Keep moving, dodge his Great Knife attacks, and save ammunition by waiting out the timer. Shooting him can shorten the fight, but it is optional.
Where the first Pyramid Head fight happens
The encounter takes place at the end of the Blue Creek Apartments sequence. After solving the clock puzzle, enter the room behind the S-marked door. This guide covers that first timed fight, not the later encounter with two Pyramid Heads.

Choose whether to attack or evade

There is no established universal fight duration across difficulties. GamesRadar reported that, in two of its own runs on normal combat and puzzle difficulty, the encounter lasted a little over three minutes without attacking and under two minutes when it attacked. Those are outlet-specific observations, not guaranteed timings.
How to avoid Pyramid Head’s attacks
Keep a gap and stay mobile
Do not let the Great Knife’s reach surprise you. Keep moving around the arena and avoid stepping straight back into range after a dodge. The cages and debris may obstruct Pyramid Head briefly, but he can break through them, so they are not dependable cover.

Dodge according to the attack
- Horizontal swipe: move backward.
- Overhead swing: dodge sideways.
- Grab: move to the right. This is especially dangerous if it connects.
Use the flashlight and listen
Keep your flashlight trained on Pyramid Head so you can track him in the dark. If he disappears from view, listen for the Great Knife scraping along the floor to locate him and prepare to move.
Use the arena and recognize the ending cue
Resources are placed around the arena’s perimeter, so collect them when you have a safe opening rather than stopping near Pyramid Head. Pipes bursting and rubble falling are spectacle and progress cues, not attacks you need to counter. When the siren sounds, the encounter is ending: Pyramid Head moves toward the heavy doors and the exit sequence follows.

PlayStation’s official ending guide associates fleeing from Pyramid Head in this first fight with the conservative Leave route, while dealing him as much damage as possible fits the aggressive In Water route. These are route tendencies, not conditions for winning the encounter: survival is enough to finish the fight. The remake has eight endings, according to PlayStation.
