These symptoms have many possible causes, and some people with lung cancer have no symptoms at all; a doctor must interpret the pattern and order the appropriate tests.
The National Cancer Institute (NCI) advises that symptoms lasting “for a couple of weeks” warrant medical attention. That is a prompt to arrange care, not a diagnostic cutoff. Severe breathing difficulty, substantial bleeding, fainting, or rapidly worsening illness requires urgent medical help.

What are the first signs of lung cancer?
Lung cancer can cause respiratory symptoms, whole-body symptoms, or no noticeable symptoms. Early disease is sometimes found incidentally when imaging is performed for another reason. A symptom by itself cannot establish cancer: infections, asthma, chronic lung disease, reflux, heart problems, medication effects and other conditions can produce similar complaints. As the NCI puts it, “Only a doctor can tell if your symptoms are caused by cancer.”
Respiratory and chest symptoms
- A cough that does not go away or becomes worse. A new cough, a change in a long-standing cough, or a cough that keeps returning deserves attention when it persists.
- Chest discomfort or pain. Pain can be felt in the chest and may reflect disease affecting nearby tissues; its character and location are not specific enough to diagnose a cause.
- Shortness of breath or trouble breathing. Breathlessness can occur with many lung and non-lung conditions, so worsening or persistent symptoms should be examined.
- Wheezing. A new or unexplained whistling sound when breathing is another reason to seek assessment.
- Blood in sputum (hemoptysis). Even a small amount should be reported to a clinician; substantial bleeding is an urgent situation.
- Hoarseness. Persistent voice change can occur when nearby structures are affected and should not be ignored.
- Trouble swallowing. Difficulty swallowing may result when a tumor or associated changes affect nearby structures, but it also has many other causes.
- Swelling in the face or neck, or prominent neck veins. This can reflect compression of nearby vessels and needs medical evaluation.
Whole-body symptoms
- Unexplained weight loss or reduced appetite. Weight loss that is not intentional is more concerning when it accompanies respiratory symptoms.
- Fatigue or a general feeling of illness. Malaise and persistent tiredness are nonspecific but matter when they are new, progressive or combined with other changes.
Symptoms may result from a tumor growing into local tissue or compressing nearby structures, as described in the NCI clinician guidance. The same symptom list can occur with benign disease, so the useful action is evaluation rather than self-diagnosis.

How long should a cough last before you see a doctor?
The NCI’s general advice is straightforward: “If you have symptoms that last for a couple of weeks, it is important to see a doctor.” Apply that guidance to a cough that remains present, keeps returning, or is worsening instead of improving. Do not wait for a specific number of days if the cough is accompanied by blood, chest pain, increasing breathlessness, fainting, or rapid deterioration.
A cough that crosses the “couple of weeks” point is not proof of lung cancer. It is a practical trigger to contact a clinician, who can review your history, examine you and decide whether testing is needed. Mention when the cough began, whether it is dry or produces sputum, any blood, changes in breathing, pain, fever or weight, smoking or other exposure history, and medicines you take. Those details help the clinician distinguish a likely short-term illness from a problem requiring imaging or further investigation.
When symptoms require urgent help
Seek urgent medical care for severe difficulty breathing, substantial bleeding, fainting, or illness that is worsening rapidly. The NCI symptom pages do not provide a complete emergency-triage protocol, so use local emergency services when a situation feels immediately dangerous rather than waiting for a routine appointment.
Can you have lung cancer without symptoms?
Yes. Lung cancer may be asymptomatic, particularly when it is small, and can be discovered incidentally on an image obtained for another reason. The absence of a cough or pain therefore does not rule it out. Conversely, having one of the listed symptoms does not mean cancer is present.

For people without symptoms who are at high risk, screening is a separate question from diagnostic testing. In the NCI clinician guidance, low-dose helical computed tomography (CT) is the only screening method shown to change lung-cancer mortality. Chest radiography and sputum cytology have not shown that mortality benefit. Whether screening is appropriate depends on individual risk and should be decided with a clinician through shared decision-making; the evidence does not support ordering a screening scan for everyone.
How lung cancer is diagnosed
Diagnosis is a sequence of steps, not a single scan or blood test. The NCI states, “There is no single test that can diagnose cancer.” Doctors combine clinical information, imaging and, when indicated, tissue sampling.
1. History and physical examination
The clinician asks about the symptom timeline, severity and progression; smoking and other exposures; past lung disease; family history; medicines; and general health. A physical examination may look for abnormal breathing, signs of infection, enlarged nodes or swelling. These findings help determine which tests are sensible and how urgently they are needed.

2. Laboratory tests
Blood and other laboratory tests can assess overall health and help investigate alternative explanations or prepare for procedures. They do not, by themselves, establish that a lung tumor is cancer.

3. Imaging
A chest X-ray or CT can show an abnormal area, a mass, enlarged lymph nodes or changes elsewhere in the chest. Imaging may also be performed for another complaint and reveal a lesion unexpectedly. A scan can suggest cancer and help map its extent, but imaging alone usually cannot provide the definitive diagnosis.
4. Biopsy or other sampling
A biopsy is often required to establish the diagnosis. Depending on where the abnormality lies, tissue or cells may be obtained through a procedure directed at the lung, airway or another accessible site. The pathology report identifies whether cancer is present and helps classify it. The safest and most informative sampling method depends on the imaging findings, suspected location and the person’s health.
5. Additional tests for treatment planning
Once cancer is confirmed or strongly suspected, doctors may order tests to determine whether it is confined to the lung, involves nearby lymph nodes or structures, or has spread elsewhere. For non-small-cell lung cancer, molecular findings can influence treatment selection, so the pathology work-up may include testing for relevant alterations. The exact panel and procedures vary by case.
What do lung cancer stages mean?
Stage describes how far the cancer has extended at the time it is assessed. The TNM system records three dimensions: the primary tumor (T), involvement of regional lymph nodes (N), and distant metastasis (M). Those findings are combined into broad groups from stage 0 through stage IV. Stage is not a symptom score and does not by itself predict an individual outcome.

Stage does not get “reset”
The stage assigned at diagnosis remains the original stage label even if the disease later changes. If cancer progresses or recurs, clinicians add information describing that change rather than replacing the original diagnostic stage with a new one.

Doctors broadly classify lung cancer as non-small-cell lung cancer (NSCLC) or small-cell lung cancer (SCLC). The distinction matters because the diseases behave differently and the treatment approach is selected accordingly. Pathology identifies the type, while stage, molecular findings, whether the tumor can be removed, and overall health also shape decisions.
Non-small-cell lung cancer
NSCLC includes several histologic subtypes. In selected early-stage cases, surgery or focused radiation may be considered. Stage II and III treatment often combines modalities, such as surgery, radiation and drug therapy, according to resectability and other findings. For stage IV disease, treatment commonly relies on systemic therapy chosen from the tumor’s histology and molecular results, with symptom control included in the plan.

SCLC is managed with a different treatment strategy from NSCLC. The appropriate combination of drug treatment, radiation and other measures depends on the extent of disease and the person’s condition. A pathology diagnosis and staging work-up are therefore essential before discussing a treatment plan.

How stage and test results guide treatment
Treatment cannot be selected from a symptom list alone. Clinicians consider the cancer type, TNM stage, molecular findings, whether it can be removed safely, previous treatments, lung function and overall health. NCI lists surgery, radiation therapy, chemotherapy, targeted therapy and immunotherapy among lung-cancer treatments.
- Localized disease: Surgery or precisely delivered radiation may be options for selected people with early-stage NSCLC.
- Regional disease: Stage II and III care frequently combines treatment types, with sequencing determined by anatomy, resectability and tumor characteristics.
- Distant disease: Stage IV care commonly uses systemic therapy selected by histology and molecular findings. Doctors also address symptoms and quality of life alongside tumor control.
These are treatment contexts, not promises about cure or an individual prognosis. A multidisciplinary team may revise the plan as pathology, scans and the person’s response become available.
What to bring to a medical appointment
Preparing information can make the evaluation more efficient. Bring a timeline of symptoms, a list of medicines and supplements, prior imaging or reports if available, relevant family history, and details of smoking or occupational exposures. Write down whether symptoms are stable, improving or worsening and whether they limit activity, eating or sleep.

Useful questions include: What conditions besides cancer could explain these symptoms? Which test comes next and what will it show? If imaging finds a lesion, will a biopsy be needed? What type of sample is safest? If cancer is confirmed, which tests establish stage and molecular characteristics? Ask when and how results will be communicated so that abnormal findings do not go unaddressed.
Common pitfalls to avoid
- Waiting for certainty from symptoms. No symptom pattern can confirm or exclude lung cancer; persistent or worsening changes need professional assessment.
- Treating a screening scan as a diagnosis. A low-dose CT can find an abnormality in a high-risk person, but additional evaluation, often including biopsy, is needed to determine what it is.
- Assuming a normal chest X-ray ends the work-up. The appropriate next step depends on the clinical picture; one test does not answer every question.
- Using home devices or supplements as substitutes for care. A pulse oximeter, supplement, air purifier or symptom checker cannot diagnose or stage lung cancer and should not delay a clinician visit.
- Comparing your stage with someone else’s. TNM details, tumor biology, resectability and overall health can differ substantially even when two people share a broad stage label.
When to make the call
Arrange a medical assessment when a cough or other concerning symptom lasts around a couple of weeks, returns repeatedly, or worsens. Contact urgent services for severe breathing difficulty, substantial bleeding, fainting or rapidly worsening illness. A clinician can determine whether the cause is cancer, another lung condition or a problem outside the lungs, then choose the appropriate tests rather than relying on a single symptom or scan.
