In The Witcher 3, toxicity is accumulated exposure, while the overdose threshold is the point at which penalties begin. Meditation replenishes known alchemy items when suitable strong alcohol is in your inventory. If a recipe seems missing, first determine whether Geralt knows its formula or simply lacks ingredients or a suitable base.
Why is my toxicity so high?
Potions add toxicity as you use them. The Witcher Wiki’s potions reference says vitality loss begins when toxicity reaches a critical level. The toxicity amount and the overdose threshold are different: toxicity is the exposure you have accumulated; the threshold is where overdose penalties apply.

Alchemy skills can raise that threshold. The Official Witcher Wiki’s Alchemy skill reference lists Heightened Tolerance as progressing from a 75% baseline to 80%, 85%, 90%, 95%, and 100% across its five ranks. Treat these as the reference’s listed progression, not a guarantee for every build or game version; the cited mechanics references are not current in-game testing.

Yes. The cited potions reference says meditation replenishes potion doses, automatically using hard alcohol from Geralt’s inventory. Prima Games’ alchemy and potion guide also describes decoctions being replenished during meditation with strong alcohols.

In practical terms, keep suitable strong alcohol available and meditate to restock alchemy items you have already learned to make. You do not need to gather each potion’s ingredients again for every refill: the cited references describe alcohol-based restocking.
Why can’t I see or craft an alchemy recipe?
- Find the exact formula name. Check the Journal’s Alchemy list; the formula reference describes known formulae as represented there.
- Check whether the formula is known. Some formulae are available from the start, while others must be acquired during play. An unknown formula is different from a known formula that cannot currently be prepared.
- If it is known, check ingredients and base. The potion preparation reference says a formula without its required ingredients is unavailable for selection. Check that you have the necessary ingredients and any required base.
- Pin the formula if your interface supports it. The formula reference says pinning can display ingredient needs in the shop panel, a feature it notes as dating from patch 1.07.
- If it is unknown, investigate how to acquire it. Formulae may be learned by reading, finding or buying them, getting information from NPCs, and in some cases experimenting. Check relevant herbalists and alchemists, loot, and NPC or quest information.
- Check whether it belongs to an expansion. CD Projekt Red identifies Hearts of Stone and Blood and Wine as major expansions. A formula associated with expansion content may depend on which expansion you own; the references here do not provide a complete formula inventory for either one.
Where can I find a missing formula?
Start with the exact name of the formula: vendor suggestions are useful only after you know which recipe you are hunting, and a location listed in a guide is not a guarantee for every edition or game build. Prima Games’ purchase appendix gives examples, including Black Blood at the Gedyneith druid, a Novigrad herb dealer, and the Gedyneith master alchemist, and Cursed Oil 1 with Keira Metz and a Skellige herbalist. These are examples, not an exhaustive or guaranteed location map.

If those leads do not fit, check likely alchemists and herbalists, loot, and relevant NPC or quest information. For a location-specific answer, the key details are the exact recipe name, platform and game build, and whether you have Hearts of Stone or Blood and Wine. The cited references are community-maintained wiki and guide material; CD Projekt Red’s official page establishes expansion identity but does not settle detailed formula locations or version-specific alchemy behavior.
