If IIS reports that php5isapi.dll is missing or cannot load, identify whether the server uses an old ISAPI configuration or should be using FastCGI, then repair that PHP installation.
What this file actually does
php5isapi.dll is the PHP 5 ISAPI SAPI module. Its internal description is ISAPI, and it was published by The PHP Group for PHP on Windows.

ISAPI was an older way to connect PHP with IIS and other ISAPI-compatible web servers. Historical PHP documentation identified the file as:

The file belongs with the PHP installation. It is not:

- A Microsoft runtime DLL
- A graphics or game file
- A COM library that should be registered with
regsvr32 - A file that belongs in
C:WindowsSystem32 - A file that belongs in
C:WindowsSysWOW64
The important complication is that PHP Windows packages changed over time. Some older PHP 5 packages included an ISAPI module, while later PHP 5 distributions, particularly packages from the PHP 5.3 era, commonly moved IIS support toward FastCGI and did not include php5isapi.dll.
That means a missing file does not necessarily indicate corruption. An old IIS tutorial or handler mapping may simply be requesting a file that was never included in the PHP package you installed.
PHP 5 is obsolete and unsupported. If the application can be updated, migrating to a supported PHP release is safer than rebuilding an old ISAPI deployment.

Fix 1: Replace the old ISAPI mapping with FastCGI
This is the safest and most likely solution for an existing IIS installation.
PHP’s recommended IIS integration uses FastCGI and php-cgi.exe, not php5isapi.dll. FastCGI is also the practical choice when the PHP package does not contain the historical ISAPI module.
1. Find the PHP installation you intend to use
Look for the directory containing:
php-cgi.exe
php.exe
php.ini-development
php.ini-production
ext
Do not assume that the PHP directory named in an old handler mapping is still valid. Common problems include a deleted directory, an old drive letter, or multiple PHP installations with different versions.
Keep one complete PHP installation together. Do not combine php-cgi.exe, extensions, php.ini, and DLLs from separate PHP archives.

2. Confirm that IIS has the CGI feature
- Press
Windowsand search for Turn Windows features on or off. - Open it as administrator.
- Expand Internet Information Services.
- Expand World Wide Web Services.
- Expand Application Development Features.
- Select CGI.
- Select OK and allow Windows to install the feature.
- Restart IIS if Windows requests it.
The CGI feature is required for IIS FastCGI support. If IIS Manager does not show FastCGI-related options after installation, restart the server or open IIS Manager again.
3. Remove the obsolete PHP ISAPI mapping
- Open IIS Manager.
- Expand Sites.
- Select the affected website.
- Open Handler Mappings.
- Find any mapping whose executable points to:

php5isapi.dll - Record the old path if you may need to document the change.
- Remove the obsolete mapping, or disable it while testing.
Do not leave a broken php5isapi.dll mapping above the new FastCGI mapping. IIS may continue trying to use the old handler.
4. Add the FastCGI handler
- In Handler Mappings, select Add Module Mapping.
- Enter:
- Request path:
*.php - Module:
FastCgiModule - Executable: the full path to your matching
php-cgi.exe, such as:

PHP_via_FastCGI - Select Request Restrictions if shown.
- Confirm that the mapping applies to the required request types.
- Select OK.
- Accept the prompt to create the FastCGI application if IIS displays one.
The executable in the handler mapping and the FastCGI application must point to the same php-cgi.exe.
5. Recycle the application pool
- In IIS Manager, select Application Pools.
- Identify the pool used by the PHP site.
- Right-click it.
- Select Recycle.
If the site still returns an error, restart IIS from an elevated Command Prompt:

Use iisreset during a suitable maintenance window because it affects IIS sites on the server.

6. Test PHP without exposing sensitive settings
Create a temporary file in the website directory named:
php-test.php
Use:

echo 'PHP FastCGI is responding';
Browse to that file through the site. A successful response confirms that IIS is reaching PHP through FastCGI.
Remove the test file immediately after testing. Do not leave a diagnostic page containing PHP configuration details publicly accessible.
Command-line alternative
The PHP IIS example uses commands similar to these. Replace the path if your PHP directory is different:
%windir%system32inetsrvappcmd set config /section:system.webServer/fastCGI /+[fullPath='C:PHPphp-cgi.exe']

Run Command Prompt as administrator. If AppCmd reports that the entry already exists, inspect the current IIS configuration instead of adding duplicate entries.
Fix 2: Check the PHP package before replacing anything
A missing php5isapi.dll can be expected.
- Open the PHP directory in File Explorer.
- Search that directory and its subdirectories for:

php5isapi.dll - Check the archive or installation package you originally used.
- Determine whether it was an older PHP 5 package intended for ISAPI or a later package intended for FastCGI.
If the package does not contain the file, do not download an isolated copy. Use php-cgi.exe and FastCGI, or obtain the complete matching historical PHP archive from the official PHP Windows archive.
If the application absolutely requires the historical ISAPI interface, recover the complete PHP release that originally supported it. The DLL must match the rest of that release, including its PHP runtime, extensions, configuration expectations, and architecture.

Fix 3: Check the IIS path and all PHP environment settings
A stale path is common after a PHP upgrade or disk migration.
Check these locations:
- In IIS Manager, open the site’s Handler Mappings.
- Inspect the executable path for every PHP mapping.
- Open FastCGI Settings at the server level.
- Confirm that the
fullPathvalue points to the samephp-cgi.exe. - Check the site’s Basic Settings and confirm that the physical path is correct.
- Check system and user environment variables for old PHP directories.
- Look for
PHPRCor extension-directory settings that refer to a removed installation. - Confirm that the
php.inibeing used belongs to the selected PHP directory.
After correcting the paths:
- Recycle the application pool.
- Run:
bat
iisreset - Test a simple PHP page.
If one site works and another fails, compare their handler mappings, application pools, physical paths, and web.config files. Do not assume the server-wide PHP configuration applies identically to every site.
Fix 4: Match 32-bit and 64-bit architecture
A PHP binary must match the IIS worker process that loads it. Historical PHP 5 ISAPI deployments were commonly 32-bit, while a 64-bit IIS application pool normally uses a 64-bit worker process.
Check the application-pool setting:
- Open IIS Manager.
- Select Application Pools.
- Right-click the pool used by the site.
- Select Advanced Settings.
- Find Enable 32-Bit Applications.
- Set it to True only if you are deliberately using a 32-bit PHP installation.
- Set it to False for a 64-bit PHP installation intended for a 64-bit worker process.
- Select OK.
- Recycle the pool.
Do not change this setting blindly. Confirm whether the PHP build is 32-bit or 64-bit from the official package information or the installation records.

For FastCGI, select a PHP build whose architecture matches the intended IIS process. Avoid placing a 32-bit PHP executable beside 64-bit extensions or files from another installation.
A bitness problem can produce loader errors that look like a missing or damaged DLL. If the message changes after correcting the pool setting, the original problem was likely an architecture mismatch.
Fix 5: Check thread-safety and PHP build consistency
For IIS FastCGI, PHP documentation recommends the non-thread-safe build. A thread-safe build is associated with other hosting models and should not be mixed casually with a FastCGI setup.
Check that these items come from the same PHP release:
php-cgi.exe- PHP extensions in
ext php.ini- Runtime DLLs
- The selected architecture
- Any application-specific PHP extensions
Do not copy a php.ini from one PHP release and assume every extension setting remains valid. Preserve a backup of the old configuration, then create a clean configuration for the selected installation and add only settings the application requires.
Fix 6: Reinstall PHP from a complete official archive
Use this fix when extraction was incomplete, the file was deleted, or several PHP DLLs appear to be missing.
- Back up the current
php.ini. - Record the PHP directory, IIS mappings, site configuration, and required extensions.
- Stop or place the affected site into maintenance mode.
- Obtain the complete PHP archive from the official PHP distribution source.
- Extract it into a clean directory.
- Do not extract over a damaged installation.
- Copy configuration settings manually after reviewing them.
- Recreate the IIS FastCGI mapping to the new
php-cgi.exe. - Recycle the application pool.
- Test the application and review its logs.
If the application needs an old PHP version, test the recovered installation in a controlled environment first. PHP 5 applications may depend on functions or extensions that are unavailable in current PHP releases, so migration may require application changes.
Fix 7: Check antivirus quarantine
If php5isapi.dll existed and then disappeared, inspect the security product’s history.
- Open Windows Security.
- Select Virus & threat protection.
- Select Protection history.
- Look for an event involving the PHP directory or the missing file.
- Confirm that the file came from a complete official PHP archive.
- Do not restore a file merely because its name matches.
- If the installation is trusted, restore it according to your organization’s security policy.
- Reinstall the complete PHP archive if there is any doubt.
- Add an exclusion only after verifying the installation and obtaining approval under your security policy.
There is no reason to assume that every missing php5isapi.dll report is an antivirus false positive. Treat quarantine as one possible explanation, not as proof that the file is safe.
Fix 8: Check dependencies instead of replacing the named DLL
“The specified module could not be found” can mean that Windows cannot load a dependency required by the named PHP module. The top-level file may exist while another required DLL is missing.
Check:
- The PHP directory for incomplete extraction.
- IIS logs for the failing request.
- Event Viewer:
- Right-click Start.
- Select Event Viewer.
- Check Windows Logs and relevant IIS or application entries.
- PHP startup output or application logs.
- The exact PHP build and architecture.
- Whether extensions were copied from another installation.
Do not install random Visual C++ packages until you know which dependency is missing. A runtime package that does not match the PHP build will not repair an inconsistent PHP directory.
Fix 9: Repair Windows components only when Windows itself is damaged
php5isapi.dll is a third-party PHP file. System File Checker and DISM do not normally restore it.

Use Windows repair commands only when you also have evidence of damaged Windows components, failed servicing, or broader system-file problems.
Open Windows Terminal (Admin) or Command Prompt (Admin) and run:
DISM.exe /Online /Cleanup-Image /RestoreHealth
Wait for it to complete, then run:
sfc /scannow
Restart Windows and test IIS again.
A successful SFC or DISM repair does not prove that PHP was repaired. If the PHP file is still missing, reinstall or re-extract the complete PHP package and rebuild the IIS mapping.

Fix 10: Check drivers only when the wider Windows problem points there
A PHP IIS error normally has nothing to do with a device driver. Consider driver maintenance only if the same machine also shows graphics, storage, chipset, or other device failures.
To check manually:
- Right-click Start.
- Select Device Manager.
- Expand categories with warning icons.
- Right-click the affected device.
- Select Properties.
- Review the General and Driver tabs.
- Install drivers from the hardware manufacturer or your organization’s approved source.
- Restart Windows if required.
Do not use Device Manager to search for php5isapi.dll; it is not a hardware driver.

Do not download an isolated DLL
Random DLL download sites may provide:
- The wrong PHP release
- The wrong architecture
- A modified or malicious file
- A file with missing dependencies
- A file that does not match
php-cgi.exe, extensions, orphp.ini
Use the complete PHP archive from the official PHP source instead. If the application vendor supplied PHP as part of its product, use that vendor’s complete installer or repair package.
Do not copy it into System32 or SysWOW64
IIS must load PHP through its configured handler. Copying the file into a Windows directory does not correct the handler path, architecture, dependencies, or PHP version.
It can also make later troubleshooting harder by leaving an untracked copy that looks valid but is not used by the intended site.
Do not use regsvr32
regsvr32 is intended for DLLs that expose registration entry points such as DllRegisterServer. php5isapi.dll is an IIS PHP server module, not generally a COM component.

regsvr32 php5isapi.dll
An error from regsvr32 does not diagnose the PHP installation. Configure the module through IIS, or replace it with a FastCGI mapping.
Final escalation checklist
Before escalating the issue, record:
- Windows version and system architecture
- PHP version and complete package source
- Whether the PHP build is 32-bit or 64-bit
- Whether it is thread-safe or non-thread-safe
- The exact IIS handler mapping
- The application-pool bitness setting
- The full path to
php-cgi.exe - Whether
php5isapi.dllwas ever present - The complete error text
- IIS and Windows Event Viewer entries
- Antivirus quarantine history
- The result of a clean PHP extraction
If the site is configured for ISAPI but the installed PHP package does not contain php5isapi.dll, stop trying to repair the missing file and migrate the handler to FastCGI. If the application cannot yet migrate, recover the complete matching historical PHP installation rather than downloading a replacement DLL.
