Choose to fight
At the final confrontation, you can kneel and avoid the fight or refuse and take on the boss. Gameranx reports that after the battle you face another choice: kill the defeated enemy or have him serve in your cult. The guide does not document every consequence, so treat that decision as a separate story choice rather than assuming a particular ending outcome. Gameranx’s 2022 guide covers the confrontation and these choices.

Fight Baal, then Aym
Baal
Baal uses spinning melee attacks, cross-shaped and wide attacks, and summoned followers, according to Gameranx. Avoid staying close through his spinning attacks; dodge out of danger, then use the opening to land hits. Watch the wider attack patterns rather than committing to a long attack string.
Aym
Aym can teleport, fire homing circular projectiles, and create floor tracks that produce spikes. Keep moving to avoid being lined up with the projectiles or spike tracks. When Aym pauses or finishes an attack, take the opening to strike, then return to dodging.

These enemy-specific tactics come from guides published in 2022; exact timings and damage behavior have not been established for every current game version. Gameranx describes Baal and Aym’s attacks, while Pro Game Guides covers The One Who Waits.
Survive The One Who Waits’ opening phase
The One Who Waits uses projectiles and spike attacks, and Gameranx also reports attacks drawn from those used by the two guards, plus a rotating fireball attack. Stay mobile and dodge through projectiles when that is the safer route. When the boss charges a spike attack, retreat beyond its range; move back in to attack once the danger passes. Avoid treating any specific timing as guaranteed.

Defeat the three eyes
The final sequence centers on three floating eyes, each firing its own projectile patterns. Prioritize staying alive: keep moving, look for gaps between volleys, and attack an eye when its pattern leaves room. Pro Game Guides reports that eyes can go underground and that projectile rings may cross the arena. Gameranx describes volleys as eyes are defeated and says the last eye attacks aggressively, so do not assume the danger drops as the sequence progresses.
Pro Game Guides also reports that followers sometimes drop Fervor during this phase. Pick it up when safe and use it for Curses, but do not cross a dangerous projectile path just to collect it.

The tactical descriptions above are reported strategies, not verified guarantees for exact timing or damage.
The available guides do not establish a best weapon, Curse, difficulty setting, or loadout, nor do they document patch-specific timing or difficulty adjustments. Build around the tools you have: use movement and dodging to stay safe, and take damage opportunities when attacks end.

Refighting the bosses in Purgatory
An official patch-note search result says the game added the ability to refight Baal, Aym, and The One Who Waits in Purgatory. That statement establishes the feature, but not whether Purgatory uses the same rules, attack behavior, or progression as the campaign encounter. Cult of the Lamb’s official Steam news is the relevant patch-note source.
