Silent Hill 2 remake fans interpret the opening car scene as showing Mary’s body in the back seat, but the evidence here does not establish that as an unambiguous, officially confirmed reveal. The original game’s related source text asks whether her body could be in the car; it does not answer the question.
What the original game says about Mary’s body
The original game’s Book of Lost Memories raises the possibility that Mary’s body is in James’s car without resolving it. The relevant passage, section XI, “Justice,” reads: “The real reason James came to Silent Hill was to take his own life in a place of memories. If this is the case, could Mary’s body be in the car!?”

That wording is a question, not a confirmation. Silent Hill Memories’ plot analysis quotes the passage and likewise notes that it does not outright reveal the answer. Its broader timeline discussion is fan interpretation, so it should not be treated as an additional statement of canon.
What the remake appears to show
The remake-specific claim is that Mary can be seen in the back seat beneath a blanket during the opening. Fan discussion describes that reading, but the material available here does not include a direct developer or publisher statement confirming it. That distinction matters: an interpretation of what a scene depicts is not the same thing as an official confirmation.

Accordingly, the most careful answer is that the remake is said by fans to show Mary in the car, while the original source wording leaves the question open. Without a direct confirmation, it is too strong to present the remake’s scene as a settled explanation of the original game’s ambiguity.

The remake’s Maria ending is a separate story beat. In Gameranx’s October 25, 2024 guide, James attempts to leave Silent Hill with Maria after the final boss, and she begins coughing as they approach his car. That ending imagery concerns Maria and does not establish where Mary’s body is in the opening scene.
