What changes if you spare every Cyberpsycho?
Regina’s concluding dialogue changes depending on whether V kept the targets alive, according to the community-maintained Psycho Killer quest reference. That is the confirmed difference for an all-alive run: the source does not establish a separate bonus item or payment for sparing everyone.

The same quest reference says the completion reward is still given if some targets are killed. In other words, keeping them alive affects the conversation, but the evidence does not indicate that killing a target forfeits the quest reward.
What reward does Psycho Killer give?
Psycho Killer is the overarching quest for Regina’s Cyberpsycho sightings. A GameFAQs walkthrough describes 17 targets and a concluding meeting with Regina after they have all been incapacitated.

CD Projekt Red’s official Patch 2.11 notes say, “Improved the reward for neutralizing all Cyberpsychos.” They also say players who had already claimed the reward can return to Regina and collect the new reward from a weapon crate. The notes do not name the item or say that it requires an all-alive run.
The walkthrough’s reward comment refers specifically to version 1.12, so it should not be treated as current reward guidance. The later official patch notes confirm the reward was improved but leave its exact contents unspecified.

How to keep a Cyberpsycho alive
In this quest, “alive” means incapacitated rather than finished off. The GameFAQs walkthrough lists non-lethal weapon modifications and fists as ways to take targets down without killing them.
- Use a non-lethal weapon modification or fists to reduce the target’s health.
- Stop attacking once the target is incapacitated; avoid follow-up attacks that could kill them.
- Complete the remaining Cyberpsycho sightings and report back to Regina to finish the quest.
Does sparing everyone unlock a better reward?
That is not established by the available sources. The quest reference connects sparing targets to Regina’s remarks, while CD Projekt Red’s patch notes connect the improved reward to neutralizing all Cyberpsychos. Neither source says an all-alive run unlocks an additional item, extra cash, or a higher reward tier.
