Start in Windows Security → Virus & threat protection → Protection history before changing any setting. Windows Defender may have quarantined a file, removed it after blocking it, stopped a download with SmartScreen, or prevented an application from changing a protected folder with Controlled Folder Access. The correct fix depends on that event. Restore a quarantined file only after checking its publisher, download source, and integrity. Use a narrowly scoped exclusion only when the file is legitimate and the detection continues. Do not routinely switch off real-time protection or exclude an entire Downloads, Users, or system directory.
What does “deleted” mean in Windows Defender?
A file that appears to have been deleted is often still represented by a security action in Protection history. Most files detected by Microsoft security software are quarantined: Defender moves them to a protected location and blocks them from running. A Threat blocked event means the item was removed, while SmartScreen and Controlled Folder Access can stop activity without an antivirus quarantine.

| Protection History event | What happened | Appropriate next action |
|---|---|---|
| Threat quarantined | The file was moved to a safe location and blocked. | Review the detection, then use Restore only if the file is independently verified as safe. |
| Threat blocked | The file was removed before it could run. | Verify the source first; if it is legitimate, download it again after choosing Allow on device for the relevant detection. |
| SmartScreen block | A website or downloaded application was stopped because its reputation or content raised a warning. | Recheck the publisher and download source rather than treating the warning as an ordinary quarantine. |
| Controlled Folder Access block | An application was prevented from changing a protected folder such as Documents, Pictures, or Desktop. | Allow the verified executable through Controlled Folder Access, not through a broad antivirus exclusion. |
Protection History retains events for two weeks, according to Microsoft Support. Record the detection name and path promptly if you may need to investigate a false positive or report it.
How do I restore a file that Windows Defender quarantined?
Use the Windows Security interface to inspect the exact detection and restore it only when you trust the file. Restoration returns the item to its original location; Defender can detect it again immediately.

- Open Windows Security.
- Select Virus & threat protection.
- Under Current threats, select Protection history.
- Filter for Quarantined items, open the detection, and review its name, path, and action. Windows may require administrator privileges to show all details.
- Check the publisher, where you obtained the file, and whether its integrity is trustworthy. If those checks pass, select Restore.
- If Defender immediately finds it again, open the new Threat found – action needed card. Select Allow on device only after repeating the safety check.
A remediation can require a restart. If Windows Security requests one, reboot before testing the restored file; Microsoft notes that remediation may remain incomplete until the restart is finished.
When should you not choose Restore?
- The publisher is unknown or does not match the software you intended to download.
- The download came from an untrusted mirror, unsolicited message, or unexpected pop-up.
- The detection is for a password stealer, ransomware, Trojan, or another behavior that would expose your accounts or files.
- You cannot verify that the file has not been modified since publication.
In those cases, leave the item quarantined or removed and obtain a clean copy from the developer’s official distribution channel. A familiar filename is not proof of safety.

Restore and Allow on device do different jobs. Restore returns an item that is currently quarantined. Allow changes how Defender handles a future detection; it does not recreate a file that a Threat blocked event already removed.
For a blocked item, first establish that the software is legitimate, then download it again from the verified source. When Defender presents the new detection, open its action card and choose Allow on device only if the publisher, source, and file integrity still check out. If the file is quarantined rather than removed, use Restore instead.

How do I exclude one program without turning off antivirus?
Use the smallest exclusion that addresses the specific failure. Windows Security offers exclusions for an individual file, a folder, a file type, or a process. Microsoft describes every exclusion as a protection gap, so remove it when the task is complete.
| Exclusion scope | Use it when | Risk-control practice |
|---|---|---|
| Single file | One verified executable or data file is repeatedly misidentified. | Enter only that file’s full path and keep the file in a dedicated location. |
| Folder | A trusted application creates or updates several files in one controlled working directory. | Exclude a dedicated folder, not a whole user profile or system directory. |
| File type | Every file with a particular extension is trusted in your narrowly defined workflow. | Use sparingly; it can cover future files with the same extension, including unsafe ones. |
| Process | One known application needs Defender to stop inspecting files it opens. | Specify the process’s fully qualified path so another program with the same name is not covered. |
To add an exclusion, open Windows Security → Virus & threat protection → Manage settings → Exclusions → Add or remove exclusions, choose the required scope, and enter the exact path or process. Labels can vary slightly by Windows release, but the exclusions page is under Microsoft Defender’s virus-and-threat settings.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteThese exclusions apply to Microsoft Defender Antivirus real-time scanning. Scheduled scans and third-party antimalware products may still inspect the item, so an exclusion is not a universal “never scan this” command.
How do I remove the exclusion afterward?
Return to Windows Security → Virus & threat protection → Manage settings → Exclusions, select the entry, and choose Remove. Removing a temporary exception closes the protection gap once the software has been installed, updated, or repaired.

What if Controlled Folder Access is blocking the program?
If an application can open a document but cannot save, modify, or delete files in Documents, Pictures, Desktop, or another protected location, the problem may be Controlled Folder Access rather than a quarantined executable.
- Open Windows Security.
- Go to Virus & threat protection → Ransomware protection → Manage ransomware protection.
- Check Controlled folder access and the blocked-app notifications.
- Choose the option to allow an app through Controlled Folder Access.
- Browse to the exact, verified executable that needs access and add only that application.
- Retry the save or update operation, then remove the allowance if the program no longer needs protected-folder access.
Controlled Folder Access is designed to let trusted applications work while blocking unknown or untrusted programs from changing protected folders. Do not solve this symptom by excluding the entire Documents folder. On an organization-managed computer, an administrator may have to add the allowed application or protected folder through Defender policy.
Quick wins for a faster PC:
Scan for outdated or missing drivers - takes under a minuteDriver Scan →Repair Windows errors before they cause bigger problemsFix Now →Is a download warning caused by SmartScreen or antivirus?
Microsoft Defender SmartScreen evaluates websites and downloaded applications for phishing, malware, and potentially unsafe files. Its warning is a reputation or download-protection event and is not necessarily the same as an antivirus quarantine.
Protection History can also record potentially unwanted applications (PUAs) and SmartScreen blocks. Treat an unfamiliar publisher or installer as unsafe until you can verify who produced it, where it came from, and what it is supposed to do. An antivirus exclusion is not a substitute for that verification, especially when the warning appears before the file has been executed.

Can I restore a quarantined file from Command Prompt?
Microsoft documents a command-line method for an elevated Command Prompt. You must first change to the current Microsoft Defender platform directory, then run the utility with administrator rights.
- Open Command Prompt as administrator.
- Change to the current Defender platform directory used by your Windows installation.
- List quarantined items with:
MpCmdRun.exe -Restore -ListAll
- After identifying the exact item name, restore it with:
MpCmdRun.exe -Restore -Name <filename>
Use the command only after independently checking the file. A successful restore does not prove that the detection was wrong; Defender may identify the file again, requiring you to review the new action card.
What should I do if Protection History has no useful entry?
Check the event category rather than assuming deletion. A two-week retention window means an older event may no longer appear. For a current incident, look for separate cards describing quarantine, a blocked threat, a SmartScreen action, or ransomware protection. Record the exact detection name and path while the entry is available.
If the application is failing to save files rather than losing an executable, inspect Controlled Folder Access. If a download is stopped before it reaches the disk, inspect SmartScreen and the download warning instead of adding a Defender exclusion.

What should you never do to make one installer work?
- Do not broadly disable real-time protection as the routine fix. Microsoft warns that this leaves the device vulnerable, and Defender can turn itself back on when no other active security product is present.
- Do not exclude the entire Downloads, Users, Windows, or another system directory to accommodate one file.
- Do not approve an unfamiliar publisher merely because the program is urgent or needed for a game.
- Do not assume that an exclusion covers scheduled scans or security software supplied by another vendor.
- Do not use a contextual exclusion as a permanent answer to a suspected false positive.
For a legitimate file that continues to be detected, submit the sample to Microsoft Security Intelligence, or to the Microsoft Defender portal where that service is available, for analysis. That creates a path to correcting a false positive without weakening protection for an entire location or file class.
Which fix should you choose?
- Quarantined file: inspect the event and use Restore only after verifying the publisher, source, and integrity.
- Blocked file: verify it, download it again, and use Allow on device for the next detection only when justified.
- One trusted workflow: add the narrowest file, folder, file-type, or fully qualified process exclusion, then remove it when finished.
- Save or modify failure in a protected folder: allow the verified application through Controlled Folder Access.
- Download reputation warning: investigate SmartScreen and the publisher rather than treating it as a standard antivirus quarantine.
- Persistent suspected false positive: submit the file for Microsoft analysis instead of disabling protection broadly.
This approach preserves Defender’s protections while addressing the specific action that interrupted your file or application.
Frequently Asked Questions
Can Windows Defender delete a file without showing a warning?
It can remove or quarantine an item while Windows is running, with the action recorded in Protection history rather than displayed as a separate desktop prompt. Check that history as soon as you notice the missing file because entries are retained for only two weeks.
Why does Windows Defender turn real-time protection back on after I disable it?
Microsoft says Defender may re-enable itself when no other active security product is present. That behavior is intended to prevent the computer from remaining unprotected, so a narrow, temporary exception is safer than relying on a disabled-antivirus state.
Can an organization prevent me from restoring or allowing a file?
Yes. On a managed device, Defender policy can control allowed applications, protected folders, and other settings. If the relevant control is unavailable or immediately reversed, contact your administrator with the detection name and file path instead of repeatedly changing local settings.
What information should I include when reporting a false positive?
Keep the exact detection name, original file path, publisher information, download source, and whether the event was quarantine, blocking, SmartScreen, or Controlled Folder Access. Those details distinguish a misclassified file from an application that lacks permission to change a protected folder.
