Its headline addition was Mortis Trials, a wave-based activity built around escalating objectives and temporary ability enhancements; it also refreshed the Havoc end-game mode and reworked Ogryn talents, weapons, and other systems.
What Nightmares & Visions added
Fatshark announced the update on March 5, 2025, and said it would be free. Its launch notes said the update began rolling out across platforms on March 25. Fatshark’s announcement framed the release around two main areas: Mortis Trials and a Havoc refresh, alongside broader gameplay changes.

A later Fatshark retrospective, published January 30, 2026, lists Nightmares & Visions among four major free updates released during 2025. That confirms the update as a completed release, but does not establish whether a limited-time event associated with it is active now. Fatshark’s 2025 wrap-up
How Mortis Trials work
Mortis Trials are visions prepared by Sefoni, the Psyker aboard the Mourningstar. Players fight successive enemy waves while choosing enhancements called Indulgences. Fatshark described successful runs as a way to uncover information about the ship’s characters.

Wave progression and objectives
The developer’s March 21 feature blog lays out the activity’s progression:
- At the start, players receive a small buff related to their archetype.
- Waves three, six, and nine are objective waves. Completing an objective grants supplies and a stronger buff to choose from; selections accumulate during the trial.
- After completing the wave-nine objective, the team must survive three additional waves.
The result is a run that combines ordinary wave survival with periodic objectives and increasingly powerful stacked effects. The exact Indulgences available can shape how a team approaches the rest of its trial. Read the Mortis Trials developer blog.

What changed in Havoc
Havoc is the end-game mode that received a separate refresh. Fatshark described the changes as a response to player feedback and highlighted increased difficulty, a reassessed ranking system, and four new mutators. The launch notes add more detail, including a ranking overhaul, campaign rotation, adjustments to horde pacing and composition, enemy-behavior changes, and quality-of-life updates.
Mortis Trials and Havoc therefore serve different purposes: Mortis Trials is a wave-based activity with objective breaks and buffs that build through a run; Havoc’s refresh changes the challenge and structure of the existing end-game mode. The update’s patch notes also described a Mortis Trials event with progress for consecutive waves and in-game rewards. Those notes document launch-era event details, not current availability. See the Nightmares & Visions patch notes.

Ogryn, weapons, and other adjustments
Beyond its new activity and Havoc changes, Nightmares & Visions included an Ogryn talent-tree rework, weapon tweaks, balance adjustments, and general improvements. The patch notes record detailed weapon changes and mention a Psyker Mettle talent issue at launch. They do not establish whether that issue remains in the current version, so it should be treated as a historical launch note rather than a statement about the game today.
