Quick answer: Choose Drest if you want to keep his future quests available; choose Iona if you accept losing those quests in exchange for the Druid’s Talisman and her revenge. If Age of Excuse is unfinished, complete it before killing Drest. Neither choice is established as a universally correct moral answer.

Where the choice happens

During Black Tar Prophecies in the Horns of the South, Iona interrupts Drest’s cleansing ritual at the Ancient Cromlech. After hearing Iona out, you must decide which of them to support. This is a consequential branch, not a dialogue choice with a mechanically clean outcome.

What happens if you side with Drest?

Iona dies, while Drest survives and remains available for later Horns of the South quests. Choose this branch if retaining access to Drest’s future content matters more to you than Iona’s revenge or the loot available from Drest’s body.

What happens if you side with Iona?

Drest dies, and future quests that require him become unavailable. His body can be looted for the Druid’s Talisman. If you want that item, loot Drest before speaking to Iona.

Finish Age of Excuse first

If Age of Excuse is still active, do not kill Drest until you have completed it: the community wiki warns that killing him automatically fails the quest. The branch therefore closes more than his later quest availability if you leave this quest unfinished.

Which choice should you make?

Priority Better fit Tradeoff
Keep Drest’s future quests available Side with Drest Iona dies.
Get the Druid’s Talisman and support Iona’s revenge Side with Iona Drest dies, and his future quests are lost.
Preserve unfinished Age of Excuse Do not kill Drest yet Complete the quest before choosing the Iona branch.

For a first or single playthrough where preserving available quests is the priority, siding with Drest is the safer choice. If you prefer Iona’s revenge or want the talisman and are willing to close Drest’s questline, siding with Iona is a coherent role-playing choice. The available quest guides document the consequences, not a canonical moral verdict; choose based on the cost you are willing to accept.
