A plague-specific cleanse or immunity skill is not identified in the currently published material. Do not assume that a remedy, resistance item or hero removes a plague debuff unless the current in-game tooltip names that effect. The practical answer is therefore mitigation and sustain first, with status-counter claims treated as unconfirmed until the game gives you a named mechanic.

What “plague damage” means in the current information
Last Asylum Plague presents itself as a mobile survival RPG in which you “become a plague doctor in a world overrun by sickness and the Rat Swarm.” Its official description focuses on treating survivors, gathering supplies, upgrading a refuge and leading heroes into ruined territory. It also describes treating patients, crafting remedies and keeping the sanctuary alive. Those setting terms do not, by themselves, establish a separate combat damage category called plague damage.

The combat information that is documented divides incoming damage into physical and energy types. It separately describes hero roles, faction matchups and skills that reduce or restore health. A plague icon, damage-over-time effect or named disease debuff may appear in a particular encounter, but the currently available material does not provide a complete status-effect catalogue, a named cleanse, or a universal immunity skill.

That distinction matters. If the combat readout labels an attack physical or energy, use the corresponding resistance slot. If the readout shows a status icon without naming a counter, treat the icon as an unresolved effect rather than guessing that armor, boots, a remedy or a faction bonus will remove it.
The six-step counter sequence
- Identify the enemy faction. Where a mode lets you choose a faction, use the published counter: Warriors against Warlocks, Rangers against Warriors, or Warlocks against Rangers. The advantage is 20% less damage taken from the countered faction.
- Read the incoming damage type. Check whether the attack is physical or energy. Do not choose gear from the word “plague” alone; choose it from the damage label shown by the encounter or combat readout.
- Assign the resistance slot. Equip armor for physical resistance, boots for energy resistance, and gauntlets when broad all-damage resistance is more valuable than specializing in one type.
- Protect the first line. Put two durable heroes in the two frontline positions. Keep damage dealers and support in the three back positions so the opening pressure lands on tanks rather than your healer.
- Add sustain when damage is prolonged. Support is the role explicitly described as having ally-healing skills. The published roster index lists five support heroes, so use one when your defeat comes from gradual health loss rather than an immediate burst.
- Use timed mitigation, not an assumed cleanse. If Harper is available, Vitality Bubble protects the lowest-HP ally for four seconds. The base value reported for the skill is 8.2% damage reduction, with stronger values at higher skill levels and star thresholds. It creates a survival window; it is not documented as a plague-status removal.
Exploit the faction triangle
The faction matchup is the clearest documented way to reduce incoming hero damage. It is a directional triangle, not a universal bonus: each faction counters one other faction and is itself countered by a third.
| Your faction | Reduced damage from | Published reduction | Use it against |
|---|---|---|---|
| Warriors | Warlocks | 20% less damage taken | Warlock opponents |
| Rangers | Warriors | 20% less damage taken | Warrior opponents |
| Warlocks | Rangers | 20% less damage taken | Ranger opponents |
Against Warlocks
Choose Warriors when faction selection is available. This is the direct counter shown by the matchup rules. Combine it with armor if the battle readout identifies the incoming attacks as physical, or boots if it identifies energy damage. The faction reduction and the gear resistance address different layers, so one does not replace the other.
Against Warriors
Choose Rangers. If the enemy’s attacks are physical, armor is the focused resistance choice. If the encounter’s damage is energy-based, boots are the relevant slot. When the battle mixes both types or you cannot identify a dominant type, gauntlets provide the documented all-damage option.
Against Rangers
Choose Warlocks. Then make the same physical-versus-energy check before changing equipment. A Warlock counter bonus does not tell you whether the enemy attack is physical or energy, and it does not establish protection from a separate status effect.
When faction choice is unavailable
Some modes may lock your faction or determine it from the roster. In that case, do not force a faction-based build that the mode cannot use. Move to the next layer: identify damage type, strengthen the two front positions, and add healing or timed mitigation. The 20% rule is useful only where the game actually permits the matchup to be applied.

Choose armor, boots or gauntlets by damage type
The documented equipment mapping is straightforward:
| Gear slot | Resistance provided | Best use | What it does not prove |
|---|---|---|---|
| Armor | Physical-damage resistance | Encounters whose attacks are labeled physical | It is not documented as a plague cleanse or status immunity |
| Boots | Energy-damage resistance | Encounters whose attacks are labeled energy | They do not automatically counter every disease-themed effect |
| Gauntlets | All-damage resistance | Mixed damage, uncertain readouts or a need for broad mitigation | All-damage resistance is not the same as removal of a debuff |
If the readout says physical
Prioritize armor on the heroes taking the hits, especially the two frontliners. The purpose is to reduce the damage component identified as physical. Keep the faction counter in place if possible; resistance gear and faction advantage work on separate decision axes.
If the readout says energy
Use boots for the heroes exposed to energy attacks. If your frontline survives the opening but your back row is being chipped down by energy attacks, distribute the relevant resistance according to who is actually losing health rather than automatically stacking it on one tank.
If the encounter mixes types
Gauntlets are the documented broad option. They are also the sensible starting point when the battle contains both physical and energy attacks and you cannot make one resistance type dominant. Once you can identify which type causes most of the failed attempts, specialize the most exposed heroes instead of treating “all damage” as always superior.
If the only clue is a plague label
Do not infer the gear slot from the theme. “Plague” can describe the encounter, enemy or status without specifying physical or energy damage. Look for the combat damage label or tooltip. If none is provided, use gauntlets for broad mitigation and rely on formation and healing while you determine what is actually killing the team.
Build a formation that survives the Rat Swarm
The documented formation pattern is two frontline tanks and three heroes in the back row. It is not merely a visual arrangement: the front positions are the intended place for heroes who can absorb the initial pressure, while damage dealers and support need time to act.

| Position | Recommended job | Reason |
|---|---|---|
| Front 1 | Primary tank | Receives opening pressure and buys time for the team |
| Front 2 | Secondary tank or durable bruiser | Prevents one-target collapse from exposing the back row |
| Back 1 | Damage dealer | Maintains pressure while protected by the frontline |
| Back 2 | Damage dealer or utility hero | Fills the team’s damage or control requirement |
| Back 3 | Support | Provides ally healing when sustained damage is the problem |
Why two tanks matter
A single tank can leave the second frontline position exposed, allowing pressure to reach your back row sooner. Two frontliners distribute the initial threat and give support more time to restore health. If a battle is lost before your damage dealers can act, replacing a back-row damage slot with a second durable frontliner is usually a more relevant test than changing plague remedies that have no documented combat effect.
Where to place support
Keep support in the back row unless the current hero description explicitly says otherwise. Support is the only role documented as healing allies, so losing that hero early removes your principal answer to attrition. If the support hero is the lowest-health ally, Harper’s bubble can create a short window for the healer to survive and restore the team.
When the back row still collapses
Check whether the encounter is dealing area damage or bypassing the tanks. The published formation guidance does not establish a special plague-status position, so do not assume that moving a hero to another slot cures a debuff. First confirm the damage type, then use the broad resistance option if both rows are being hit.

Use support healing and Harper’s Vitality Bubble
Support heroes are the documented source of ally healing. The roster index lists five support heroes, but the available information does not establish that every support has the same healing amount, cooldown or secondary effects. Choose the support whose current in-game description fits the encounter rather than assuming that a role label guarantees a cleanse.
Harper’s mitigation window
Harper’s Vitality Bubble targets the lowest-HP ally and reduces damage received for four seconds. The base description reports 8.2% damage reduction, with stronger values as the skill is upgraded and as star thresholds are reached. That makes it a timed defensive tool:
- Use it when the lowest-health ally is about to receive another burst, not after the ally is already defeated.
- It can stabilize a tank during the opening seconds or keep a support hero alive long enough to heal.
- It reduces incoming damage; the description does not say that it removes an existing plague status.
- Exact values may differ after a patch, so read the current skill tooltip before planning around 8.2%.
Healing versus mitigation
Healing repairs lost health; Vitality Bubble reduces future damage for a short duration. A team facing repeated damage generally benefits from both: tanks absorb the first pressure, support restores health, and Harper’s bubble covers the most vulnerable moment. Neither mechanic is documented as immunity to a named plague effect.

What is—and is not—confirmed about plague status effects
The current published information does not name a plague-status catalogue, a cleanse skill, a disease-removal item or a universal immunity effect. That is an evidence limit, not proof that no future patch or individual hero can have one. Until a current tooltip identifies such a mechanic, describe status protection as unconfirmed.
How to verify a status in your own build
- Read the enemy or encounter tooltip and note whether the effect is labeled physical, energy or a separate status.
- Inspect each candidate hero skill for explicit terms such as “cleanse,” “remove debuff,” “immunity” or the exact status name.
- Check whether the effect is damage over time, a stat reduction, a control effect or another condition. Do not treat all icons as the same plague mechanic.
- Compare attempts with and without the skill while keeping faction, gear and formation constant. Change one variable at a time.
- Use the current in-game wording as the authority if it conflicts with an older community table.
Claims to avoid
- Armor automatically removes plague damage.
- Boots grant immunity to every disease-themed attack.
- Gauntlets cleanse a status because they reduce all damage.
- A support hero can cleanse simply because that hero can heal.
- Harper’s Vitality Bubble prevents or removes a plague debuff.
Those statements are not established by the published mechanics. The safe description is narrower: armor, boots and gauntlets mitigate documented damage categories; support heals allies; Harper reduces incoming damage for four seconds.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Practical builds for common enemy patterns
Warlock pressure
Use Warriors when the mode allows faction selection. Start with two tanks in front, a support in the back, and the remaining back-row slots for damage. Choose armor or boots from the combat readout, not from the Warlock label. If one ally repeatedly drops first, use Vitality Bubble to cover that low-health window.
Warrior pressure
Use Rangers for the faction advantage. Armor is the focused resistance against physical attacks; boots are the focused choice against energy attacks. Keep two frontliners even if the Rangers’ counter reduces incoming damage, because the faction bonus does not replace formation durability.
Ranger pressure
Use Warlocks where permitted. If the damage is mixed or unclear, begin with gauntlets and observe which heroes lose health. Shift toward armor or boots only after the readout establishes a dominant type.

Use the broadest defensible setup: two tanks, one support, gauntlets on the heroes exposed to multiple damage types, and the available faction counter if applicable. Record whether defeats come from burst damage, gradual attrition or an unexplained status icon. That diagnosis tells you whether to change frontline durability, healing timing or a still-unconfirmed status response.

Troubleshoot a team that still loses
You chose the counter faction but die quickly
Confirm that the mode applied faction selection and that the enemy faction is what you identified. Then inspect damage type. A 20% faction reduction cannot compensate for using energy resistance against physical attacks, or for placing a fragile hero in the frontline.
Your tanks survive, but everyone eventually falls
This is an attrition problem. Add a support hero, protect that hero in the back row and use Harper’s bubble during the most dangerous four-second window. If the fight contains both damage types, test gauntlets before committing every slot to one resistance.
The healer dies before it can act
Keep support in the back row, verify that two durable heroes occupy the front, and use Vitality Bubble on the lowest-HP ally when the healer is the threatened target. Do not solve this by assuming that a status cleanse exists; first fix the damage path that is reaching the back line.

The values in a guide do not match your game
The community wiki tables identify game version 1.0.87 data. Its hero index was verified on 31 August 2026 and edited on 2 September 2026. Skill values, roster availability and descriptions can change in later patches, so use the current in-game tooltip when a percentage or star threshold conflicts with an older table.
How to compare two anti-plague builds
When deciding between builds, score the same five dimensions instead of judging from one item or hero:
| Axis | Question to ask | What a stronger answer looks like |
|---|---|---|
| Faction advantage | Does the selected faction counter the enemy? | The applicable 20% reduction is active |
| Damage resistance | Does gear match the readout? | Armor for physical, boots for energy, or gauntlets for mixed damage |
| Frontline durability | Can two heroes absorb the opening pressure? | Both front positions remain functional long enough for back-row skills |
| Sustain and mitigation | Can the team recover from repeated hits? | A support healer survives, with Harper’s four-second window when available |
| Positioning | Are damage dealers and support protected? | Two tanks in front and three back-row heroes behind them |
Change one axis at a time. If you swap faction, gear and formation together, you cannot tell which change solved the encounter. This method is especially important for plague-themed fights, where the name of the encounter may conceal whether the actual failure is physical damage, energy damage, attrition or an unresolved status effect.
