qtaccessiblewidgets4.dll is a Qt 4 accessibility plugin, not a Windows system file or Microsoft Visual C++ component. If Windows says it is missing or cannot be loaded, repair the application that installed it rather than copying a random DLL into a system folder.
What this file actually does
Qt applications use plugins to add features outside the main executable. qtaccessiblewidgets4.dll provides accessibility support for Qt widgets, such as controls that assistive technologies can inspect or interact with.
It normally belongs to the application’s private Qt runtime in one of these locations:
<Application>pluginsaccessibleqtaccessiblewidgets4.dll
<Application>accessibleqtaccessiblewidgets4.dll
The exact layout depends on how the developer packaged Qt. It does not normally belong in either of these Windows folders:

Do not treat the filename as proof that every copy comes from one publisher. Qt-based applications and games can redistribute different Qt builds, and a DLL copied from another program may have the wrong architecture, compiler runtime, or Qt build.
This file is not:
- A Windows component
- A DirectX file
- A .NET file
- A device driver
- A Visual C++ Redistributable file
- A normal COM or ActiveX library
That is why regsvr32 is generally not the answer. Qt finds this plugin through its directory structure and plugin loader. Registering it will not recreate the application’s missing pluginsaccessible path.
Match the symptom to the likely cause

Error wording varies between Windows versions and application launchers. “The specified module could not be found” can refer to a dependency of qtaccessiblewidgets4.dll, not necessarily the file itself.
Fix qtaccessiblewidgets4.dll in order
1. Identify the application that needs the file
Start with the program named in the error window. If the message does not identify it, use the shortcut or launcher that you clicked.

- Right-click the program’s shortcut.
- Select Properties.
- Open the Shortcut tab.
- Read the Target field.
- Select Open File Location if available.
You can also check running and startup references:
- Press Ctrl + Shift + Esc to open Task Manager.
- Select Startup apps.
- Look for the affected application or an old launcher.
- For more detail, open Settings > System > About only to confirm the Windows installation; Windows bitness does not tell you whether the application is 32-bit or 64-bit.
If the error appears during startup:
- Press Win + R.
- Enter
eventvwr.msc. - Press Enter.
- Open Windows Logs > Application.
- Look for an error at the same time as the popup.
- Check the application name and executable path in the event details.
Do not begin by downloading the DLL. First determine which program owns it.
2. Check the application’s Qt plugin folder
Open the application’s installation directory and look for either of these structures:
pluginsaccessibleqtaccessiblewidgets4.dll
accessibleqtaccessiblewidgets4.dll
To check the usual location:
- Right-click the application shortcut.
- Choose Properties.
- Select Open File Location.
- Search the folder for
qtaccessiblewidgets4.dll. - If it is present, confirm that it is inside the application’s expected Qt plugin structure.
If the file is in a temporary folder, Downloads, System32, SysWOW64, or an unrelated application directory, do not use it as a replacement. Its location is suspicious or incompatible with the program’s normal plugin search path.
If the entire accessible directory is absent, restore the application rather than creating the folder manually. The plugin may depend on other files from the same Qt build.

3. Use the launcher’s Verify, Repair, or Scan option
This is the safest fix for a game or application installed through a launcher.
Open the launcher and find the affected program in its library. Look for a menu named Verify integrity, Verify files, Repair, Scan and repair, or a similar option. Start the operation and wait for it to finish before launching the program again.
For an application installed with its own setup program:
- Press Win + I.
- Select Apps.
- Choose Installed apps on Windows 11, or Apps & features on Windows 10.
- Find the affected application.
- Select the three-dot menu on Windows 11, or select the application on Windows 10.
- Choose Advanced options if available.
- Select Repair.
- Test the program.
If there is no Repair option, open Control Panel > Programs > Programs and Features, select the application, and look for Change, Repair, or Modify.

4. Reinstall the affected application from an official source
Use this step when verification or Repair is unavailable, reports unresolved files, or leaves a bad-image error.
- Back up the application’s saved games or settings if they are stored outside a cloud service.
- Open Settings > Apps > Installed apps.
- Select the affected application.
- Choose Uninstall.
- Restart Windows.
- Download the installer from the application developer, publisher, or store where you originally obtained it.
- Install the application again.
- Launch it once before restoring optional modifications or third-party add-ons.
A clean reinstall replaces the complete private Qt runtime. It is safer than copying only qtaccessiblewidgets4.dll, because the application may require a specific Qt major version, matching libraries, and a compatible compiler runtime.

If the application is old or abandoned, use its original installer or a preserved package from the publisher or store. Do not assume that installing the Qt developer framework will repair a third-party program. A framework package is intended for development and may not match the runtime bundled by the application.
5. Check antivirus quarantine
Security software can remove or quarantine a plugin after an update or scan.
In Windows Security:
- Open Start > Windows Security.
- Select Virus & threat protection.
- Choose Protection history.
- Look for an entry involving the affected application or
qtaccessiblewidgets4.dll. - Expand the entry and inspect its original path and detection details.
Only restore the file if it clearly belonged to the trusted application installation and you recognize the publisher and location. Afterward, run the application’s Verify or Repair operation. A quarantine restore alone may leave other Qt files missing.
If security software repeatedly removes the file, do not add a broad exclusion for the entire drive. Update the application from its official source and investigate the detection with the security vendor.
6. Check architecture and dependencies
A 64-bit Windows installation can run 32-bit applications. Windows bitness therefore does not determine which copy of this plugin the program needs.

The application and its Qt plugin must match. Common failures include:
- A 32-bit application receiving a 64-bit replacement
- A 64-bit application receiving a 32-bit replacement
- A Qt 4 plugin copied from a different application
- A plugin built for a different compiler or private Qt runtime
- A dependent DLL missing beside the application executable
If Windows reports “The specified module could not be found,” the missing item may be a dependency rather than qtaccessiblewidgets4.dll itself. Repairing or reinstalling the complete application is the reliable response.
Do not copy a 32-bit DLL into System32 or a 64-bit DLL into SysWOW64. Those folders do not fix the application’s Qt plugin search path.
7. Install only runtimes the application actually requires
Do not install Visual C++ Redistributable packages merely because the missing item ends in .dll. Microsoft’s packages, such as vc_redist.x86.exe and vc_redist.x64.exe, provide Microsoft runtime libraries, not Qt accessibility plugins. If the application’s official documentation or installer specifically requires a Visual C++ package, obtain it from Microsoft’s latest supported Visual C++ Redistributable downloads.

Do not expect Windows Update, DirectX setup, or a device driver installation to normally recreate this Qt plugin.
8. Repair Windows components only when other system files also fail
Because this is normally a third-party application file, Windows component repair is a secondary step. Use it when several programs report corrupted system files, Windows features are failing, or the problem followed broader system corruption.
First open an elevated terminal:
- Open Start.
- Type
Command Prompt. - Right-click Command Prompt.
- Select Run as administrator.
- Select Yes at the User Account Control prompt.
- Run:
DISM.exe /Online /Cleanup-Image /RestoreHealth
Wait for it to complete. Then run:
sfc /scannow
Microsoft recommends running DISM before SFC. Restart Windows after both commands and test the application again. These commands repair protected Windows components; they will not normally recreate a missing Qt plugin inside a third-party application.

9. Update drivers only for broader graphics or device symptoms
A driver update is not a normal fix for a missing Qt accessibility plugin. Consider it only if the same application also crashes with graphics errors, several programs show display-related failures, or Device Manager reports a device problem.
To inspect drivers:
- Right-click Start.
- Select Device Manager.
- Expand the relevant category, such as Display adapters.
- Right-click the device.
- Choose Update driver.
- Select Search automatically for drivers.
You can also select Properties > Driver to review the installed driver or use Roll Back Driver if the issue began immediately after a driver change. 10. Do not use regsvr32 for this plugin
regsvr32 is intended for DLLs that expose registration entry points such as DllRegisterServer. Qt accessibility plugins are loaded by Qt from the application’s plugin directories.

Running a command such as this is not a normal repair:
regsvr32 qtaccessiblewidgets4.dll
It may produce a registration error or create the false impression that the application was repaired. The correct response is to restore the application’s original files and directory layout.

Downloading a standalone copy is risky because the file may have:
- The wrong 32-bit or 64-bit architecture
- An incompatible Qt build
- A different compiler runtime requirement
- Malware or tampered contents
- A filename that matches but does not belong to your application
It may also leave the dependent libraries missing. Never place an unverified copy in System32, SysWOW64, or a random application directory.
Use the application developer, publisher, official store, or a known-good backup as the source. If the application installer itself supplies the file, let the installer or launcher restore it.
If reinstalling does not work
Remove any manually copied replacement, restart Windows, and reinstall the complete application again. Confirm that the error points to the intended executable rather than an obsolete shortcut or startup task.
If every application reports bad-image errors, investigate broader causes such as malware, disk failure, damaged Windows servicing, or system-wide corruption. If only one old application fails, keep troubleshooting that application’s installer and private Qt runtime.
System Restore is a last resort:
- Press Win + R.
- Enter
rstrui.exe. - Press Enter.
- Choose a restore point from before the file disappeared.
- Review affected programs.
- Start the restore only if reversing those changes is acceptable.
FAQ
Is qtaccessiblewidgets4.dll a Windows file?
No. It is a Qt 4 accessibility plugin distributed with a third-party application.
Is it safe to delete?
Do not delete it from an installed application. Removing it can prevent that program from starting or loading its accessibility support.
Should I copy it to System32?
