Fearful and Frightened are not interchangeable in Baldur’s Gate 3. Ordinary Frightened stops a character from moving and gives disadvantage on ability checks and attack rolls. The Fear spell applies Fearful instead: its target runs from the source, cannot take additional actions, and may get another saving throw if it ends its turn out of sight. Checking the exact condition name tells you which rules—and which counter—apply.
What does Frightened do in BG3?
The basic Frightened condition prevents movement and imposes disadvantage on ability checks and attack rolls. “Cannot move” is the key combat restriction: a Frightened character cannot use its movement to close distance, retreat, reposition, or reach a better angle. The disadvantage also affects attacks and ability checks made while the condition remains active, so a character that is technically still able to act can be much less reliable.

The condition page lists sources such as Form of Dread, Meenlock Fear, Wrathful Spirits, and other variants. Those names identify fear-related mechanics, but they do not automatically give each source the same movement, action, duration, or recovery rules.
That distinction matters in combat: a Frightened character cannot reposition, while the separate Fearful condition from the Fear spell makes the target flee from the source. When you need to choose a response, start by checking the name shown on the affected character and then inspect the detailed entry supplied by that source.

How to check which effect landed
- Pause long enough to read the condition label on the character’s portrait or examine the combat log.
- Confirm whether the label says Frightened or Fearful.
- Read the listed movement, action, saving-throw, and duration text instead of relying on the enemy’s animation or the ability’s name.
- Choose a counter that matches that exact entry. A remedy that addresses the Frightened status group may not automatically describe every effect called Fearful.
This quick check prevents a common mistake: spending a turn trying to hide from the source of an ordinary Frightened effect when the tooltip does not offer a repeat save, or expecting a Fearful target to stand still when the spell specifically makes it run.
What is the difference between Fearful and Frightened?
Fearful is the condition applied by the level 3 Fear spell; it has a more specific control pattern than ordinary Frightened. Both impose disadvantage on ability checks and attack rolls, but their movement and recovery rules differ.
| Condition | Movement and actions | Attack rolls and ability checks | Recovery detail |
|---|---|---|---|
| Frightened | The affected entity cannot move. | Disadvantage on both. | Use an applicable removal effect or prevent the condition; details can depend on the source. |
| Fearful, from Fear | The target runs from the source of fear and cannot take additional actions. | Disadvantage on both. | If the target ends its turn unable to see the source, it can make another saving throw. |
Fearful therefore combines the attack and check penalty shared with Frightened with forced flight and a stronger action restriction. The target’s turn is not merely a stationary, disadvantaged turn: it must run from the source and cannot take additional actions. That can pull an enemy away from its formation, break its intended position, or make it abandon a useful location.
Fearful also has a built-in recovery route that is easy to miss. The target can make another saving throw only when it ends its turn unable to see the source. Simply moving behind cover during the turn is not enough if the target still ends that turn with the source in sight. Conversely, breaking sight at the end of the turn can create a genuine chance to shake off the effect on the next save.
Fear also causes the target to drop its weapon for one turn, according to the spell entry. Treat that as a consequence of this spell, not as a universal effect of every Frightened condition. A Frightened character affected by another source should not be assumed to drop a weapon unless that source’s own tooltip says so.
Why the wording changes your plan
With ordinary Frightened, the immediate problem is immobility plus disadvantage. You may need to remove the condition, protect the character from another application, or simply wait while using whatever actions remain possible. With Fearful, movement is not merely unavailable; it is directed away from the caster or other source. If the source is visible, the target may spend its turn fleeing. If the target can end the turn out of sight, it may earn a repeat save.

That means the same battlefield can reward opposite positioning decisions. A target affected by Fearful may benefit from reaching a corner, doorway, or other visual obstruction at the end of its turn. A character with ordinary Frightened has no equivalent line-of-sight recovery rule unless the specific effect says otherwise.
How does the Fear spell work?
Fear is a level 3 illusion spell that applies Fearful after a failed Wisdom saving throw. Its cone, concentration requirement, and action cost make positioning and timing important.
| Property | Fear spell |
|---|---|
| Spell level and school | Level 3, illusion |
| Saving throw | Wisdom |
| Area | 9 m / 30 ft cone |
| Cost | Action and a level 3 spell slot |
| Duration mechanic | Concentration |
| Condition applied | Fearful |
The 9 m (30 ft) cone means the caster must line up targets rather than select any creature anywhere in a radius. A caster spends an Action to cast it and a level 3 spell slot, then must maintain concentration. If concentration ends, the spell’s ongoing effect ends with it; protect the caster if you are relying on Fear to control several enemies.

The Wisdom saving throw is made when the spell is applied. A failed save produces Fearful and its associated penalties, flight, action restriction, and weapon-drop consequence. A target that later ends its turn unable to see the source receives another opportunity to save, so the spell is not necessarily permanent even if the first save fails.
Because Fearful is tied to this spell’s specific behavior, the spell’s line-of-sight recovery opportunity should not be assumed for every source of Frightened. The current condition tooltip is the most useful guide to the effect in a particular encounter. Do not infer the spell’s cone, dropped-weapon rule, or repeat-save trigger from a different enemy ability merely because both effects use fear imagery.
Practical positioning around Fear
If you are casting Fear, angle the cone so that affected enemies begin with limited routes toward the source and must travel away from it. If you are defending against it, look for a path that lets a Fearful character finish its turn without seeing the caster. The recovery rule cares about the target’s end-of-turn sight line, not just whether the target passed behind an object briefly during movement.
Because Fear uses concentration, attacking or otherwise pressuring the caster can be a useful answer when a direct cleanse is unavailable. That is a tactical response to the spell’s concentration requirement, not a replacement for reading the target’s condition: the affected characters still follow the Fearful rules until the spell ends or they recover.
How do I get rid of an active fear effect?
Match the remedy to the condition name. For Fearful from Fear, line of sight can create another saving throw; for the broader Frightened status group, documented direct removal options include Dispel Evil and Good, Stillness of Mind, and Calm Emotions.
If the target is Fearful
- Check whether the effect is Fearful rather than Frightened.
- Move or otherwise position the target so it ends its turn unable to see the source of Fear.
- If that condition is met, the target can make another saving throw to shake off the effect.
This is a conditional chance to recover, not an automatic end to the spell. The target still has to end its turn out of sight, and the additional saving throw can fail. If the target cannot reach cover safely, a removal or prevention tool may be more dependable.
Fear requires concentration, and the effect’s spell entry separately describes the dropped-weapon consequence. If a character dropped a weapon, account for that lost equipment for the turn; do not treat the drop as proof that every Frightened source has the same behavior.
If the target is Frightened
Consider a direct removal option available to your party. The documented tools for the status group include:
- Dispel Evil and Good: listed as a direct removal effect.
- Stillness of Mind: listed as a direct removal feature.
- Calm Emotions: can end Charmed, Frightened, or enraged conditions when cast on an already affected target.
These are removal answers for an effect that is already present. They are different from buffs that prevent a future fear application. Check the action, spell, feature, and targeting requirements in the current tooltip before committing a turn, especially when the affected character cannot move and may be unable to reach a desired position.

Calm Emotions is a concentration spell with a 6 m / 20 ft radius and a 10-turn duration. Its area and concentration requirement matter when choosing where and when to cast it. It can both end Charmed, Frightened, or enraged conditions on an already affected target and prevent Frightened during its duration, so it can serve as an emergency cleanse or a short-term protective zone. Make sure the affected ally is inside the spell’s radius and remember that maintaining concentration may compete with another concentration spell you planned to use.
Removal versus waiting for a save
For Fearful, breaking sight may cost movement but does not necessarily consume a separate spell slot. It is attractive when the character can reach cover and the source cannot immediately restore line of sight. For ordinary Frightened, no equivalent universal repeat-save route is documented; a listed removal tool or a condition-specific rule is the safer assumption.
Do not confuse “the effect may end after another save” with “the spell ends as soon as the target hides.” The target must end its turn unable to see the source, and the resulting saving throw still determines whether Fearful is removed.
How can I prevent or resist fear before it lands?
Prevention can save a character from losing movement, attacks, checks, or actions during an enemy turn. Choose a targeted spell, a nearby ally’s aura, a party buff, a class feature, or an immunity source according to who is threatened and what your party has available.

Heroism: an early targeted answer
Heroism is a level 1 spell that makes its target unable to become Frightened for 10 turns and grants 5 temporary hit points each turn. It is useful before an encounter or before an enemy known to use fear gets a turn. Its limitations are equally important: it is targeted rather than a party-wide aura, and it requires concentration. Casting it on one front-line character may leave the rest of the group vulnerable.
Aura of Courage: stay near the Paladin
Aura of Courage is a level 10 Paladin feature. While the aura is active, the Paladin and nearby allies cannot become Frightened. The listed radius is 3 m (10 ft), so coverage depends on formation and movement. The aura disappears if the Paladin falls unconscious, which makes the Paladin’s survival part of the protection plan. Allies who move outside the radius no longer receive that nearby-aura coverage.
Heroes’ Feast: long-rest party preparation
Heroes’ Feast grants immunity to the Frightened status group, increases maximum hit points by 12, and gives advantage on Wisdom saving throws. Its buff lasts until long rest according to the spell entry. This makes it a preparation tool for a full adventuring stretch rather than a quick reaction during one turn, and it protects multiple party members at once. The spell’s level 6 requirement means it is not an early-game answer.
Protection from Evil and Good, class features, and passive sources
Protection from Evil and Good is listed as granting immunity to the Frightened status group. Mindless Rage provides the same kind of protection through a Berserker Barbarian passive while the character is Frenzied, and it becomes available at Berserker level 6. Nature’s Ward is also listed as a passive immunity source. These options differ in access and activation: one is a spell, one depends on a subclass state, and one is a passive feature. Read the current tooltip to confirm the precise condition and duration in your version.
Equipment and condition-based immunity
The status-group page lists Cloth of Authority, Cerebral Citadel Armour, and Sarevok’s Horned Helmet among items granting immunity to the group. Item-specific behavior can change with patches or differ from what a name suggests, so check the item’s current tooltip before building a character around it. Psionic Ward, Psionic Ward Aura, and Spore Servant are also listed as conditions that grant immunity to the whole group; their availability depends on the character state or encounter that supplies them.

Calm Emotions also prevents Frightened during its duration. Unlike a purely preventive buff, it can be cast on an already affected target to end the listed conditions, making it a flexible option when a fear effect has already landed. Its 6 m (20 ft) radius, 10-turn duration, and concentration requirement still apply, so plan placement and concentration accordingly.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Which counter should I use in combat?
Use the cheapest reliable answer for the character and condition at risk: a recovery roll may cost no extra spell slot, while a preventive effect can protect an ally before the enemy acts. These distinctions help prioritize your response.
- Fearful, and the source is visible: if the target can safely end its turn out of sight, that may enable the repeat saving throw. Make sure the target actually finishes the turn unable to see the source.
- Frightened is already active: use an available direct-removal option such as Calm Emotions, Dispel Evil and Good, or Stillness of Mind. Ordinary Frightened does not automatically provide Fear’s hiding-and-saving-throw route.
- A fear effect is likely but has not landed: consider Heroism for one target or a broader source of protection, such as Aura of Courage or Heroes’ Feast.
- The encounter repeatedly threatens the same party: persistent or party-wide protection may avoid spending a turn to recover each time, if its class, spell, or item requirements are met.
- The Paladin is carrying the defense: keep allies within the 3 m (10 ft) Aura of Courage radius and avoid allowing the Paladin to become unconscious.
- You are preparing for a dangerous adventuring segment: Heroes’ Feast can cover the party until long rest while also adding 12 maximum hit points and advantage on Wisdom saving throws.
- The tooltip names a different fear effect: read its own entry; do not apply Fear’s Fearful rules or the basic Frightened rules without checking.
Action economy and concentration
Prevention often wins on action economy. Casting Heroism before the enemy’s fear effect can preserve the protected character’s future movement and attacks instead of spending a later turn recovering. Calm Emotions is more flexible because it can cleanse an existing condition, but it still uses a spell action and concentration and requires the target to be inside its radius.
Recommended Free Tools
Fear itself also uses concentration. If you are using it offensively, protect the caster and choose a cone that forces enemies away while preserving the possibility that they cannot see the source at the end of their turns. If you are fighting it, breaking the caster’s concentration can be valuable, but do not rely on that alone when a direct removal or immunity option is available.
Single-target protection versus group coverage
Heroism is the focused option: one chosen target receives prevention and recurring temporary hit points. Aura of Courage covers the Paladin and allies within 3 m (10 ft), but the group must remain close and the Paladin must stay conscious. Heroes’ Feast is a party-scale preparation buff that lasts until long rest. Calm Emotions has a 6 m (20 ft) radius and can affect multiple creatures in that area, but its concentration and positioning requirements make it less persistent than a long-rest buff.
Condition scope matters
Several listed protections are described as immunity to the Frightened status group. Fear applies Fearful, a separately named condition. The available descriptions do not establish that every immunity to the Frightened group blocks Fearful as well. Before relying on a protection against the Fear spell, check the current tooltip for its exact scope instead of treating the two labels as synonyms.
Fear mechanics are documented in community-maintained game references and can change with patches. If an ability or item behaves differently in your game version, use its current in-game tooltip as the final check.

Does every fear-themed ability in BG3 work like the Fear spell?
No. Fear is documented as applying Fearful, while other fear-related sources can use their own condition entries. A familiar name or animation is not enough to establish that an effect shares the spell’s movement, action, or recovery rules.
What does ordinary Frightened prevent?
The basic condition prevents movement and gives disadvantage on ability checks and attack rolls. The source’s own entry can add or change details, so inspect the exact tooltip.

What is the special recovery rule for Fearful?
A Fearful target can make another saving throw if it ends its turn unable to see the source of the Fear spell. Hiding briefly during movement is not sufficient if the target still ends the turn with the source in sight.
Does the Fear spell make a target drop its weapon?
The Fear spell entry lists a weapon drop lasting one turn. That is a spell-specific consequence, not a rule to assume for every effect that applies Frightened.
How do I remove Frightened after it lands?
Documented direct options include Dispel Evil and Good, Stillness of Mind, and Calm Emotions. Calm Emotions can end Charmed, Frightened, or enraged conditions when cast on an affected target.
Does Calm Emotions prevent fear as well as remove it?
Yes. It prevents Frightened during its duration and can immediately end Charmed, Frightened, or enraged conditions on a target already affected. It is a concentration spell with a 6 m (20 ft) radius and a 10-turn duration.
What makes you immune to fear in Baldur’s Gate 3?
Listed sources include Heroes’ Feast, Protection from Evil and Good, Aura of Courage, Mindless Rage while Frenzied, Nature’s Ward, several equipment pieces, and the Psionic Ward, Psionic Ward Aura, and Spore Servant conditions. Verify the current tooltip, especially when distinguishing Frightened-group immunity from the separately named Fearful condition.
Do all listed fear immunities cover Fearful too?
The listed immunity sources are described as applying to the Frightened status group, while Fear applies Fearful. The available descriptions do not establish that every group immunity blocks Fearful, so check the relevant current tooltip rather than assuming the names are equivalent.
Do these 3 things before closing this tab:

Aura of Courage protects the Paladin and nearby allies within 3 m (10 ft) while the Paladin remains conscious. Heroes’ Feast is a level 6 party buff lasting until long rest and also grants 12 additional maximum hit points plus advantage on Wisdom saving throws. Calm Emotions can cover creatures within 6 m (20 ft), but it requires concentration and lasts 10 turns.
Is Heroism useful against repeated fear effects?
Heroism prevents its target from becoming Frightened for 10 turns and grants 5 temporary hit points each turn. It is an early, targeted option, but it requires concentration and does not automatically cover the rest of the party.
The Bottom Line
Read the condition name first. Frightened stops movement and imposes disadvantage; Fearful from Fear makes the target flee, blocks additional actions, drops its weapon for one turn, and allows another save after ending a turn out of sight. Use line of sight for Fearful recovery, direct removal for active Frightened, and prevention or immunity—such as Heroism, Aura of Courage, Heroes’ Feast, or a qualifying feature or item—before the next fear effect lands.
