In Diablo IV’s Season of Divine Intervention, Azmodan is a permanent World Boss in the rotation on Seasonal and Eternal realms, but Blizzard’s announcement gives no exact spawn times. The season also has three separate mechanics that are easy to confuse: the Urn of Calamity grants additional World Boss gear, Essence of Hellfire rewards Lair Boss Keys, and the Heavenly Forge is used to Sanctify items.
When does the Hellfire World Boss spawn?
“Hellfire World Boss” is not the name of a single unified mechanic. Blizzard describes Azmodan as a permanent World Boss who appears in the rotation alongside other World Bosses on both Seasonal and Eternal realms. The official season announcement does not list exact spawn times or an interval. Check the current in-game World Boss schedule for the live timing rather than relying on an old timetable.

Is there a +1 loot limit?
Blizzard describes the Urn of Calamity as providing “additional gear rewards from World Bosses.” The announcement does not specify that this means exactly one extra item, so “+1 loot” should not be treated as a guaranteed item-count rule.
What does Essence of Hellfire give?
Essence of Hellfire is a Divine Gift separate from the Urn of Calamity. Surviving World Boss encounters earns Lair Boss Keys; the reward is doubled while the gift is Purified. Blizzard does not state the baseline number of keys in the announcement.

Essence of Sin is another separate gift: it awards Activity Keys for surviving World Boss encounters. Those are not Lair Boss Keys from Essence of Hellfire.
What do I need to use the Heavenly Forge?
Heavenly Sigils transport players to the Heavenly Forge for Sanctification. Blizzard says Sigils are needed only to Sanctify Ancestral-quality items, and that they can be earned in Torment difficulties. The announcement does not provide an exact Sigil cost or drop rate.

In pre-Torment difficulties, defeating a Lesser Evil in one of its invaded game modes can offer a rare chance to Sanctify an item at the Heavenly Anvil. Sanctification can add or change item properties, but makes the item unmodifiable; treat it as the item’s final upgrade.

Blizzard’s announcement presents these as distinct systems. It does not say that bonus gear from World Bosses is needed for, or directly connected to, Sanctification.
