The catacombs’ exit leads into the woods; the map fragment is farther along the trail.
Before you start: unlock the Shadow of the Erdtree
Your save must have defeated Starscourge Radahn and Mohg, Lord of Blood to enter the Land of Shadow. From the base game, enter at the Site of Grace named Cocoon of the Empyrean. After activating a Site of Grace in the Land of Shadow, you can use the map to travel between the base game and the DLC. The official Elden Ring access notice lists these prerequisites.
Find the hidden passage in Shadow Keep

Head into Shadow Keep and make your way from around Main Gate Plaza toward the area with burning boats. If a gate blocks the route, take the longer path around through the keep.

Find the ladder leading down to the flooded area. Pass through the waterfall, then take the next ladder down to a lower room containing a red painting.

Strike the wall near the painting to reveal a concealed passage. Follow it to the coffin and choose the “Rest in coffin” prompt. You will be transported to Castle Watering Hole.
If you cannot find the passage, retrace your steps to the room beyond the waterfall and inspect the wall beside the red painting. PowerPyx’s route guide and Push Square’s guide describe this concealed route.

Follow the river south to Darklight Catacombs

From Castle Watering Hole, continue south through the ruins and along the river area.

Reach Recluses’ River Downstream, then descend the rocky platforms toward the graveyard. This stretch is a series of drops rather than one continuous road, so look for the next safe descent as you move down.

Enter Darklight Catacombs from the graveyard and work through the dungeon. Defeat Jori, Elder Inquisitor, at the end.
The archway beyond Jori marks entry into the Abyssal Woods. The catacomb exit gets you into the region; it is not where you pick up the map fragment.

After entering the valley, continue toward the Woodland Trail Site of Grace. Follow the trail southeast to the remnants of a building, where the map fragment is located. The fragment is collected after you have already reached the woods, rather than at the catacombs’ exit. Push Square’s route details cover the fragment’s location.
