Aliens: Dark Descent has five difficulty choices: Story, Medium, Hard, Nightmare, and Custom. Choose Story if you want lower pressure and to focus on the narrative; choose a higher preset if you want a tougher experience; choose Custom if you would rather tune individual challenge settings. The game’s exact preset-by-preset numerical effects are not established in the available official information, so check the descriptions shown in your game before deciding.
What do the difficulty options change?
The presets provide different levels of challenge, while Custom lets you adjust aspects of that challenge. Secondary guides identify enemy health and alien aggression or its timing as settings players can tune, but they do not provide a complete, authoritative specification of every option.

That means there is no reliable basis here for assigning exact health, damage, aggression, infestation, timer, or resource values to each preset. Read the in-game descriptions for the settings and values available in your version rather than relying on an assumed multiplier or a preset comparison from an unverified source.
How to choose a difficulty
Story
Consider Story if you want less pressure and would prefer to concentrate on the narrative. In a PlayStation Blog interview, a Tindalos Interactive representative said the team added lower difficulty levels “for players who want to focus on the story.”
#1 Best Overall
- Face off in a gripping original Alien story against iconic Xenomorph creatures ranging from Facehuggers to Praetorians, Alien Queens and many more, including rogue human commandos and a brand-new threat unique to this Alien storyline
- Lead strategically and change squad tactics from mission to mission, carefully managing your soldiers’ health, resources, and sanity, to avoid permanent team losses and mental breakdowns
- Forge unique paths for survival in a persistent world, uncovering shortcuts, creating safe zones and setting up motion trackers to stay one step ahead of these creatures
- Assemble and level up squads composed of 5 starting Marines classes, with dozens of specializations, unique abilities and weapons
Medium
Medium is a reasonable choice if you want a standard challenge rather than explicitly opting for the lowest or more demanding named presets. The available sources do not verify how its individual settings compare numerically with Story or the higher difficulties.
Hard and Nightmare
These names indicate more demanding choices, but the available sources do not establish exactly which values change, or by how much, on either preset. If you are deciding between them, use the in-game descriptions as your guide rather than assuming a particular increase in enemy health, damage, or escalation.

Custom
Choose Custom if you want more direct control over the challenge. Guides describe options related to enemy health and alien aggression timing; treat those as guide-level explanations, not a full official settings list. Adjust one factor at a time and consult the on-screen label so you can see what each change is intended to do.
Other ways to make the experience more manageable
Difficulty is not the only control mentioned by the game’s official listings. Steam’s store page lists adjustable difficulty and save-anytime as accessibility features. A Tindalos Interactive representative also described the option to turn slowdown mode into a full pause in the PlayStation Blog interview. These options may help some players, but the sources do not establish that they address every accessibility need.

Can you change difficulty during a campaign?
The available official sources do not settle whether difficulty can be changed after starting a campaign. Community claims on this point conflict, so do not rely on an assumption either way; check the game’s current menus or documentation before starting if the ability to change later matters to you.

The available authoritative information does not establish whether Custom difficulty changes achievement eligibility. If achievements matter to your run, check current in-game information or official documentation rather than treating community claims as definitive.
