You can’t use tampons in unmodded The Sims 4. The feature comes from mods such as WickedWhims: with its menstrual-cycle system enabled, put a tampon in your Sim’s inventory and the Sim will use it automatically by default when needed. You can change that behavior from the item’s inventory menu.
Use tampons with WickedWhims
- Enable the menstrual cycle. Open Wicked Settings → Pregnancy Settings → Menstrual Cycle Settings and check that the menstrual-cycle feature is switched on under Pregnancy Settings. The creator’s guide documents these controls and explains that the feature has changed over time: WickedWhims menstrual-cycle guide.
- Get a tampon. Use a computer’s Wicked category or find the item in Build/Buy mode. The guide describes the products as purple boxes labeled Kaya.
- Place it in the Sim’s inventory. With a product in inventory, autonomous use is on by default: the Sim seeks a nearby toilet to apply it and disposes of the used product.
- Adjust automatic use if you want. Click the tampon or pad in the Sim’s inventory and change its Auto Use behavior.
The creator’s documented instructions focus on automatic use and inventory controls; they do not establish a manual interaction labeled “Use Tampon” that appears in every version. If you do not see a manual command, check that the item is in the Sim’s inventory, the cycle feature is enabled, and the Sim is in the relevant period state.
Change cycle and product settings
In WickedWhims’ menstrual-cycle settings, you can adjust Menstrual Cycle Tampon/Pad Absorbency. The creator also documents an Extra Absorbency reward trait that costs 1,000 Satisfaction Points. This is an in-game reward cost, not a real-world price.
If you do not want Sims to use a toilet to change a product, disable Use Toilet to Change Pad/Tampon in Advanced Settings. Advanced Settings also include a Period Bleeding toggle; the menstrual-cycle feature itself can be switched off in Pregnancy Settings.

Install the mod on a supported platform
EA supports player-installed mods on PC and Mac, not PlayStation or Xbox. EA’s general instructions are to download a mod, unzip it if needed, put it in the Sims 4 Mods folder, enable Enable Custom Content and Mods under Game Options → Other, then relaunch the game. WickedWhims has additional requirements: put the WickedWhimsMod folder containing its PACKAGE and TS4SCRIPT files directly in Documents → Electronic Arts → The Sims 4 → Mods. In Game Settings → Other, enable Custom Content and Script Mods, apply the changes, and restart. The creator notes that game patches can turn these settings off, so check them again after an update. Choose one menstrual-cycle mod
WonderfulWhims is a standalone alternative whose CurseForge listing describes menstrual-cycle gameplay and purchasable tampons and pads. Do not assume it can run alongside WickedWhims: the WickedWhims download page lists WonderfulWhims as conflicting. Check the creators’ current compatibility notes before installing, since mod compatibility can change with game patches.
Compatibility details are time-sensitive. When checked on October 8, 2026, the WickedWhims download page listed version v185k, dated May 23, 2026, and Sims 4 version 1.128.90 (September 22) among supported versions. Check that page for current files and supported game versions before downloading.
