLL from the game’s folder to System32. The installer may contain several matching dependencies that need to be installed together.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Fix 4: Check 32-bit and 64-bit compatibility

A 32-bit game needs 32-bit runtime components, even when it is running on 64-bit Windows. A native 64-bit program needs 64-bit components.

On 64-bit Windows:

  • C:\Windows\System32 normally contains native 64-bit system files.
  • C:\Windows\SysWOW64 normally contains 32-bit system files.

These names are confusing, but this is how Windows maintains compatibility. Microsoft’s file-system redirector documentation explains the behavior.

Determine whether the program is 32-bit or 64-bit

The easiest method is usually to check the program’s documentation or launcher requirements. You can also inspect the executable with a trusted system or development tool, but do not guess based only on the folder name.

Clues that the application is older and probably 32-bit include:

Free tools Windows power users keep installed

One-click scans. No signup required.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • It was designed for an older Windows release.
  • It installs under a legacy game directory.
  • Its launcher or executable is clearly labeled x86.
  • Other legacy DirectX files are missing from the same installation.

You normally do not need to select individual x86 or x64 files when using the official DirectX installer. Install the complete runtime normally and let the application load the architecture it needs.

Why the file can exist and still fail

A file existing at C:\Windows\System32\XAPOFX1_5.dll does not prove that the failing program can use it. The file may be:

  • The wrong architecture.
  • Corrupted.
  • Replaced by an incompatible local copy.
  • Missing one of its dependencies.
  • Blocked or quarantined.
  • Shadowed by a damaged DLL in the game directory.

A 32-bit process cannot load a 64-bit DLL, and a 64-bit process cannot load a 32-bit DLL. Do not copy files between System32 and SysWOW64 manually.

Fix 5: Verify or repair the game installation

If only one game fails, the game installation is a strong suspect. This is especially true after an interrupted update, mod installation, launcher crash, or partial download.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Steam

  1. Open Steam.
  2. Select Library.
  3. Right-click the affected game.
  4. Select Properties.
  5. Select Installed Files.
  6. Select Verify integrity of game files.
  7. Wait for the check to finish.
  8. Restart Steam.
  9. Launch the game.

Steam may replace damaged or missing local files. If the error returns after verification, install the official DirectX runtime separately because game-file verification may not restore a system-wide legacy component.

Epic Games Launcher

  1. Open Epic Games Launcher.
  2. Select Library.
  3. Find the affected game.
  4. Select the three dots beside the game.
  5. Select Manage.
  6. Select Verify.
  7. Wait for verification to complete.
  8. Restart the launcher.
  9. Launch the game.

If the game has its own DirectX installer, run that installer after verification.

Other launchers

For GOG or another launcher, look for a Repair, Verify, Scan, or Reinstall option. If no repair option exists, reinstall the program using its original publisher installer.

Do not remove the entire game before trying verification unless the installer itself is damaged or verification does not resolve the problem.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Fix 6: Repair a Bad Image or 0xc000012f error

A Bad Image message is different from a simple missing-file message. It indicates that Windows found a file but could not load it as a valid compatible module.

The same applies to error 0xc000012f. It does not prove that the file is malware, and it does not necessarily mean the file is absent.

Try these steps in order:

  1. Close the game or program.
  2. Verify or repair the game installation.
  3. Run the official June 2010 DirectX installer again.
  4. Restart Windows.
  5. Check whether antivirus software quarantined the file.
  6. Run DISM and SFC if other programs also report Bad Image errors.
  7. Avoid replacing the file with an unverified download.

If the error names a file in the game directory rather than the Windows directory, the local game copy may be damaged. Verification or reinstalling the game is usually more appropriate than modifying Windows system folders.

Public file metadata may show a version or size for a particular copy of this DLL, but those details are identification guidance rather than a universal checksum guarantee. Architecture, package revision, and application deployment can affect the copy present on a particular computer.

What’s actually slowing this PC down?

Pick the symptom - the matching free tool is one click away.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Fix 7: Check Windows Security quarantine

Security software can remove or quarantine a file after installation. This is more likely to matter when the file disappears immediately after the DirectX installer or game installer finishes.

To inspect Windows Security:

  1. Open Start.
  2. Type Windows Security.
  3. Open the app.
  4. Select Virus & threat protection.
  5. Select Protection history.
  6. Review recent detections.
  7. Open an entry that refers to the affected file or installer.
  8. Read the detection details before taking action.

Microsoft explains the process in its Protection History guidance.

Restore or allow a file only when all of the following are true:

  • It came from Microsoft’s official DirectX package or the trusted game installer.
  • The detection clearly identifies the file you expected to install.
  • The publisher and source can be verified.
  • You have no other signs of malware.

Do not allow an unknown DLL simply because its filename matches the error. If you are unsure, leave the file quarantined and run a full security scan.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

After restoring a trusted file, reinstall the complete DirectX runtime or repair the game so related components are restored together.

Fix 8: Handle anti-cheat “Untrusted system file” errors

Some games use anti-cheat software that rejects a damaged, modified, or unexpected system file. Epic’s Easy Anti-Cheat guidance treats an “Untrusted system file” message as a possible system-level corruption problem.

Try this sequence:

  1. Verify the game files through its launcher.
  2. Check Windows Security Protection history.
  3. Restart Windows.
  4. Run DISM and SFC as described below.
  5. Reinstall the official DirectX runtime.
  6. Launch the game again.

Do not download a replacement DLL from an unknown source to satisfy anti-cheat software. A modified file can create another integrity problem.

Fix 9: Repair Windows components with DISM and SFC

Use DISM and SFC when:

  • Several unrelated programs report Bad Image errors.
  • Windows system files are failing.
  • Anti-cheat rejects multiple system files.
  • The DirectX installer cannot use required Windows components.
  • The problem began after an interrupted update or system failure.

These commands are not a replacement for the legacy DirectX runtime. They repair Windows components, while xapofx1_5.dll is generally supplied through the optional legacy DirectX package.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Run DISM first

  1. Open Start.
  2. Type Command Prompt.
  3. Right-click Command Prompt.
  4. Select Run as administrator.
  5. Select Yes if User Account Control asks for permission.
  6. Enter:

bat
DISM /Online /Cleanup-Image /RestoreHealth

  1. Press Enter.
  2. Wait for the operation to finish.
  3. Keep the window open even if progress appears paused.

When DISM finishes, note whether it reports that the restore operation completed successfully or that it could not repair the image.

Run SFC second

In the same elevated Command Prompt, enter:

sfc /scannow

Press Enter and wait for verification to reach 100 percent.

Restart Windows after both commands finish. Then run the DirectX installer again and retry the affected application.

Microsoft documents this order and these commands in its System File Checker and DISM instructions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If DISM or SFC reports that it could not repair files, repeat the procedure only as directed by the result. Do not delete system DLLs manually before running repairs.

Outbyte PC Repair can reduce the guesswork when you are repeatedly hunting through Windows repair results and cannot tell what changed; its free scan identifies reported issues, while repair actions are handled by the full version. It is optional, and when the file belongs to one specific game, repairing or reinstalling that game remains the definitive fix.

Fix 10: Install Windows updates

Windows updates can repair operating-system components and resolve compatibility problems, although updating Windows is not normally the direct installation method for the legacy DirectX file.

  1. Open Start.
  2. Select Settings.
  3. Select Windows Update.
  4. Select Check for updates.
  5. Install available updates.
  6. Restart Windows.
  7. Try the game again.

Microsoft’s Windows Update instructions provide the current navigation.

Free tools Windows power users keep installed

One-click scans. No signup required.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If the error remains after updating, install the June 2010 DirectX runtime. A current Windows build can still lack optional legacy components required by older software.

Fix 11: Reinstall a graphics or audio driver only when symptoms support it

A graphics driver is not the normal source of xapofx1_5.dll. Driver work is justified when the DirectX error appears together with:

  • Display-device failures.
  • Audio-device failures.
  • Crashes in several games.
  • Screen corruption.
  • Problems that started immediately after a driver update.
  • Device Manager warnings.

To reinstall a device driver through Device Manager:

  1. Right-click Start.
  2. Select Device Manager.
  3. Expand Display adapters for a graphics device or Sound, video and game controllers for an audio device.
  4. Right-click the affected device.
  5. Select Uninstall device.
  6. Confirm the removal if prompted.
  7. Restart Windows.
  8. Allow Windows to reinstall the driver, or install the correct driver supplied by the device manufacturer.

Microsoft documents this process in its Device Manager driver guidance.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Outbyte Driver Updater can make the manual process easier when you are checking many devices and trying to identify which driver is stale; its free scan reports the driver issues, while driver installation is performed by the full version. It is optional, and a driver reinstall should not replace the DirectX runtime repair when this DLL is the only failure.

Fix 12: Check Windows Features only when the program specifically requires one

Windows optional features are not the normal source of xapofx1_5.dll. Do not enable random legacy features as a substitute for installing the DirectX runtime.

If a game specifically reports that it needs a Windows feature such as DirectPlay, you can inspect optional features as follows:

  1. Press Windows key + R.
  2. Enter:

text
optionalfeatures

  1. Press Enter.
  2. Review the feature named by the game’s error or documentation.
  3. Enable only that requested feature.
  4. Select OK.
  5. Restart Windows if prompted.

Enabling an unrelated optional feature will not reliably restore xapofx1_5.dll. The correct repair for this file remains the official legacy DirectX installer.

What’s actually slowing this PC down?

Pick the symptom - the matching free tool is one click away.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Fix 13: Read the DirectX setup logs

If DXSETUP.exe fails, inspect the logs before changing system files.

Check these locations:

C:\Windows\DXError.log
C:\Windows\DirectX.log

Depending on Windows and the installer version, DirectX-related logs may also be under:

C:\Windows\Logs

The logs can distinguish between:

  • A missing dependency.
  • A permissions problem.
  • A failed LoadLibraryEx() operation.
  • An architecture mismatch.
  • A damaged installer.
  • A component that could not be copied.

An error such as “The specified module could not be found” does not always mean that xapofx1_5.dll itself is absent. Windows may be unable to load a dependency required by that DLL.

If the installer says that a newer DirectX version is already installed, continue with the offline package and run DXSETUP.exe. Modern DirectX and optional legacy components are separate, so that message does not prove that the older runtime is available.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not use regsvr32 for this file

regsvr32 is not the correct repair for xapofx1_5.dll.

The command is intended for self-registering OLE or ActiveX-style DLLs that expose registration entry points such as DllRegisterServer. XAPOFX is an audio-effect library used through XAudio2 APIs. Microsoft’s regsvr32 documentation and XAPOFX documentation describe different purposes.

Running a command such as this is not a valid fix:

regsvr32 xapofx1_5.dll

It may produce an error because the DLL is not designed for COM registration. Install the runtime or repair the application instead.

Is manually copying xapofx1_5.dll ever safe?

Manual copying is appropriate only in limited situations:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • A game publisher explicitly supplies the matching DirectX redistributable.
  • A developer is deploying an application-local runtime.
  • An administrator extracts the file from Microsoft’s official package as a documented recovery step.

For ordinary users, installing the complete runtime is safer because it restores related dependencies and the correct architecture together.

Avoid these actions:

  • Downloading the file from an unknown DLL website.
  • Copying an x86 file into a 64-bit application directory.
  • Copying an x64 file into a 32-bit application directory.
  • Overwriting System32 or SysWOW64 casually.
  • Deleting the existing DLL before reinstalling the runtime.
  • Allowing an antivirus detection without verifying the source.

A file with the correct name can still be corrupted, unsigned, modified, or incompatible with the application.

Fix 14: Use System Restore for a recent, identifiable change

System Restore is appropriate when the error clearly began after a recent:

  • Driver installation.
  • Windows update.
  • Game utility or cleanup tool.
  • Security-software action.
  • System configuration change.

To open System Restore:

  1. Press Windows key + R.
  2. Enter:

text
rstrui.exe

  1. Press Enter.
  2. Select a restore point from before the problem began.
  3. Select Next.
  4. Review the affected programs and drivers.
  5. Select Finish.
  6. Allow Windows to restart.

Microsoft explains the process in its System Restore guidance.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

System Restore normally affects system files, registry settings, drivers, and installed programs rather than personal documents. Use it only when the timing is clear, because it can remove software installed after the selected restore point.

When to reinstall the affected program

Reinstall the game or application when:

  • Verification reports damaged files repeatedly.
  • The local game directory contains a bad DLL copy.
  • The error started immediately after a failed update.
  • The program installer itself is damaged.
  • The program is the only application that fails.
  • The official DirectX runtime installs successfully but the same application still cannot start.

Use the original publisher installer or the launcher’s own reinstall option. Save configuration files and game saves first if the program stores them inside its installation directory.

If reinstalling the application restores its bundled DirectX installer, run that installer as administrator before launching the program.

A practical repair order

For most users, follow this order:

  1. Close the affected program.
  2. Download Microsoft’s June 2010 DirectX End-User Runtime.
  3. Extract it to a local folder.
  4. Run DXSETUP.exe as administrator.
  5. Restart Windows.
  6. Verify the game files through Steam, Epic, or the relevant launcher.
  7. Check Windows Security Protection history.
  8. Confirm that the program’s architecture is compatible with the installed runtime.
  9. Run DISM followed by SFC if other programs also fail.
  10. Install Windows updates.
  11. Reinstall drivers only when display or audio symptoms justify it.
  12. Use System Restore only when a recent system change clearly caused the problem.

FAQ

Is xapofx1_5.dll a virus?

The file is a legitimate Microsoft DirectX component, but malware can use a familiar filename. Source matters. A copy installed by Microsoft’s DirectX package or the original game installer is safer than a copy downloaded from an unknown website.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Check the file’s location, digital signature, antivirus results, and installation source. Do not assume that every file with this name is genuine.

Which DirectX package contains it?

The legacy DirectX End-User Runtime components are supplied through Microsoft’s June 2010 offline package. Microsoft’s web installer can also download required legacy components.

Use the offline package when the web installer fails or the computer has a restricted connection.

Why does DirectX 12 not fix the error?

DirectX 12 is the modern Windows graphics and multimedia API version. Older games can still require optional side-by-side libraries from the legacy DirectX SDK family, including XAudio 2.7-related components.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Installing the June 2010 runtime adds those components. It does not replace or downgrade DirectX 12.

Should I install the 32-bit or 64-bit version?

Install the complete official runtime and let the application use the required architecture. Older 32-bit games need x86 components, while native 64-bit programs need x64 components.

Do not move DLLs manually between System32 and SysWOW64.

Why does the file exist in System32 but the error remains?

The copy may be damaged, incompatible, blocked, missing a dependency, or irrelevant to the architecture of the failing process. A game-folder copy can also take precedence over the Windows copy.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Verify the game, reinstall the official runtime, and inspect the exact path in the error message.

Should I use regsvr32?

No. xapofx1_5.dll is an XAudio2 audio-effect library, not a self-registering COM or ActiveX control.

What does “Bad Image” or 0xc000012f mean?

It means Windows found a module that it could not load as a valid compatible binary. The file may be damaged, the wrong architecture, replaced by an incompatible copy, or dependent on another missing or corrupted file.

Reinstall the runtime, verify the game, check antivirus history, and use DISM and SFC when broader Windows corruption is present.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What if the installer reports “An internal system error occurred”?

Use the offline June 2010 package, extract it to a normal local folder, and run DXSETUP.exe as administrator. Then inspect C:\Windows\DXError.log, C:\Windows\DirectX.log, and, where applicable, C:\Windows\Logs.

The error may involve permissions, a missing dependency, an architecture mismatch, or a damaged installer rather than this DLL alone.

Should I enable a Windows Feature?

Not for this DLL by itself. Optional Windows features such as DirectPlay address separate compatibility requirements. Enable one only when the game specifically names it.

Can a graphics driver fix xapofx1_5.dll?

Usually not. Reinstall a graphics or audio driver only when the problem also includes device failures, display corruption, audio failures, or crashes across multiple games.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What details should I provide if the error remains?

Record:

  • The exact error text.
  • The program or game name.
  • The full path shown in the message.
  • Whether the program is 32-bit or 64-bit.
  • Your Windows version.
  • Whether the problem affects one program or several.
  • Whether the file exists and where.
  • Whether antivirus quarantined it.
  • Whether the June 2010 DirectX installer completed.
  • The relevant DirectX setup log entries.
  • Whether launcher verification found damaged files.

Those details separate a missing legacy runtime from a bad local copy, dependency failure, Windows corruption, or an application-specific installation problem.
