Hamstring Strain Recovery: Exercises, Timelines, and When to Seek Medical Advice

Hamstring strain recovery varies with severity. Learn about first-step care, gradual exercise progression, realistic healing timelines and signs that warrant medical advice.

Hamstring strain recovery depends on how badly the muscle is injured and how well it regains function—not on a fixed number of days. For a mild injury, protect the leg and use NHS-recommended self-care for the first two to three days; then return to gentle movement as pain allows. Seek medical advice for severe or worsening symptoms, and progress strengthening, running and sport gradually rather than returning just because pain has eased.

What a hamstring strain can feel like

A strain commonly causes sudden pain at the back of the thigh during activity, tenderness, pain when stretching or activating the muscle, and reduced function. Swelling or bruising, difficulty standing or walking, and a popping or snapping sensation can also occur. These symptoms do not establish the severity or confirm a diagnosis; a clinician can assess an injury, particularly when symptoms are severe.

The NHS hamstring injury guidance describes sharp pain, tenderness, swelling or bruising, and difficulty standing or walking. Severe injuries may involve a snapping sensation or pop.

What to do in the first two to three days

For a mild to moderate injury, NHS guidance recommends PRICE during the first two to three days:

  • Rest: avoid activities that aggravate the pain.
  • Ice: wrap ice in a towel and apply it for up to 20 minutes every two to three hours.
  • Compression: use a snug bandage, not one that feels tight.
  • Elevation: raise the leg when practical.

For the first three days, avoid heat, massage and alcohol, as the NHS advises. When pain no longer stops you moving, gentle movement can help prevent stiffness. This self-care advice is for mild to moderate injuries; it is not a substitute for assessment when an injury is severe or you are unsure what has happened.

When to seek medical advice

In the UK, contact NHS 111 for urgent advice if pain is very severe or getting worse, swelling or bruising is substantial or worsening, standing or walking hurts, the leg is very stiff or difficult to move, self-treatment is not helping, or you have a high temperature or feel shivery. Outside the UK, contact your local urgent-care or health-advice service for comparable symptoms.

A complete muscle tear may need specialist care and can require surgery. The NHS also advises seeking physiotherapy advice if recovery is slow or you have had hamstring injuries before. A clinician can determine whether examination or further treatment is needed.

How long does a hamstring strain take to heal?

There is no reliable single recovery date for everyone. The NHS says mild hamstring injuries usually improve in a few days, while severe injuries can take weeks or months. A complete tear that needs surgery may take several months to recover from. Those ranges describe broad possibilities, not an individual prognosis.

Recovery depends on injury severity and the return of function. Pain-free walking, strength, tenderness and how the leg responds to progressively harder activity are among the factors clinicians may consider. A prior hamstring strain is a reinjury risk factor. The 2022 clinical practice guideline recommends considering several factors—including strength, initial pain, days until pain-free walking and the area of tenderness—rather than using any one of them as a guaranteed return-to-play calculator.

Broad NHS advice for sprains and strains mentions that many feel better after two weeks and advises avoiding strenuous exercise such as running for up to eight weeks. Those are general sprain-and-strain figures, not a hamstring-specific prediction, so they should not be used to set a personal return date.

Hamstring strain recovery exercises and progression

Exercise should be matched to the injury and progressed according to symptoms and capacity. The 2022 clinical practice guideline recommends a comprehensive functional program that includes progressive strengthening and eccentric training. It also describes stretching and functional work, and recommends progressive agility and trunk-stabilization work alongside those elements to help reduce reinjury risk. It does not establish one exercise sequence or dosage for every person.

Early phase: protect the healing muscle

A 2018 clinical review proposes beginning with work to reduce pain and swelling and restore slower neuromuscular control, while protecting healing fibers from excessive lengthening. This is a framework, not a universal prescription. In the early rehabilitation stage, the 2023 London International Consensus advises avoiding high strain loads and high loading rates.

Middle phase: build strength gradually

As capacity improves, rehabilitation can increase exercise intensity and introduce eccentric resistance work—when the hamstring produces force as it lengthens. The review’s proposed framework places this work in a later phase. Clinicians can use symptoms, strength and the response to previous loading to decide when and how to progress.

Later phase: prepare for running and sport

Later rehabilitation may include faster neuromuscular and eccentric work in lengthened positions, followed by running and sprinting that match the demands of the person’s sport. The London consensus recommends individualizing rehabilitation to the athlete, sporting demands, injured muscle, and injury type and severity. Experts did not agree on a universally optimal exercise order or dosage, or on universal flexibility and strength benchmarks.

The athletic rehabilitation guidance is not a ready-made home program for every hamstring strain. Exercise selection, starting point and workload should be individualized. Do not push through worsening pain or treat a published athlete framework as clearance to train.

Returning to running, sport or full activity

Do not return solely because a certain number of days has passed or because pain has reduced. Readiness involves how the leg responds to loading and whether function has recovered sufficiently for the intended activity. For athletes, later progression should include sport-relevant running and sprinting.

A football-specific 2017 Delphi consensus defines return to play as criteria-based medical clearance combined with mental readiness for full availability for selection or training. Its supporting considerations include medical-staff clearance, no pain on palpation, strength, flexibility and functional testing, comparable flexibility, field testing and psychological readiness. These are athlete-focused consensus criteria, not a self-test or a universal checklist for recreational activity.

The 2022 guideline cautions against return to play without completing an appropriately progressed functional rehabilitation program that includes eccentric training. If the injury is severe, recurrent or slow to improve, ask a healthcare professional about the appropriate progression and return-to-activity decision.
