Fallout 4 includes the F word in spoken dialogue. The ESRB rating summary explicitly says it is heard, but does not give a count or identify the scenes where it occurs.
What do the ratings say about language?
The Entertainment Software Rating Board (ESRB) says: “The words ‘f**k,’ ‘sh*t,’ and ‘a*shole’ are heard in the dialogue.” Its Fallout 4 rating summary assigns the game Mature 17+ and lists Strong Language, alongside Blood and Gore, Intense Violence, and Use of Drugs.

Other listings also flag strong language: the Xbox storefront shows Mature 17+ and Strong Language, while Steam’s Fallout 4 listing includes Strong Language in its ESRB descriptors.
How often is the F word used?
The rating summaries establish that the word is present, not how many times it occurs or where. Common Sense Media characterizes the language as “Occasional but very strong” and says it appears in text and spoken dialogue, including the F word and S word, but does not provide a count or scene list. Does the language differ by platform or edition?
The cited ratings do not establish whether dialogue differs between platforms, editions, or localized versions. The ESRB page identifies Windows PC, PlayStation 4, and Xbox One, but listing those platforms is not evidence that the dialogue is identical—or different—across them.
What should parents take from this?
If strong language is a concern, the ESRB’s Mature 17+ rating and Strong Language descriptor are relevant alongside its separate warnings for violence and drug use. The rating confirms that profanity is spoken; it does not tell you whether its frequency or context fits your household’s standards.
