No—Borderlands 3 does not call its post-campaign difficulty system Ultimate Vault Hunter Mode (UVHM). The game’s named endgame difficulty route is Mayhem Mode, which unlocks after you finish the main campaign. Mayhem is not simply another name for UVHM; it is Borderlands 3’s own system of escalating difficulty, randomized modifiers, and stronger weapon drops.
What is Borderlands 3’s endgame difficulty mode?
Borderlands 3 uses Mayhem Mode for its post-campaign difficulty progression. In its April 23, 2020 announcement, 2K described the mode this way: “Mayhem Mode unlocks when you finish the main Borderlands 3 campaign and allows you to crank up the game’s difficulty in exchange for better loot.” 2K’s announcement introduced Mayhem 2.0, which added ten difficulty levels and randomized modifiers.

So if you are looking for a UVHM unlock in Borderlands 3, there is no separate mode by that name to unlock. Finish the main story to access Mayhem instead.
How does Mayhem progression work?
Mayhem is a gear-up loop: higher levels raise the challenge and, in turn, improve the power of weapons dropped at those levels. The point is to test whether your current character build and equipment can handle tougher enemies, then use the rewards to prepare for the next step.

- Modifiers: Mayhem 2.0 introduced randomized modifiers, changing the conditions of a run. The original rollout described more than 25 new modifiers.
- Loot: Weapons dropped at higher Mayhem levels have improved power, helping you progress into tougher content.
Gearbox’s April 2020 Mayhem 2.0 overview describes ten levels and the ability to adjust Mayhem options through the ECHO device menu. These figures and menu details describe the 2020 rollout; they should not be read as a complete account of every later balance or interface change.
How should you decide whether to raise Mayhem?
Use the next level as a readiness check rather than assuming that each step will suit every build. If you can clear encounters reliably and your gear is keeping pace, try increasing Mayhem. If fights become slow or costly, stay at the level where you can defeat enemies consistently and collect upgrades before moving on.

- Check your current build and equipment. Consider whether your weapons and overall setup are effective against the enemies you are facing.
- Raise Mayhem and assess the run. Watch how enemy durability and modifier combinations affect your ability to survive and deal damage.
- Keep or lower the setting based on results. A higher level is useful when its added challenge remains manageable and the stronger drops help you advance; it is not progress if it stalls your ability to farm or complete fights.
The official 2020 materials establish that Mayhem levels increase enemy toughness and weapon power, but do not provide a current, universal build threshold for moving up. Your readiness depends on your character and gear.
Are there Mayhem-specific Legendary drops?
As part of the April 2020 rollout, 2K said certain bosses could drop eight new Legendary weapons starting at Mayhem 6. That is a dated update detail, not a complete or current loot table: the announcement does not establish present-day drop rates or cover every later change. Is Mayhem the same as True Vault Hunter Mode?
No. The documented distinction relevant to this question is that Mayhem Mode is Borderlands 3’s named post-campaign difficulty system; UVHM is not its name. The available official Mayhem 2.0 material does not support a comprehensive current comparison with True Vault Hunter Mode, so it is safer not to treat the names or systems as interchangeable.

No separate UVHM unlock or add-on is identified in the official Borderlands 3 endgame announcements.
