How the campaign is structured
You play as Enzo Favara, a former forced laborer in Sicily’s sulfur mines who joins Don Torrisi’s crime family. The base campaign is a cinematic, largely linear story, not an open-world game with freely selectable mission order. Some missions offer route choices, but the story determines the broader sequence.

Choosing stealth or combat
Stealth is a viable approach in most encounters, but it is not always optional: some story sections require it. When a mission permits a choice, use cover and Instinct mode to spot enemies through environmental obstacles before deciding whether to sneak past them or fight.

When a fight breaks out
Combat combines firearms, melee attacks, and one-on-one knife duels. Pay attention to the environment: throwable or breakable lanterns can ignite enemies. Cox’s gameplay overview demonstrates these combat elements and Instinct mode in the official gameplay feature.

Difficulty and accessibility settings

The Community Team’s Q&A lists Easy, Medium, and Hard, customizable aim assist, simplified struggle controls, subtitle controls, and vehicle and navigation options.

Exploration and collectibles
The launch-version Community Team Q&A says an Explore option becomes available after the Prologue. It lets you revisit the world for collectibles, spend Dinari, and ride vehicles and horses. That does not establish that every story item can be recovered there, so do not assume an item is safe to leave behind if you want a complete collection.
Collectible types
The Q&A lists Notes, Newspapers, Saints’ Cards, photographs, Trinacria, Charms, and a mysterious hidden collectible. It does not provide a verified total or a complete location-by-location route.

Man of Honor expansion walkthrough scope
Man of Honor is expansion content, separate from the base campaign. Launched August 14, 2026 for PS5, Xbox Series X|S, and PC via Steam, it adds two story chapters set in winter 1905 Sicily, where Enzo works alongside Ennio Salieri. It also includes Free Ride challenges and jobs built around racing, combat, and stealth. Details are in 2K’s expansion announcement.
