For everyday comfort, cotton is a sensible starting point if you prefer an absorbent fabric or are following vulvar skin-care guidance that recommends entirely cotton underwear. Microfiber is also a reasonable preference if you like a smooth, lightweight feel; its label alone does not show how well a particular garment moves moisture. Fit, seams, stretch, the garment’s full fiber content, perspiration and laundry products can matter as much as the broad fabric name.
Neither fabric is a guaranteed solution for sweat or a proven way to prevent infection. Here is how to compare them for your own comfort, and when targeted clinical guidance changes the choice.

| Microfiber, often nylon or polyester | People who prefer a smooth, soft, lightweight feel | Performance depends on the actual fibers and garment construction; the word “microfiber” alone does not establish moisture handling | Oct 2026 |
The fabric descriptions come from a 2020 Bostik Global Nonwovens whitepaper, which discusses materials in the context of feminine-care products and pad attachment rather than reporting a controlled comparison of underwear comfort. The clinical guidance cited below addresses vulvar skin care, not a universal ranking of underwear for every wearer.
What is the difference between cotton and microfiber underwear?
Cotton is a natural fiber valued here for absorbency; microfiber is a fine synthetic fabric category, commonly made with fibers such as nylon or polyester. The two labels do not tell you everything about a finished pair: fiber blends, stretch yarn, garment construction and the gusset can differ.

Cotton: absorbent, but not automatically dry
Cotton can absorb moisture, and the materials whitepaper describes it as breathable. Absorbing sweat is not the same as moving it away from skin or making a damp garment feel dry. If you perspire heavily or stay in workout clothes after exercise, cotton’s absorbency by itself does not settle whether it will feel comfortable for you.
There is a separate fit trade-off: the whitepaper says garments made entirely from cotton have limited inherent stretch. That does not mean every cotton pair will feel restrictive. A cut that fits well and a cotton-rich blend with stretch may feel different from a rigid all-cotton garment, so check both the fiber label and how the garment is made.
Microfiber: a feel description, not a moisture guarantee
The whitepaper describes nylon as smooth, soft and lightweight, qualities that can make a nylon-based garment appealing when a low-bulk feel matters. Elastane can provide stretch. These are material characteristics, not evidence that microfiber underwear is medically healthier or more comfortable for everyone.
“Microfiber” does not establish a single fiber recipe or a particular level of sweat-wicking performance. A garment’s composition and construction matter. Check whether its body is nylon, polyester or a blend, and whether the label lists elastane or other fibers. Do not assume every synthetic garment traps moisture, or that every garment sold as microfiber moves sweat effectively.
Why the gusset is not the whole garment
Some underwear has a cotton gusset with a nylon or polyester body. That is not the same thing as underwear made entirely from cotton. If you are choosing a garment to follow guidance recommending entirely cotton underwear, inspect the disclosed fiber content for both the body and gusset; a cotton crotch alone does not establish that the whole garment meets that description.
Labels can also distinguish between a cotton-rich blend and 100% cotton. Those are different compositions, and the blend may include stretch fibers. Read the exact listing or garment label rather than relying on a product name, a “cotton gusset” note or a broad fabric category.

Which fabric is more comfortable for everyday wear?
There is no evidence here establishing one universal winner for daily comfort. Start with how you want the fabric to feel, then consider fit, moisture, seams and skin-care needs; adjust based on your own comfort rather than treating one fiber as automatically right for everyone.
Choose cotton as a starting point when
- You prefer an absorbent fabric and are comfortable with the garment’s fit and feel.
- You are following vulvar skin-care guidance that specifically recommends white, entirely cotton underwear.
- You want a straightforward fiber composition and can verify that the body, not just the gusset, is cotton.
Absorbency does not mean cotton always feels dry. If a pair becomes damp and uncomfortable, changing into a dry garment may be more useful than assuming that another cotton style will solve the problem.
Consider microfiber when
- You prefer a smooth, soft or lightweight feel.
- You find that a particular garment’s cut, seams or stretch suit you better than the cotton options you have tried.
- You can verify the fiber content and are choosing for comfort rather than relying on an unsubstantiated promise that all microfiber wicks sweat.
A smooth feel is a valid comfort preference, but it does not establish a health advantage. If you have a specific vulvar skin-care concern, distinguish that preference from the more targeted clinical recommendations described below.
Compare the garment, not just the fiber name
Everyday comfort is affected by more than fiber. A waistband or leg opening that rubs, a seam in an uncomfortable place, a fit that feels too tight, or fabric that does not suit your activity can make a garment unpleasant regardless of whether its label says cotton or microfiber. These are practical things to notice when deciding whether a particular pair works for you; they do not prove that a whole fabric category is unsuitable.

Does cotton or microfiber keep you cooler and drier?
The available material descriptions do not support a blanket claim that either cotton or microfiber will keep every wearer cooler or drier. Cotton absorbs moisture; a synthetic label does not, by itself, tell you whether the garment will move perspiration or dry quickly.

For exercise or other situations when you sweat, focus on whether the garment feels damp or irritating and whether changing afterward helps. The Vulvovaginal Disorders clinical resource advises changing out of exercise clothing. It also reports that some people whose skin is irritated by perspiration benefit from wick-away Tencel clothing. That example is a useful reminder that an alternative fabric may suit some people, not proof that all microfiber garments have the same performance.
If a product advertises moisture management, check the actual fiber composition and construction rather than inferring performance from “microfiber.” The evidence cited here does not provide a head-to-head test, drying time or measured comfort result for cotton versus microfiber underwear.

What underwear does vulvar skin-care guidance recommend?
For people seeking vulvar skin-care guidance, the University of Washington Department of Obstetrics and Gynecology recommends white, entirely cotton underwear. The Vulvovaginal Disorders clinical resource also discusses plain white cotton for people with vulvar concerns, while noting that some perspiration-sensitive patients benefit from wick-away clothing.
This is targeted guidance, not proof that every other fabric causes irritation or disease. The University of Washington handout frames its goal as avoiding irritating products, excess moisture or friction. Its recommendation is best understood in that context: a practical care suggestion for people with vulvar skin concerns, not a universal rule for all underwear or a finding that cotton prevents infection.
Does synthetic underwear cause yeast infections?
The Vulvovaginal Disorders resource states: “There is no high quality evidence showing a link between vulvovaginal candidiasis and hygienic habits or wearing tight or synthetic clothing.” That does not establish that clothing can never affect an individual’s comfort; it does mean a claim that microfiber or synthetic underwear causes yeast infections is not supported by this guidance.
Do not use fabric choice alone to diagnose or explain ongoing irritation or suspected infection. The clinical resource’s cautious statements belong together: plain cotton is discussed for vulvar concerns, some people find wick-away clothing helpful when perspiration irritates their skin, and the evidence does not establish synthetic clothing as a cause of candidiasis.

What else can make underwear irritating?
Fiber is only one possible part of the comfort picture. The clinical handouts emphasize reducing irritation, moisture and friction, while the University of Washington material discusses laundry exposures. A fabric swap may not address discomfort if the underlying issue is rubbing, garment fit or a product used to wash it.
Fit, seams and elastic
Notice whether discomfort is associated with a specific seam, waistband, leg opening or area of rubbing. A different cut or a less irritating fit may be a more relevant change than moving from cotton to microfiber, though this is a personal comfort adjustment rather than a medical diagnosis.
Dyes and laundry products
The University of Washington handout discusses irritation from laundry additives. If you suspect that washing products contribute to discomfort, review the detergent and other laundry products used with the garment, and consider the handout’s guidance on avoiding irritating products. Do not assume that a fiber label alone explains a reaction.
When to seek clinical advice
If irritation continues or you suspect an infection, do not self-diagnose based only on whether your underwear is cotton or synthetic. A clinician can help assess symptoms rather than treating a fabric switch as a diagnosis or cure.

Clear out junk files and repair common Windows errorsFree Scan →Scan for outdated or missing drivers - takes under a minuteDriver Scan →Can pads or liners affect the fabric choice?
They can affect the practical choice: the Bostik whitepaper notes that fabric fibers, finishes and stretch can influence adhesive pad attachment. This is a product-compatibility consideration, not evidence that cotton or microfiber is clinically superior.
If an adhesive pad or liner does not stay in place, the garment’s surface and stretch may matter along with the product. Check how the specific underwear works with the pad or liner you use rather than assuming all fabrics behave the same. The whitepaper does not provide a clinical comfort ranking or a universal rule for which underwear works best with every product.

How should you choose a pair for your routine?
Use a short checklist to compare actual garments. It keeps the decision grounded in what the label says and how the pair feels, rather than broad claims about an entire category.
- Start with the reason for choosing. Decide whether your priority is an absorbent feel, a smooth and lightweight feel, following targeted vulvar-care guidance, or comfort during a sweat-producing activity.
- Read the full fiber composition. Check the body and gusset where they are separately disclosed. Distinguish entirely cotton underwear from a synthetic-bodied garment with a cotton gusset, and distinguish cotton-rich blends from 100% cotton.
- Check the construction that affects fit. Consider the cut, stretch, waistband, leg openings and seam placement. The label’s main fiber name cannot tell you whether those details will suit you.
- Think about moisture in your routine. If exercise leaves the garment damp or your skin feels irritated by perspiration, changing afterward is one practical step supported by the clinical resource. Do not infer that every microfiber option will wick sweat.
- Account for laundry and other products. If discomfort persists, consider whether laundry additives or friction may be contributing rather than assuming that the fabric is the only factor.
- Reassess based on comfort. A preference for one fabric is not a medical verdict on the other. If symptoms continue or you suspect infection, seek clinical advice instead of relying on underwear choice alone.
What is the practical verdict?
For ordinary daily wear, cotton is a sensible starting point if you value absorbency or are following the cited vulvar-care recommendation for white, entirely cotton underwear. Microfiber is a legitimate choice when its smooth, lightweight feel and a particular garment’s fit suit you. Neither label guarantees a dry feeling, and the available clinical guidance does not establish that synthetic underwear causes yeast infections.

Frequently Asked Questions
Is a cotton gusset the same as 100% cotton underwear?
No. A cotton gusset can be attached to a body made from nylon, polyester or a blend. If you need entirely cotton underwear to follow specific care guidance, verify the composition of the whole garment rather than relying on the gusset description.
Can I wear microfiber underwear while exercising?
A microfiber label does not establish how a garment will handle sweat, so the label alone cannot answer that for every pair. The clinical resource advises changing out of exercise clothing; choose based on the specific garment’s comfort and whether it leaves you feeling damp or irritated.
What should I do if cotton underwear feels uncomfortable?
Cotton is not a universal comfort solution. Consider whether fit, seams, elastic, friction or laundry products might be involved, and remember that the clinical resource reports some perspiration-sensitive people benefit from wick-away clothing.
Does underwear fabric need to be white for everyday comfort?
The white-color recommendation comes from vulvar-care guidance for its target context; the evidence cited here does not establish that white underwear is more comfortable for everyone. If you are following that guidance for a vulvar concern, use its specific recommendation rather than generalizing it into a universal rule.
